Sonography of the painful hip in children: 500 consecutive cases.

Five hundred children with a painful hip or a limp were evaluated prospectively by plain films and sonography. The clinical, radiographic, and sonographic findings were correlated with the final diagnoses. Sonography disclosed hip effusion in 235 patients, and plain films were abnormal in 58 of these 235 patients and in four others. Both sonography and plain films were normal in 261 patients. No sonographic signs served to differentiate sterile, purulent, or hemorrhagic effusion. Follow-up sonograms were performed in 202 patients. Sonography showed that 73% of patients with presumed transient synovitis had no effusion 2 weeks after diagnosis. Patients with hip disorders other than transient synovitis had persistent effusion for more than 2 weeks; however, that was also observed in 27% of patients with presumed transient synovitis. Sonography was more sensitive than plain films for detecting hip effusion. However, sonographic detection of effusion changed the therapeutic approach in only six patients.

Chronic granulomatous disease of childhood. Report of two cases with unusual involvement of the gastric antrum and spleen.

Chronic granulomatous disease (CGD) of childhood is a rare entity. The disease is characterized by recurrent infections with granuloma and abscess formation caused by an inherited defective neutrophil leukocyte function. The most common sites of involvement are the lungs, lymph nodes, skin, liver, spleen and bones. Rarely are other organs affected. Two children with CGD are presented. The children were cousins, the older with bone, lung and splenic involvement. The younger had circumferential thickening of the gastric antrum. Some of the lesions were well delineated with ultrasonography. The unusual gastric antrum wall and focal splenic involvement in this disease are emphasized.

Growth rate, muscle protein composition, intestinal absorption of D-galactose and L-leucine and nitrogen balance in rats fed the raw legume Chamaecytisus proliferus L.

Growth rate, nitrogen balance, skeletal muscle nitrogen fractions and in vivo intestinal absorption of D-galactose (2 mM) and L-leucine (20 mM) have been measured in male growing rats (90-100 g initial body weight) fed 12% protein diets containing either casein (control) or the raw leafy legume Chamaecytisus proliferus L. (Western Canary Islands). As compared to control casein-fed rats, those fed the raw legume diet exhibited a significant reduction (P less than 0.01) in the rate of growth, nitrogen balance parameters, sarcoplasmic nitrogenous fraction of skeletal musculature and in vivo intestinal absorption of galactose and leucine; however, the non-protein nitrogenous fraction has been found significantly higher (P less than 0.01) than that of control animals, whereas the myofibrillar nitrogenous fraction remained unchanged. The possible nature of these findings is discussed.

Fetal subdural hematoma: diagnosis in utero. Case report.

A subdural hematoma was diagnosed antepartum in an infant by means of sonography. An elective caesarean section was performed because cephalopelvic disproportion, and later the hematoma was drained with a poor evolution due to basal ganglia and intraventricular hemorrhage. No etiological factor of bleeding was found. This is the third published case of fetal subdural hematoma diagnosed antepartum.

Littoral cell angioma of the spleen: US and MR imaging findings.

In this article, we describe the ultrasound and magnetic resonance imaging findings of a littoral cell angioma of the spleen. This benign vascular neoplasm of the spleen has been described, but to our knowledge there has been no case published in the literature that describes its imaging features.

[A patient with visceromegaly: risk factor for performing percutaneous endoscopic gastrostomy. A clinical case].

Percutaneous endoscopic gastrostomy is an easy and safe technique to provide an enteral access for patients needing long-term enteral nutrition. Minor complications may occur in 9% to 13% of patients. Life-threatening complications appear in 1-3% of cases. Perforation of a hollow viscus leading to peritonitis is a rare condition; hepatic perforation after placing a percutaneous endoscopic gastrostomy (PEG) has not been reported previously. In patients with massive visceromegaly an abdominal ultrasound may help in localizing the place of punction avoiding surrounding organs.

[Traumatic section of the thoracic aorta: its repair by direct aortic clamping].

Traumatic section of descendent aorta, severe complication of a thoracic traumatism, requires an early recognition and restoration because its high mortality rate. Between 1988 and 1990, three patients underwent surgical approach, by direct aortic clamping [correction of clamplaje], at our Service. In two cases, a heart stoppage appeared during the clamping/unclamping [correction of clampaje/desclampaje] maneuvers. One of them had previously electrocardiographic abnormalities, suggestive of heart contusion. Two patients died, one of them during surgical procedure because an irreversible heart stoppage, and the second patient died after 7 days because of a brain hemorrhage. One case presented postoperative paraplegia. Respective rates and literature about the main factors implicated in the diagnosis and treatment of such pathology are reviewed.

[Ergotamine poisoning. Two cases of peripheral ischemia].

Ergotism is a rare disorder, generally caused by iatrogenia after the administration of drugs containing ergotamine derivates. Two cases of ischemic incidents at the extremities caused by a chronic intoxication with ergotamine tartrate, after its prolonged administration in therapeutical doses as a treatment for hemicrany, are reported. A short bibliographic review about semiology and therapeutic possibilities of such toxic pathology has been made.

[Renal duplex: clinical usefulness].

It is the purpose of this report to focus attention on the clinical usefulness of Renal Duplex for the diagnosis of patients with vasculo-renal diseases in terms of: 1. Accuracy of Duplex/Angiography in the measurement of the renal stenosis degree. 2. Correlationship between Duplex ans Isotopic Renogram with respect to the study of the parenchyma's perfusion. 3. The effect of the inhibitors of the conversor enzyme (Captopril) on the Doppler signal of the parenchyma, comparing it with the results from the captopril test about the peripheral plasmatic renin activity and the isotopic renogram, in patients with vasculo-renal HTA. Results obtains by Duplex and Angiography were compared in 92 renal arteries from 46 patients. For both technics, three degrees of stenosis were established: 0-59%, 60-99% and occlusion. The Duplex technique identified 49/54 stenosis < 60%, 28/33 stenosis > 60% and 5/5 occlusions (Kappa 0.8). Sensibility and specificity of Duplex for the diagnosis of stenosis > 60% were, respectively, 89.5% and 90.7%; with an exactness of 90.2%. The angiographies showed stenosis > 60% in 23 patients with HTA (diastolic pressures > 100 mmHg). In all of the patients, a measurement of the plasmatic renin activity, an isotopic renogram and a Doppler of the interlobar arteries basal and post-captopril, were performed. The correlationship between Duplex and isotopic renogram with respect to the measurement of the relative renal perfusion was statistically significant (r = 0.91; p < 0.0001). The captopril test for renin and isotopic renogram were positives for 5 patients (4 with unilateral stenosis an 1 with bilateral stenosis). p53 oncoprotein as a prognostic indicator in patients with breast cancer.

UNLABELLED: p53 is a tumor suppressor gene located on the human chromosome 17 that is thought to regulate (suppress) the proliferation of normal cells. The mutant protein accumulates in the nuclei of tumor cells that may then have a proliferative advantage over normal cells. The purpose of this study was to investigate the relationship between levels of mutant p53 expression and the clinical outcome of patients with node-positive and node-negative breast cancer. Expression of mutant p53 was evaluated in 655 human breast carcinomas (349 node-positive and 306 node-negative patients) with long-term clinical follow-up by immunohistochemistry in sections from paraffin embedded tumors. RESULTS: Immunoreactivity was found in 37.3% of breast tumors. There was no significant correlation between the expression of p53 and tumor size, nodal involvement, age or histological type. However, p53 overexpression was clearly related to histological grade and steroid receptors, with a trend to higher overexpression in ER-tumors or in those with a high histological grade (p < 0.01). On univariate analysis positive tumors were associated with reduced DFS in the total group (p < 0.001) as well as in node-positive patients (p < 0.05) and in node-negative patients (p < 0.01). In conclusion, these results suggest that the immunoreactivity of p53 may be a biologic marker of prognostic significance in both node-positive and node-negative patients.
