Anastomotic healing in dogs under cortisone treatment. A pilot study comparing compression and stapled anastomosis.

Colonic anastomoses made both by a new Compression Anastomotic Device (CAD) and by a traditional stapler (Autosuture CDEEA) were evaluated in impaired anastomotic healing induced by systemic cortisone in the dog. Twenty dogs were given daily i.m. hydrocortisone (25 mg/kg) starting one month before surgery and then until sacrifice. Eight untreated dogs served as controls. Surgery consisted of colonic transection and anastomosis done with CAD-25 in half the cases and with CDEEA-25 in the remaining half. The dogs were sacrificed six and 13 days after surgery. Macroscopic assessment, bursting pressure test, and histology were performed on the anastomosis. One dog died from peritonitis due to anastomotic dehiscence. No other clinical complications were observed. Although the number of observations was too small to attain statistical significance, CAD anastomoses appeared better than stapled ones as regards peri-anastomotic adhesions, anastomotic index, and histology. This preliminary study suggests that compression is as reliable as the stapler in the construction of colon anastomosis even in such situations of delayed anastomotic healing. Further experience is required to substantiate this conclusion.

[Identification of human herpesvirus HHV-6 sequence in a case of Hodgkin's disease by polymerase chain reaction].

In the last decades new approaches to the diagnosis and treatment of Hodgkin's Disease (HD) have contributed to improved rates of survival and cure but the pathogenesis of the disease still remains unknown. Data have been collected suggesting a relationship between viral infections and HD. HD patients with evidence of Epstein-Barr virus genomes in their affected tissues have been recently reported. Human Herpes Virus six (HHV-6) is a newly isolated virus, derived from patients with lymphoproliferative disorders. In order to investigate the possible role of this virus in the pathogenesis of HD we looked for a specific segment of HHV-6 genome by means of the polymerase chain reaction (PCR) in tissue samples obtained from peripheral blood and lymphnodes in HD patients: one clearly positive case has been identified. This result is the first indication of HHV-6 sequences associated with a case of HD and raises the possibility that this virus might be involved in the pathogenesis of this lymphoproliferative disorder. The relationship between HHV-6 and HD therefore warrants further investigation.

[Mitochondrial DNA deletion in a case of progressive ophthalmoplegia].

Mitochondria are unique among intracellular organelles because they contain their own DNA, which can be transcribed and translated to form proteins. Mitochondrial diseases include myopathies and multisystem disorders. The case of a patient showing bilateral ophthalmoplegia with proximal limb weakness, severe dysphagia and short stature, without family history, is described. The analysis of mitochondrial DNA of the patient muscle revealed a deleted form accounting for 65% of the total mitochondrial DNA. The Southern Blot Analysis of mtDNA allows a rather precise localization of deletions giving new insights in the pathogenesis of mitochondrial myopathies and representing a new precious diagnostic tool in these diseases.

Dissolution of bile duct stones with monooctanoin: in vitro study.

Mono-octanoin (Mo) is the drug of choice in the topical litholytic treatment of residual gallstones following cholecystectomy. Although this drug does not produce significant side effects, it requires a lengthy period of treatment (15-20 days). The purpose of this study was to verify the in vitro efficacy of the mixture Mo + 10% H2O vs pure Mo in human cholesterol stones. The findings indicate that this mixture can reduce dissolution time by 15.8% and increase the dissolution rate by 27.9% vs pure Mo. A further significant reduction (p = 0.0001) in dissolution times can be obtained by constant exchange of the solvent at the surface of the stone (stirring).

Surgical treatment of malignant tumors of the Oddian region.

From 1974 through 1984, 24 patients with malignancies of the Oddian region underwent surgery. Four ampullectomies were carried out with one more than 10-year survivor. Nineteen duodenopancreatectomies were performed with no operative mortality, a 21% complication rate and a 4% reoperation rate; mean survival was 42 months; 1-, 3- and 5-year survival was 95%, 53%, and 37% respectively. Nodal metastasis was the major determinant of long-term survival. Duodenopancreatectomy should be considered the operation of choice for malignancies of the Oddian region.

[Heterotopic allotransplant of the heart and lungs in the rat. Technical note].

The modalities for the heterotopic transplant of heart-lungs in the rat are described in detail. As in the case of all other rat transplants, this type is fairly easily performed using the techniques and instruments of microsurgery. Special attention is required at the moment of isolating the heart-lung preparation, at perfusion of the coronary circulation and with respect to the exact positioning of the heart-lung block in the abdomen of the recipient. The experimental model may be used for strictly immunological researches or for physiology, pharmacology and haemodynamics research.

Microsurgical treatment of lymphedemas of the limbs.

Lymphaticovenous anastomosis for surgical treatment of lymphedema of the limbs has been performed for several years with good results. The authors report on their experience, gained since 1976, after having operated on primary or secondary lymphedemas (five upper and 15 lower limbs). Lymphaticovenous anastomosis was performed according to Degni's technique using a special needle by which lymphatic vessels are inserted in the vein and secured to its wall. Eighty-three per cent of the subjective results were good, and the percentage of mean volumetric reduction in the operated limbs was 43% in 16 controlled cases. The postoperative course of secondary lymphedemas of the upper limbs was sometimes complicated by sudden lymphangitis attacks. In dealing with primary lymphedemas, the best results were obtained when all the operated limbs had great volumetric reduction, allowing the patients to resume their normal activities. The authors conclude that, after a long period of preoperative anti-inflammatory and anti-microbial therapy and regardless of lymphographic patterns, it is always worthwhile to perform a surgical exploration of the crural lymph nodes.

Endoscopic ultrasonography for preoperative staging of gastric carcinoma.

BACKGROUND/AIMS: To assess the accuracy and limitation of endoscopic ultrasonography in preoperative staging of gastric cancer, we performed a prospective study on 99 patients. MATERIAL AND METHODS: Ninety-nine patients with gastric cancer had preoperative staging with endoscopic ultrasound (EUS) and CT. RESULTS: The depth of infiltration (T parameter) was correctly defined by EUS in 58/65 patients (89%). The lymph node involvement (N parameter) was correctly classified in 44/65 patients (68%), the sensitivity was 74% and the specificity was 54%. The most frequent cause of understaging T parameter was microscopic tumor invasion, whereas overstaging was due to peri-tumor inflammation. CONCLUSIONS: We believe that EUS is a reliable method, superior to all diagnostic tools, in the evaluation of locoregional extension of gastric cancer.

Primary multiple epithelioid leiomyosarcoma of the liver.

Sarcomas represent only 1-2% of primary malignant tumors of the liver. We report the twenty first case of primary hepatic leiomyosarcoma in the literature. Metastases from gastrointestinal tract, female genital system and lung have to be excluded before confirming diagnosis of a primary neoplasm. Extensive involvement of the liver parenchyma contraindicated surgery, the treatment of choice when the tumor is solitary or multiple but located in one lobe. Chemotherapy, as employed in our patient, is the alternative therapeutic option in inoperable cases, but its role has to be clarified.
