Prognostic value of plasma-cell immunophenotype in patients with multiple myeloma.

A review is given of the prognostic significance of immunophenotyping of blood lymphoplasmocytic cells. From a group of 250 patients followed from 1981 through 1991 a subgroup of 70 patients (followed 1986 through 1991) were phenotyped at 6-month intervals by immunofluorescence tests with monoclonal antibodies for cytoplasmic immunoglobulin, kappa-lambda index, CD71, CD10, CD20, CD38, and HLA-DR receptors. In course of a longitudinal study it was found that prognostic significance for shortened survival can be derived from the presence of circulating CD10, CD71, and CD20 positive undifferentiated cells in peripheral blood. There was a correlation between increase of CALLA positive and CD71 positive cells. Further, an increase of undifferentiated clone occurred during transition of the disease to an aggressive phase. The median survival of the total group of 250 patients treated by the VMCP/MOCCA protocol, according to statistical analysis, was 90 months, the median survival of the aggressive stage with plasmoblastic and lymphoplasmocytic cell type, respectively, was only 12 months. The significance of phenotypization in the prognostic evaluation of variant heterogenous myeloma types is stressed.

Does the moon influence the predatory activity of mites?

Periodicity in predatory activity was observed in overwintered females of Typhlodromus pyri Scheuten (Acari: Phytoseiidae). Chronobiometrical analysis revealed that an approximately circabiseptan (about-14-day) rhythm is statistically significant, with a striking depression around the full moon.

Transfer factor of 131I from the fallout to human thyroid dose equivalent after the Chernobyl accident.

A similar pattern of variation with time in observed maxima of daily dose equivalent rates in human thyroids (TD - microSv.d-1) and of daily fallout radioactivities (FR - kBq.m-2) has been found after the Chernobyl accident. An estimate of the time-lag between the maxima in TD lines and the preceding FR peaks was made of about seven days for adult and nine days for juveniles. Applying this time-lag it was possible to estimate transfer factors from the fallout to thyroid dose equivalent: the highest estimated values were 221 microSv/kBq.m-2 for adult and 641 microSv/kBq.m-2 for juvenile thyroids. These values differ from those published by UNSCEAR (United Nations 1988), which have been calculated for various regions of Czechoslovakia, from ingestion and inhalation intake estimates. A broad variation of transfer factor values could be expected to result from such transfer calculations using ingestion and inhalation estimates. The findings also support the concept of a need for prolonged iodine prophylaxy after emissions of radioiodine into the environment.

[Immunotyping of medullary and circulatory cells for prognostic evaluation of plasmacytoma].

The authors discuss the prognostic impact of immunophenotyping of circulating lymphoplasmatic cells in the peripheral blood stream in patients with generalized plasmocytoma. From a group of 250 patients followed up from 1981 to 1991 they selected a sub-group of 70 patients where they evaluated in 1986-1991 after six-month intervals the phenotype of medullary and circulating cells. They used the method of immunofluorescent detection of the presence of cytoplasmic Ig, the kappa-lambda index and phenotyping of antigens CD 9, CD 10, CD 20, CD 38, HLA-DR by monoclonal antibodies. In a longitudinal investigation of the survival period they revealed that the finding of circulating cells with signs of non-differentiation (presence of antigen CD 10 detected by antibody CALLA, presence of antigens B 1 (CD 20), CD 9 on circulating lymphocytes) has a prognostic meaning suggesting shorter survival. There was a direct correlation between the increase of CALLA positive cells and CD 9 positive cells. The authors found also that release of the clonus with signs of immaturity was present when the disease developed into the aggressive stage. While the group of 250 patients had according to statistical analyses, when treated according to protocol VMCP/MOCCA, a median survival of 90 months, the median survival of the aggressive stage (with the plasmoblast and lymphoplasmocytic type resp.) was only 12 months. The authors emphasize the prognostic importance of immunological typing of heterogeneous plasmocytoma populations.

[The favorable effect of hydrolytic enzymes in the treatment of immunocytomas and plasmacytomas].

At present attention is focused on research of biomodulating influences on tumorous processes, in particular inhibition of metastatic spread of tumors. In the aetiopathogenesis an important part is played by immune complexes, interaction of cytokines. The authors tested the supporting effect of hydrolytic enzymes in plasmocytoma and immunocytoma. The enzymes were administered along with cytostatic preparations according to the MOCCA pattern. They recorded a more rapid onset and longer persistence of remissions, a marked decline of total proteins, paraproteins, beta-2-microglobulin. Complications associated with paraprotein (hyperviscosity syndrome, nephrotic syndrome, peripheral angiopathy) improved. A combination of chemotherapy and enzymatic treatment proved effective and suitable, in particular for patients with interferon intolerance.

Circadian rhythm of plasma beta-thromboglobulin in healthy human subjects.

The circadian variation in platelet function may play a role in the morning increase of coronary thrombotic complications. The circadian variation of plasma beta-thromboglobulin levels--as an indicator of in vivo platelet activation--was investigated in seven healthy volunteers using a commercial radioimmunoassay kit. There was a statistically significant rhythm culminating with an amplitude of 30 micrograms/l (95% tolerance interval/15-45 micrograms/l) at 10.00 hours (95% tolerance interval/07.00-13.00 hours). The morning increase of plasma beta-thromboglobulin indicates an increased in vivo platelet activation at this time of day. This finding could correlate with the morning peak in the onset of myocardial infarction. It might also help in timing therapy of diseases which are associated with platelet hyperactivation.

Biochemical pattern of the ascitic fluid in liver cirrhosis and in neoplastic diseases.

In 22 subjects with liver cirrhosis and in 18 with malignancy, both with ascites, 10 biochemical parameters have been investigated in ascitic fluid. The evaluation of data by cluster analysis, optimized with the aid of multivariate normality testing, was able to recognize correctly a majority of cases. For diagnosis of malignancy, an 89% sensitivity and 82% specificity was obtained. The described way of data collecting and processing is recommended for the clinical practice.

The effect of prazosin therapy on platelet activation in essential hypertension.

1. Plasma levels of beta-thromboglobulin, initial and total platelet aggregation (induced by adrenaline or adenosine diphosphate [ADP]) were determined in 26 normotensive subjects and 26 patients with untreated essential hypertension. Groups of 18 essential hypertensive patients and 18 age- and sex-matched normotensives were compared. 2. After 7 days of treatment with prazosin in a dose of 2-8 mg daily the above measures were repeated in 18 essential hypertensive patients. A significant increase in plasma levels of beta-thromboglobulin, initial and total adrenaline-induced as well as ADP-induced platelet aggregation was found in hypertensives. Prazosin restored the mean arterial blood pressure in hypertensives to normal, but it had no significant influence either on increased beta-thromboglobulin levels or on initial and total aggregability. 3. The results confirm increased platelet aggregation and in vivo platelet activation in patients with essential hypertension; however there is a discrepancy with previous reports about those results obtained after prazosin therapy. The results suggest that increased platelet aggregation and in vivo activation need not be restored to normal after effective antihypertensive therapy alone. They give reason for the combination of antihypertensive together with anti-aggregatory therapy in essential hypertension.

Beta-thromboglobulin and platelet aggregation in essential hypertension and the influence of prazosin therapy.

Plasma levels of beta-thromboglobulin, initial and total platelet aggregation (induced by adrenaline or ADP) were determined in twenty-eight normotensive subjects and thirty patients with untreated essential hypertension. After 7 days of treatment with prazosin in a dose of 2-8 mg daily the above measurements were repeated in eighteen essential hypertensive patients. A significant increase in plasma levels of beta-thromboglobulin, initial and total adrenaline- and ADP-induced platelet aggregation was found in hypertensives. Prazosin restored the mean arterial blood pressure in hypertensives to normal, but it had no significant influence either on increased beta-thromboglobulin levels or on platelet aggregation. The results show that increased in vivo activation as well as increased platelet aggregation need not be restored to normal after effective decrease of blood pressure. The results suggest that a combination of drugs with an antihypertensive and antiplatelet (antiaggregating) effect (or use of a drug with both an antihypertensive and antiaggregating effect) can further decrease the development of severe complications of essential hypertension.

[Essential phospholipids in the treatment of chronic hepatitis B virus infection].

The authors investigated 24 patients with chronic infection with the hepatitis B virus. HBeAg-positive patients had significantly (alpha = 0.05) more frequently pathological initial values of biochemical and immunological indicators, as compared with anti-HBe-positive patients. Delta antigen in the liver and anti-delta antibodies in serum if examined, were always negative. All patients were subjected to 16-week treatment with Essentiale forte (Natterman). After treatment and after the subsequent 16 weeks in none of the patients loss of HBsAg or seroconversion of HBeAg-positive to anti-HBe-positive was recorded. In the sub-group of HBeAg-positive subjects after treatment a significant drop of gamma-globulins and circulating immunocomplexes occurred, in the sub-group of anti-HBe-positive subjects a significant rise of active and total T-lymphocytes was found. The authors recommend to test Essentiale forte in the prevention and treatment of immunocomplex disease in chronic infection with the hepatitis B virus.

[Coronary score and risk indicators].

Using the method of multi-dimensional statistical analysis in the evaluation of relations between the coronary score and risk factors in 120 men with ischaemic heart disease type stable angine, and in 30 men in a control group the authors revealed: age is a significant negative discriminator of the presence and indicator of the severity of atherosclerotic narrowing of the coronary arteries, cholesterol is a statistically significant positive independent discriminator of the presence and indicator of the severity of atherosclerotic narrowing of the coronary arteries, HDL-cholesterol is a significant negative independent indicator of the severity of the atherosclerotic narrowing of the coronary arteries, triacylglycerols are a positive indicator of the severity of the atherosclerotic narrowing of the coronary arteries only at the borderline of statistical significance, Broca's index of relative body weight is a positive discriminator of atherosclerotic narrowing of the coronary arteries only at the borderline of significance, there is a statistically significant positive additive relationship of quantitative risk factors with the atherosclerotic narrowing of coronary arteries, physical activity is a significant positive discriminator of the presence of atherosclerotic narrowing of the coronary arteries, the oral glucose tolerance test is a positive discriminator of the presence of the atherosclerotic narrowing of the coronary arteries.

[Noninvasive monitoring of the hemodynamic importance of the left atrium].

The authors examined 112 subjects free from cardiac disease, with essential hypertension and myocardial infarction. In all subjects a 12-lead ECG at rest was made as well as a polycardiographic and echocardiographic examination. The authors found that in the group of healthy subjects there is a direct significant linear correlation between the size of the systole of the left atrium and age. The size of the left atrium and the atrial filling fraction were significantly smaller in healthy subjects than in subjects with hypertension and myocardial infarction. The authors draw attention to the necessity to re-evaluate the accepted norm of left atrial size in patients with hypertension and myocardial infarction. The authors found that between the magnitude of the systole of the left atrium and the ejection fraction of the left ventricle a significant relationship of parabolic shape exists. The mentioned correlations suggest that with deterioration of left ventricular function the ratio of the left atrial systole increases up to a certain extent (40% of the ejection fraction); after this borderline is exceeded, the haemodynamic contribution of the left atrium declines again till it is insignificant. PTF-V1 on the ECG which is generally considered a sign of an enlarged atrium reflects in the group of patients with acute myocardial infarction above all the degree of reduction of systolic and diastolic left ventricular function and not changes in the size and function of the left atrium.

Frequency of Mg, Nya, Sc 2 and Wra antigens and antibodies in central Slovakia.

In randomly selected thousands-sized samples of the Central Slovakian population the following proportions of antigen (antibody) positivity were found: 0% for Mg (0.158% for anti-Mg), 0% for Nya (0.008% for anti-Nya), 0.284% for Sc 2 (0% for anti-Sc 2) and 0% for Wra (0.035% for anti-Wra). The difference between the antigen and antibody occurrence is statistically significant (alpha = 0.01) for Mg as well as for Sc 2. The found Mg and Nya frequency is comparable to that in the world but is significantly lower than in Switzerland and Norway, respectively; the frequency of antigens Sc 2 and Wra in Czechoslovakia is generally significantly lower.
