Patient's assessment of out of hours care in general practice.

A sample of 177 patients drawn from 13 north London practices were interviewed shortly after they had sought help from their practice outside normal surgery hours. Patients were asked to describe the process and outcome of their out of hours call, to comment on specific aspects of the consultation, and to access their overall satisfaction with the encounter. Parents seeking consultations for children were least satisfied with the consultation; those aged over 60 responded most positively. Visits from general practitioners were more acceptable than visits from deputising doctors for patients aged under 60, but for patients aged over 60 visits from general practitioners and deputising doctors were equally acceptable. Monitoring of patients' views of out of hours consultations is feasible, and the findings of this study suggest that practices should regularly review the organisation of their out of hours care and discuss strategies for minimising conflict in out of hours calls--particularly those concerning children.

Prescribing: the power to set limits.

This paper discusses drawing up a restricted list of 245 drugs for use in an inner London group practice, based on a review of prescribing patterns in November 1982. The likely impact of the recent proposals by the Department of Health and Social Security to limit drugs available for prescription under the National Health Service on this project and on patient care is considered. We conclude that generic prescribing and a limited list of drugs may improve the quality of prescribing and be the only way to curb prescribing costs but that a limited list should be flexible, responsive to patients' needs, and applied to all prescribing. There should also be a mechanism for consumer feedback and regular revision of the list.

How patients manage asthma.

In this paper we discuss how patients cope with asthma and suggest that individuals use medical advice selectively in a manner they consider appropriate to their personal and social circumstances. Use of medical services is influenced by patients' perception of the effectiveness of medical therapies, concern about the long-term use of drugs, access to alternative therapies, support and advice from others and the need to make particular sense of living with a chronic disease. Some people are reluctant to use medical services under any circumstances.

Technology requirements for nonterrestrial ecosystems.

Factors associated with planning and implementing research and development for nonterrestrial closed-ecology life support systems have been analyzed in terms of systems engineering considerations. These factors are discussed in this paper to provide a planning overview. It is anticipated that systems of this type probably will be required for future manned space activities that would involve many participants in a space habitat, particularly at remote locations or for long-duration missions. Of particular interest are design scenarios that involve nonterrestrial ecosystems. Although these approaches offer the promise of being able to provide food regeneration by conventional processes, such as photosynthesis and essentially terrestrial agricultural methodology, the potential impact of nonterrestrial environmental conditions on these processes and criteria for ecological stability are not adequately defined. The role of these factors in research planning is discussed, and potentially relevant areas of research are described.
