Colorimetric analysis of pigmented skin lesions: a pilot study with the Visi-Chroma VC-100 device.

BACKGROUND: Definition of the colour of pigmented skin lesions (PSLs) with the naked eye remains subjective and may be influenced by lighting. This problem underlines the usefulness of instrumental assessments such as epiluminescence microscopy and colorimetric devices. OBJECTIVE AND METHODS: We describe here a new method of colour analysis of PSLs with the Visi-Chroma VC-100 device, which illuminates the surface of the skin with white light-emitting diodes (LEDs) and analyses the reflected light by a red-green-blue (RGB) charge-coupled device (CCD) colour camera. Twenty-one PSLs to be excised for cosmetic or medical reasons were analysed by this device with clinicopathological correlation. CONCLUSIONS: This method is feasible and might be useful to assess the colour of PSLs and allow comparisons for changes over time. Further studies are needed to determine the usefulness of this device in clinical practice.

[Correlation between ultrasound and radiographic data for assessment of symphyseal bony callus maturation after distraction].

OBJECTIVE: To search for means of reducing ionizing radiation during treatment planning for transmandibular osteodistraction. MATERIAL AND METHODS: Eleven patients (6 female, 5 male) underwent combined bimaxillary osteodistraction at a mean age of 18 Years. The following investigations were performed to determine symphyseal callus maturation: occlusal radiographs (O-RX) preoperatively, after the active phase of distraction (mean 27 days); ultrasound investigations (US) after the active and consolidation phase. RESULTS: US showed calcification foci at the end of the active phase, when O-RX failed to do so. Both US and O-RX detected callus immaturity after 2 Months of consolidation in 1 patient. Both detected bridging ossification one Month later. DISCUSSION: US can replace radiography in the diagnosis of callus maturation related to distractor removal.

The Visi-Chroma VC-100: a new imaging colorimeter for dermatocosmetic research.

BACKGROUND/AIMS: It was the aim of this study to carry out a comparative evaluation in vitro on standardized color charts and in vivo on healthy subjects using the Visi-Chroma VC-100, a new imaging tristimulus colorimeter and the Minolta Chromameter CR-200 as a reference instrument. The Visi-Chroma combines tristimulus color analysis with full color visualization of the skin area measured. The technical performances of both instruments were compared with the purpose of validating the use of this new imaging colorimeter in dermatocosmetic research. METHODS: In vitro L*a*b* color parameters were taken with both instruments on standardized color charts (Macbeth and RAL charts) in order to evaluate accuracy, sensitivity range and repeatability. These measurements were completed by in vivo studies on different sites of human skin and studies of color changes induced by topical chemical agents on forearm skin. The accuracy, sensitivity range and repeatability of measurements of selected distances and surfaces in the measuring zone considered and specific color determinations of specific skin zones were also determined. RESULTS: The technical performance of this imaging colorimeter was rather good, with low coefficients of variation for repeatability of in vitro and vivo color measurements. High positive correlations were established in vitro and in vivo over a wide range of color measurements. The imaging colorimeter was able to measure the L*a*b* color parameters of specific chosen parts of the skin area considered and to measure accurately selected distances and surfaces in the same skin site considered. CONCLUSION: These comparative measurements show that both instruments have very similar technical performances and that high levels of correlation were obtained in vitro and in vivo using the L*a*b* color parameters. In addition, the Visi-Chroma presents the following improvements: 1) direct visualization and recording of the skin area considered with concomitant color measurements; 2) determination of the specific color parameters of skin areas chosen in the total measuring area; and 3) accurate determination of selected distances and surfaces in the same skin areas chosen.

[Rehabilitation of the anterior portions of the maxilla and mandible by means of implants and bone grafts].

This article describes the possibilities and difficulties of reconstruction in the front regio by means of implants and bone transplants in five patients. First of all, planning is the most important step. Sometimes the class III relationship is a real challenge. Different bone transplants, the inlay and onlay technique, the number of the implants, the importance of the way the implants are uncovered in the second stage are discussed. In the front region, we aim for functional and esthetic reconstruction.

[Vertical augmentation of the alveolar process by distraction osteogenesis].

This article describes the treatment of a boy with Class II deep bite and hypoplasia of the alveolar process due to agenesis of the canines and premolars. At first, the occlusion has been adjusted by advancement of the mandible and opening of the bite after bilateral sagittal split osteotomy. Later, under local anaesthesia, a segmental osteotomy of the hypoplastic alveolar process has been performed. Distraction of the hypoplastic alveolar process has been achieved by orthodontic traction on the residual dentition in the segments. After sufficient augmentation of the alveolar process, 6 implants for two bridges have been placed.

[Prevention of eyelid retraction following lower blepharoplasty].

This review covers the techniques used to prevent lid retraction after lower blepharoplasties. After a brief overview of the lower lid anatomy (anterior, middle, posterior lamella), the causes of blepharochalasis and of lid retraction are addressed. Clinical examinations are described that can detect a candidate at risk. Many preventive measures are discussed, examining patient positioning, approaches (transcutaneous, transconjunctival, combination), incision types, flap dissection (skin, skin-muscle, dermal flaps), suspension techniques (muscle-muscle, muscle-periosteum, horizontal wedge excisions, lateral and medial canthal procedures), CO2 laser resurfacing and combinations. These techniques are described and critically appraised.

Computer-aided craniofacial surgical planning implemented in CAD software.

Accurate presurgical planning is imperative for successful cranial surgery. This article introduces a simulation program developed in a computer-aided design environment. The neurocranium is introduced as a mathematical surface, since this is the part on which the actual operation will be performed. The viscerocranium, which serves as reference, is visualized using small triangular surfaces. The development of the program commenced with a classification of the different surgical techniques mentioned in the literature into six basic actions. The use of mathematically described surfaces has the advantage that the program can simulate actions which change the shape of a surface and perform an on-line estimation of the fracture risk during bending. Three-point bending tests were carried out to provide the necessary data to perform the mathematical check, as these data are not available in the literature. A database with reference distances was introduced to guide the surgeon to obtain the best possible results. During one clinical trial, the computer was taken into the operating room so that the surgical plan developed with the simulation program could be applied to the actual operation.

Non-union of a preprosthetic Le Fort I osteotomy with freeze-dried bone as an interpositional graft.

A case of non-union of a preprosthetic Le Fort I osteotomy using freeze-dried bone as an interpositional graft in a 38 year old woman is presented. The principles of natural bone repair and the different causes of non-union of bone fragments are discussed. How long does nature need for successful ingrowth of connective and vascular tissue and for the resorption-replacement mechanism using freeze-dried bone to bridge an osteotomy gap? Successful treatment of the non-union consisted of curettage of the granulation tissue followed by autogenous cortico-cancellous bone grafting.

[Mandibular fracture: an unusual complication following osteotomy of the chin].

A case of a median mandibular fracture with extensive osteolysis 8 weeks following a reduction and protrusion genioplasty is reported. A porous hydroxylapatite block, Interpore 200, was used for contour augmentation at the labiomental fold. The weakening effect of the osteotomy, a foreign body-related chronic osteomyelitis, and repetitive angulation of the mandible during mastication play a role in the pathogenesis of this fracture. Treatment consisted of removal of the porous hydroxylapatite block, removal of the screws and plates, surgical debridement of the necrotic tissue, antibiotics, and immobilization with an arch bar on the buccal side and a lingual acrylic splint to prevent rotation of the upper border of the mandible. Healing was uneventful, without loss of teeth.

[Temporomandibular joint symptoms in orthognathic surgery: a retrospective study].

The records of 152 consecutive patients who underwent orthognathic surgery in the Division of Maxillofacial Surgery A.Z. St.-Jan, Brugge, between 1/10/90 and 1/10/91 were evaluated for pre- and postoperative temporo-mandibular joint (TMJ) symptoms. Fewer TMJ symptoms were found postoperatively, than preoperatively (14.4% versus 19.7%). In the mandibular retrognathism group there were twice as much TMJ symptoms preoperatively in the low and normal mandibular angle patient group than in the high mandibular angle group (26% versus 13%). After surgery, there was a decrease of TMJ symptoms in the low and normal angle patient group (83% improvement). In the high angle absolute mandibular retrognathism group however, more new TMJ symptoms were seen postoperatively (21%). Bimaxillary surgical correction of a high angle absolute mandibular retrognathism case may provoke condylar resorption. Advice for follow-up and suggestions to reduce this annoying complication are given.

[Case study: a calcified epithelial odontogenic tumor (Pindborg tumor)].

The calcifying epithelial odontogenic tumor is a benign but locally aggressive tumor. Occasionally the term Pindborg tumor is used after the Danish pathologist who first described this tumor as an entity. The clinical feature is most commonly a slow-growing painless swelling. The tumor may show considerable roentgenographic variation. Usually this tumor shows characteristic histological features. Some variants can mimic malignant neoplasms. The invasive nature is not so obvious as that of an ameloblastoma, nevertheless it has to be treated in the same way. A long follow-up period is indicated since recurrence can occur after many years. A patient with a characteristic Pindborg tumor is presented.

Craniofacial resection for anterior skull base tumours.

This study presents a series of 10 patients with anterior skull base tumours, treated by a team of neurosurgeons and head- and neck surgeons. The series included 7 malignant tumours of the nose and paranasal sinuses and 1 retinoblastoma, all with intracranial extension through the lamina cribrosa. There were also 2 patients with an anterior base meningioma, growing into the ethmoid sinus and the nasal cavity. 8 tumours were resected by a combined bifrontal craniotomy and uni- or bilateral rhinotomy. In 2 cases a bifrontal craniotomy alone without facial incision sufficed. The skull base was closed with a pediculated pericranial flap and a split-thickness free skin graft underneath. There were no postoperative problems of wound infection, CSF-leakage or meningitis. Recurrent tumour growth or systemic metastasis occurred in 5 out of 7 patients with malignant tumours, 6 months to 2 years postoperatively. The related literature and especially questions of operative indications and technique, including different possibilities of closure and reconstruction of the skull base, are discussed.

[Lingual nerve injury during extraction of lower wisdom teeth].

A lingual nerve injury is an infrequent but unpleasant complication of the removal of lower third molars. The nerve stays at risk even with a correct technique. The classifications of nerve injuries of Seddon and Sunderland give insight in the complexity and give guidelines for prognosis and treatment. Degeneration and regeneration as in other peripheral nerves occur also here and knowledge of these phenomena is crucial for the success of the microsurgical repair. Confronted with the problem, an attempt to a correct evaluation of the extent of the injury should be done, and transfer to a specialist is necessary when the dysesthesia is still persistent after more than 2 months.
