[Prevalence of type 2 diabetes in the preoperative assessment interview].

OBJECTIVES: To determine the prevalence of type 2 diabetes in preoperative anesthetic assessment interviews during May 2004 and to compare the results to prevalences reported in other studies. PATIENTS AND METHODS: Patients over the age of 20 years who received an assessment interview were included. Type 1 diabetics, pregnant women, and critically ill patients were excluded. We followed the 1997 diagnostic guidelines of the American Diabetes Association. RESULTS: The prevalence of previously diagnosed type 2 diabetes was 10.3%. Three patients were diabetics unaware of their disease. The overall prevalence of type 2 diabetes was 11% in this population. CONCLUSIONS: Type 2 diabetes has a high prevalence among surgical patients in our hospital, higher than that reported for the general population based on other studies.

[Alkaline encrusted cystitis: factors affecting prognosis].

Alkaline encrusted cystitis is characterized by the formation of calcareous plaques in the bladder mucosa and submucosa in the presence of several factors: infection by urolithic organisms, earlier pathology of the vesical wall and precipitation of calcium phosphate salts. Treatment includes: urine acidification, correction of the urinary infection and use of acetohydroxamic acid. This paper presents three patients where the response to medical treatment was conditioned by the evolution of the existing vesical impairment. The conclusions state that in the alkaline encrusted cystitis, the concurrent pathology is a major predictor of the disease's future evolution.

[Adenomatous polyps in colon and rectum after cholecystectomy].

The frequency of rectal and colonic polyps was compared in 33 patients with history of cholecystectomy and 37 patients without it. The data were evaluated retrospectively from our medical bank records. We found a major incidence of adenomatous polyps in the group of patients with cholecystectomy, the same in females as in males, with a female prevalence in 2.7:1 ratio in contrast with the group without surgery, where the ratio was 1.5:1 with a male prevalence. We didn't find any association between cholecystectomy and colorectal cancer. All the patients with adenocarcinoma were in the group without cholecystectomy.

[Gastric cancer and routine upper digestive endoscopy].

In this paper we analyzed 5,401 upper G.I. endoscopies performed in a period of 5 years (1985-89), in the Gastroenterology Service at Hospital Jesús Yerena, in Caracas, Venezuela. We found 53 cases of stomach cancer, only 2 of them were early gastric cancer. We studied personal and epidemiological data, symptoms, place of the lesion, treatment, surgical mortality, macroscopic morphology and histopathology. We discuss the results concluding that: cancer of the stomach is a prevalent disease in our Hospital; that fibrogastroscopy is a very good diagnostic procedure; that we must organize a Program of Early Detection for this disease and that we should improve the Oncological Register.

[Epithelial tumors of the upper urinary tract].

The paper refers to thirteen patients with epithelial tumours of the upper urinary tract, diagnosed and treated at our unit during the last five years and emphasizes the need of early diagnosis, custumorized therapy and close follow-up. Evolution is clearly more favourable in well differentiated and low stage tumours, regardless the type of surgery performed, versus the poor results obtained in infiltrative and undifferentiated cases for which there is no effective treatment available. Results are analyzed based on a brief literature review and considering the short number of cases contributed.

[Urethral diverticulum in women. Echographic diagnosis].

Review of clinical, radiological and ecographical data observed in 11 cases of diverticulum of urethra in women. Mictional cystourethrography was performed to all patients and acography through suprapubic route to 7 of them. The results from the radiological study and ecography findings are discussed; also the differential diagnosis with other masses in the same location.

[Nephrogenic adenoma in urethral diverticulum in women].

Nephrogenic adenoma is a benign metaplasic lesion of uncertain aetiology. The tenth case of this curious entity in a diverticulum of urethra in women is presented here. The probable factors which intervene in its pathogenesis are discussed in this paper, including a short review of the literature.

[In situ tumor excision of a renal neoplasm].

The present work reviews the conservative surgical options to treat renal tumor formation in patients who still have both kidneys, and tries to clarify the diagnostic aspects, indications and therapeutical alternatives. The controversial items are analyzed versus treatments advocating a radical surgical approach, establishing a parallelism with current trends of surgical non-intervention in renal traumatism of moderate to severe condition (grade II). A case is presented where these two pathological circumstances are concurrent: Renal traumatism with complete parenchymatous rexhis and contralateral solid renal mass, both treated conservatively. Pre-surgical diagnosis was left renal traumatism grade II and righ renal neoformation and was based in findings with CAT and arteriographia. Histology of the piece from tumorectomy was renal angiomyolipoma. After a post-surgical evolution of twelve months, therapeutical results were assessed as excellent, with no evidence of either immediate not late complications. Considering data recently communicated by most authors, the concept of simple tumorectomy or renal tumoral enucleation is suggested as valid. However, arguments both clinical and surgical, in order to confirm its indication, require very rigorous criteria and a careful selection of patients.

[Cysts of the seminal vesicles. Diagnosis and treatment].

Congenital cysts in the seminal vesicle represent a rare by illustrative example of embryological malformation. They are often associated with ipsilateral urinary tract anomalies. We have analyzed the diagnostic and therapeutic schemes followed in the study of this uncommon pathology.

[The pediatrician and anorexia nervosa. Biological aspects].

Describing a case of anorexia nervosa the Authors review the clinical manifestations and organic alterations of this disease. The Authors point out the signs and symptoms which will usually suggest a diagnosis of anorexia nervosa. The course of the disease is monitored by the metabolic and hormonal analyses and the accompanying psychological features are underlined. The Authors present their therapeutic approach and emphasize the role of psychiatric treatment.

[Leiomyoma of female urethra].

Smooth muscle tumours located in the female urethra represent a pathology that is not observed very often. We offer two new cases and analyse the clinical aspects, differential diagnosis and treatment of this entity.

[Complications in kidney transplant: our experience in 107 cases].

We introduce our case material on urological complications occurred in 107 renal transplants, 102 of which were from corpse donors and 5 form live donors. The techniques used for the reconstruction of the urinary tract were: extravesical ureterocystoneostomy (91 = 85%), pyelo-pyelic anastomosis (15 = 14%) and uretero-ureteral anastomosis (1 = 0.9%). Sixteen cases presented urinary fistula (15%), emphasizing the high percentage of extravasations occurred in the pyelo-pyelic anastomosis (5/15). The resolution of the problem with graft preservation was achieved in 11 cases (68.75%). We encountered 6 ureteral obstructions that resolved favourably in 100% if the cases. Other important complications were: appearance of lymphocele in 8 cases, detection of post-grafting ureteral stenosis in 5 patients, finding asymptomatic vesicoureteral reflux in 16 grafts, and presence of urinary lithiasis in 2 cases. Similarly, we analyzed our series of 16 vascular complications, emphasizing that 15 out of 107 grafts had arterial or venous vascular abnormalities (14.1%), that forced to perform bench surgery in two occasions. Our statistical analysis showed that arterial thrombosis was more frequent than stenosis (8.49% versus 4.71%), and in nearly 80% of the cases (7/9) it happened in grafts concerning more than one arterial vessel, including in this series thrombosis of any of the ramus. The largest loss of grafting were due to this complication, since in 5 out of 9 cases of arterial thrombosis (55.5%) a transplantectomy had to be performed. With regard to venous complications thrombosis is a relatively rare complication, representing in our series less than 1%, usually associated to uncontrollable vascular acute rejection.

[Retrovesical pelvic masses: diagnosis].

We present two cases of pelvic masses with retrovesical localization: one consisting of a mature cystic teratoma and the other in a ganglionic metastasis of a subclinical prostate adenocarcinoma. We carry out an analysis of the clinical aspects as well as a review of the diagnostic guidelines to be followed in the light of the existence of a neoformation in this location. We analyse the possible differential diagnoses to be established with these tumours, which we should take into consideration despite their rare incidence and form of presentation.

[Intra-diverticular adenocarcinoma of the urethra in women].

We report on a female patient who presented with urethrorrhagia and a palpable urethral mass which was diagnosed as urethral diverticulum, the site of a tumor that proved to be adenocarcinoma of the urethra. The patient underwent radical urethrectomy and pelvic and inguinal lymphadenectomy. Urethral reconstruction was performed using a vesical flap following the technique described by Tanagho. Thereafter, she received 6 courses of M-VAC polychemotherapy. Diverticulum and carcinoma of the female urethra are uncommon. Adenocarcinoma is the most common tumor type encountered in these diverticula. Diagnosis is clinically simple and treatment is by radical surgery and radiotherapy. The foregoing treatment modalities have improved patient survival when the tumor is detected early. This tumor type is particularly aggressive and outcome depends on tumor stage.

[Chlamydial proctitis in homosexual men].

We present 3 homosexual male patients, with lesions confined to the rectum, produced by chlamydia trachomatis. In the 3 patients the lesions were confined to the first 10 cm from of the anal margin, they were nodular, ulcerated and with stenotic tendency, difficult to differentiate macroscopically from a neoplasm. Multiple biopsy specimens from all patients reported chronic unspecific proctitis. In the 3 patients the presence of chlamydia trachomatis was confirmed by staining with lugol and giemsa from samples obtained by rectal smear, two of them presented simultaneous infection by neisseria gonorrhoeae, all were positive for HBsAg and one for HIV. Symptoms were tenesmus rectal urge, pain, thin feces and mucosanguinolent discharge. Treatment with 100 mg. Doxycycline BID for 21 days resulted in total remission in two patients; one patient with significant clinical improvement needed rectal bougienage.

[Vesical actinomycosis: rare tumor of the urachus].

A female patient of child-bearing age using an intrauterine device (IUD) consulted our department for a hypogastric mass which turned out to be actinomycosis of the bladder. Antibiotic treatment resolved the infection and removal of the inflammatory mass was unwarranted since infected fistulous tracts had been observed. There appears to be a causal relationship with IUDs, as in genital actinomycosis. Due to the increasing use of contraceptive devices, we are likely to find this type of infection, which might be difficult to distinguish from malignant disease, increasingly frequent in urologic practice.

[Syringocele of Cowper's gland].

The dilatation of the duct of Cowper's gland is known as Cowper's Syringocele, a rarely occurring pathology, usually of a congenital origin. We offer two new cases corresponding to a perforated Syringocele and a non-perforated Syringocele. We analyse the clinical, radiological and therapeutic aspects of this entity.

Centronuclear myopathy with unusual mitochondrial abnormalities.

The case of a 34-years-old man is described with a progressive myopathy characterized by limb weakness and atrophy, involvement of facial, masticatory and extraocular muscles. The prominent features of the muscle biopsy were the presence of centrally located nuclei in most fibers. There was also an atrophy and predominance of type I fibers. Both clinical and morphological features were consistent with the diagnosis of centronuclear myopathy. Electron microscopic studies showed the presence of mitochondria with paracrystalline inclusions near the centralized nuclei but not in the subsarcolemmal position. This hitherto unreported feature led the authors to re-evaluate the hypothesis on the pathogenesis and the nosological classification of this myopathy.
