[Postmenopausal osteoporosis. Early diagnosis as an indication for preventive hormone therapy].

Idiopathic osteoporosis mainly affects postmenopausal women. The normal trabecular volume of the lumbar vertebrae in a sample of healthy perimenopausal women was established by monoenergetic computed tomography. Early diagnosis of diminished bone mass is crucial for the identification of women at risk for involuntary osteoporosis following climacteric estrogen depletion. Body weight, endogenous levels of sex steroids, renal calcium and hydroxyproline excretions are not related to individual bone mass in the lumbar spine.

[Mineral content of the spongiosa of the lumbar spine determined by computer tomography. Normal values for healthy perimenopausal women and their relation to mechanical stress of the vertebral column].

Healthy perimenopausal women (152) were examined by QCT to quantify the mineral content of the axial skeleton. The density was measured in the spongiosa of the L2-4, using single energy technique and the Cann-Genant calibration phantom. The women, aged 37 to 64, had no history of pathological fractures, ovariectomy, metabolic disease, or hormone therapy. The age-related mean values were 25 mg K2HP04/cm3 lower than the normal values reported by Genant et al. for 203 healthy American women. The 95% confidence interval was distinctly narrower (+/- 40 mg/cm3) than the American confidence interval (+/- 60 mg/cm3). To evaluate the relationship of the BMC to mechanical stress, the BMC was compared with body weight, body mass index (weight/height2), and the lumbar muscle mass estimated from cross sections of the paravertebral muscles in the CT images. The assumption that higher mechanical stress results in higher bone mineralization could not be confirmed.

Pregnancy following intracytoplasmic sperm injection of immotile spermatozoa selected by the hypo-osmotic swelling-test: a case report.

The success of intracytoplasmic sperm injection (ICSI) is poor when only immotile spermatozoa can be retrieved. In a couple with complete male asthenozoospermia the possible use of the hypo-osmotic swelling test to select spermatozoa for microinjection was examined. Following incubation in hypo-osmotic medium (Hypo 10, IVF Science, Göteborg, Sweden), 26% of immotile spermatozoa showed signs of sperm swelling (HOS-positive). After injection of HOS-positive spermatozoa, 5 out of 12 oocytes fertilized (41%) and after transfer of three embryos a healthy singleton pregnancy was achieved. In a previous ICSI cycle of this couple without preselection of spermatozoa by the HOS test, only 1 out of 10 oocytes fertilized. It is concluded that selection of spermatozoa by hypo-osmotic swelling-test prior to sperm microinjection seems to be a valuable tool to increase the fertilization rate in cases with complete asthenozoospermia.

Stimulation of bone growth through sports. A radiologic investigation of the upper extremities in professional tennis players.

This contribution addresses the following questions: Does unilateral sports-specific strain affect the skeletal system of the athlete? Specifically, can any differences be found in longitudinal growth of the bones of the forearm and hand in professional tennis players between the stroke arm and the contralateral arm? An investigation was conducted involving 20 high-ranking professional tennis players (12 male and eight female players) between 13 and 26 years of age as well as 12 controls of the same age range. The radiologic examinations of the bones of the forearm and hand yielded an increase in density of bone substance and bone diameter as well as length in the stroke arm as compared with the contralateral arm. Whereas the first results confirm previous findings, the stimulation of longitudinal growth has never been reported. This change in bone structure and size can be attributed to two factors: mechanical stimulation and hyperemia of the constantly strained extremity. It may thus be regarded as a biopositive adaptation process.

[Case reports of the Kümmel-Verneuil syndrome].

The significance of the complex clinical-radiological diagnosis is demonstrated in three cases of the Kümmel-Verneuil syndrome, the bone scintigraphy allows the demonstration and localisation of the bone process, but in respect to the differential diagnosis the results are unspecific. The CT allows an early and sure demonstration of the intravertebral vacuum phenomenon. In patients with nonspecific vertebral fractures the quantitative determination of the mineral content of the vertebral bones is indicated. The therapeutic decision is determined by the necessity of the biomechanical stabilisation.
