[Friction melanosis].

We describe 23 cases of unusual brownish pigmentation occurring over the bony tissues such as clavicle, scapula or vertebrae. There were 19 females and 4 males. The age ranged from 18 to 59, mostly between 20 and 35 years. Since the first observation in 1977, cases gradually increased. The pigmentation usually showed ripple pattern on the upper back and clavicular region, zebra-like pattern over the ribs and postage-stamp-like macule over the vertebral column. Although there was no inflammatory sign such as itching or scaling, histology revealed postinflammatory melanin deposit in the upper dermis. Exceptional detection of amyloid deposit in the papillary layer in one case could be secondary to the slight epidermal damage actually disappeared. The general condition was not affected, and laboratory data remained within normal limits. This unique melanosis should be separated from other pigmentary disorders and macular amyloidosis. We would like to propose the name of friction melanosis, because this condition should be intimately related with the repeated friction by nylon towel or brush, widely used by young peoples in the bathroom.

Weber syndrome caused by intracerebral hemorrhage in a hemophiliac boy.

A rare occurrence of the Weber syndrome in childhood is reported. The patient was a 7-year-old hemophiliac with recurrent intracerebral bleeding into the right cerebral peduncle and pontine base. Left hemiparesis involving the face and tongue developed and was later accompanied by right oculomotor nerve palsy.

Infantile bilateral striatal necrosis: chronic and acute manifestations in a single case.

We report a girl who exhibited a slowly progressive extrapyramidal disorder with onset in early infancy. CT examinations demonstrated progressive atrophy of the cerebrum. At the age of 6 years, acute exacerbation occurred with impairment of consciousness and autonomic functions. During the acute episode, CT revealed lesions in the bilateral striatum and globus pallidus.

Tuberous sclerosis: immunohistochemistry expression of tuberin and hamartin in a 31-week gestational fetus.

Tuberous sclerosis complex (TSC) is a common autosomal dominant disorder in which affected patients develop a wide variety of benign and malignant tumors. We report here on a 31-week gestational age fetus with pathological features of TSC. Developmental expression of hamartin and tuberin in various tissues was studied using immunohistochemistry. There was loss of expression of hamartin in the tuber and weak expression of the tuberin. Both hamartin and tuberin were expressed in bronchial epithelial cells, cardiac muscles, renal collecting tubules, and neural tissues. The rhabdomyomas stained negatively for tuberin and hamartin. Two genetic loci are responsible for TSC-TSCI and TSC2. The TSC1 gene on chromosome 9 encodes a protein termed hamartin that lacks sequence similarity to any known proteins, whereas the TSC2 gene on chromosome 16 codes for a protein termed tuberin. These results indicate that tuberin and hamartin may play a critical role in development and thus provide a framework for understanding the developmental and hamartomatous manifestations of tuberous sclerosis.

Digital subtraction angiography for leg venography.

Venography was performed in 25 legs of 17 patients using digital subtraction angiography (DSA). Reliable diagnoses of deep vein thrombosis or patency were made in the majority of these cases. There were no complications. Multiple injections of contrast material were required to image the entire leg; however, the total quantity of iodine necessary for each leg averaged 9 g, significantly less than with conventional techniques. The preferred method consisted of injecting 15 ml of 100 mg iodine/ml contrast material followed by 15 ml of heparinized saline. DSA proved a safe and useful means of evaluating veins of the leg.

Abnormal expression of cell adhesion molecule L1 in migration disorders: a developmental immunohistochemical study.

We studied immunohistochemically the expression pattern of a neural cell adhesion molecule, L1, in various human migration disorders associated with polymicrogyria: 4 fetuses and 11 infants having Fukuyama type congenital muscular dystrophy (FCMD) (4 cases), Zellweger syndrome (6) or thanatophoric dysplasia (3) and intrauterine brain damages (2) at different development stages, comparing to age-matched controls. There were different patterns of L1 expression, which suggested at least 3 pathogenetic mechanisms: high expression associated with neuoraxonal overgrowth (fetal FCMD and destructive event at intermigratory period); delayed expression with neuronal dysmaturation and dysmyelinogenesis (late infantile stage of Zellweger syndrome); no expression in toxic or destructive brain injury (Zellweger syndrome or destructive events at inter-or postmigratory periods).

Computed radiography angiography using storage phosphor imaging plates: eight year's experience.

We reviewed the indications, modalities, and quality of angiographic examinations performed in our department from 1987 to 1994 and compared the image characteristics and technical convenience of three modalities: conventional film-screen angiography, fluorographic digital subtraction angiography (F-DSA), and computed radiography angiography using biplane rapid changers of storage phosphor imaging plates (IP-CRA). IP-CRA has practically eclipsed conventional film-screen angiography in our radiology department. Sixty-percent of the total 700 examinations carried out in 1994 were performed using a combination of IP-CRA and fluorographic digital subtraction angiography (F-DSA). The remainder were performed with F-DSA alone. The post-processing functions of IP-CRA like subtraction and change of contrast or density were useful especially in such regions as pulmonary, bronchial, or external carotid arterial territories, where high natural contrast made image processing difficult with both F-DSA and film-screen angiography. The spatial resolution of IP-CRA was superior to that of F-DSA and comparable to the film-screen method. It was concluded that angiography using imaging plates was a useful part of the digital radiography system.
