Familial hydroa vacciniforme.

Hydroa vacciniforme is a rare, idiopathic photodermatosis with an onset in childhood and characterized by acute vesiculation, crusting and scarring following sun exposure. Familial cases are extremely rare with only one previous report. We report a brother and sister, both of whom have developed hydroa vacciniforme.

Accumulation of heavy metals in freshwater molluscs.

Heavy metals in the aquatic environment have to date come mainly from naturally occurring geochemical materials. However, this has been enhanced by human activity such as gold mining in the case of heavy metal pollution in Sg Sarawak Kanan. The high suspended solid loads in the river have quite efficiently removed most soluble metals from the water and trapped them in the bottom sediment. Three freshwater mollusc species were collected at the point source of the heavy metal pollutants and analysed for the heavy metal contents in their tissues and shells. Two of the mollusc species (Brotia costula and Melanoides tuberculata) are purely freshwater species while the Clithon sp. nr retropictus is able to survive in fresh and brackish water environments. The Brotia costula and the Clithon sp. are the edible species which are sold in the market. Accumulation of As, Cu, Fe, Se and Zn in all the three mollusc species were determined and the level of As in the tissues of Brotia costula and the Clithon sp. was much higher than the permissible level for human consumption. The mollusc species also demonstrated different preferences for the uptake of different metals. Variations in the heavy metal contents in the shell and tissues of the same species were also observed.

Infected arteriovenous hemodialysis graft presenting as left and right infective endocarditis.

Prosthetic grafts have become an acceptable alternative to the autogenous arteriovenous dialysis fistula. The major drawback in their use for the dialysis patient is infection. We present a case of infected polytetrafluoroethylene arteriovenous fistula (endarteritis) with signs of unilateral splinter hemorrhage, Janeway lesions in the affected limb and septic venous embolization to the lungs. The graft eventually ruptured requiring emergency surgery. Staphylococcus aureus is by far the most common implicated organism. An otherwise non-threatening infection at a fistula site can lead to more serious complications such as infective endocarditis.

Blood Vessel Prosthesis

Determination of naltrexone dosage for narcotic agonist blockade in detoxified Asian addicts.

Thirty-eight, adult, male, detoxified, Malaysian opiate addicts participated in this double-blind clinical evaluation of naltrexone. Addicts from three ethnic groups: Chinese, Malays, and Indians received a weekly regimen of naltrexone (100 mg on Days 1 and 3, and 150 mg on Day 5). Subjects were randomly assigned to receive intravenous challenge with either 25 mg heroin or placebo 12, 24, 48, and 72 h after the third naltrexone dose. Physiological and subjective parameters were measured at each challenge interval. Results indicated that naltrexone was effective in blocking the physiological and psychological effects of heroin for at least 48 and 72 h, respectively, in detoxified Malaysian opiate addicts. The efficacy of naltrexone did not differ among the three ethnic groups evaluated in this study.

Initial nonfunction in cadaveric renal transplantation.

Patients who receive a kidney transplant that does not function immediately have more complications and decreased graft survival than patients whose allografts function immediately. To determine the causes of initial nonfunction (INF), the authors reviewed 188 consecutive cadaveric kidney transplants performed between 1985 and 1988 at the University Hospital, London, Ont. Data were collected on 16 putative risk factors for INF, which were divided into three categories: donor, recipient and technical. INF was defined as the need for dialysis within 7 days of transplantation. Forty-eight (26%) of the 188 allografts had INF, 6 of which never functioned. Univariate analysis identified five variables associated with increased risk of INF: no donor dopamine use, back-table flush, single-organ retrieval, exchanged kidney and prolonged cold ischemic time. Multivariate analysis, however, identified only three variables associated with INF: cerebrovascular accident as the cause of donor death, no donor dopamine use and single-organ retrieval. The authors recommend (a) low-dose dopamine therapy for all donors and (b) multiorgan retrieval to produce quality kidneys for transplantation.

Cardiomyopathy in pregnancy: 2 case reports.

Cardiomyopathy developed in the peripartum period in two healthy primigravida at 41 and 42 weeks of gestation are presented. In the first case, the patient presented with pulmonary oedema and had cardiac arrest immediately after admission. After resuscitation, a live baby was delivered by Caesarean section. Second cardiac arrest occurred after 12 hours of operation and the patient was resuscitated again. On the 8th post operative day, patient died of D.I.V.C. and renal failure. The second case was a twin pregnancy associated with pregnancy induced hypertension. Caesarean section was performed for foetal distress. She developed pulmonary oedema with left ventricular failure on the second post operative day, was resuscitated and discharged on the 15th day. These are the first two cases to be reported from Hospital University Sains Malaysia. Reference is made to three other cases in which a similar pathological process might have occurred.

Adult

Coronary artery disease in patients aged 30 and younger.

Twenty-four male patients aged 18-30 (mean = 27.4) years were catheterized either after myocardial infarction (22 patients) or because of angina pectoris (2 patients). In this highly-selected group, 18 of 24 smoked but only 5 had elevated cholesterol levels. Of 14 patients with anterior myocardial infarction (AMI), 7 had a significant left anterior descending obstruction while the other 7 had nonsignificant or no coronary stenosis. In 8 patients with inferior MI (IMI), 4 had significant multivessel disease. Five of 6 patients with multivessel disease smoked while 4 had hypercholesterolemia. Risk factors were absent in 3 of 16 patients with single or no coronary lesions. We conclude that angina pectoris and inferior MI carry high risk of multivessel coronary disease; there is often one or more risk factor in this subgroup. Finally, routine catheterization in young patients with AMI is not warranted.
