[Diffuse lung hemorrhage and necrotizing glomerulonephritis in a case of Wegener's granulomatosis. Diagnostic and therapeutic approach].

We present the case of a 59 years old male with Wegener's Granulomatosis with uncommon manifestations such as diffuse pulmonary hemorrhage and acute renal failure due to necrotizing glomerulonephritis. Neutrophil anticytoplasmic antibodies determination was negative. Conventional and high resolution thoracic computed tomography showed cavitated lung nodules with small peripheral vessels. These lesions, that are characteristic of this type of vasculitis, were not appreciated on the routine chest roetgenogram. Definitive diagnosis was made by the histological study of open lung and renal biopsies. Favourable response to corticosteroids, immunosuppressive drugs and hemodialysis was obtained. Diffuse pulmonary hemorrhage is an uncommon manifestation of Wegener's Granulomatosis, and must be considered as a vital emergency that justify the use of aggressive diagnostic and therapeutic methods.

[Pulmonary lymphangiomatosis associated with renal angiomyolipoma. Presentation of a case and review of the literature].

The case concerned is one of lymphangiomyomatosis (LAM) in a woman aged 39, that after a episode of spontaneous pneumothorax was subject to medical research for progressive dyspnea of medium efforts and bloodstained sputum. High resolution CT showed numerous thin-walled cystic airspaces of varying sizes distributed diffusely throughout the lungs. The abdominal CT and ultrasound developed multiple right renal angiomyolipomas. The histopathological study of the sample obtained by open lung biopsy was conclusive of LAM. The main feature of this rare disease, which occurs almost exclusively in women of reproductive age, is the abnormal proliferation of immature smooth muscle at the level of the distal airway, small blood vessels and lung lymphatic system, including sometimes the mediastinal and retroperitoneal lymphatic system. The clinical, functional, radiological and differential diagnostic aspects with entities histopathologically similarities are discussed and the therapeutic possibilities are reviewed.

[Paratesticular rhabdomyosarcoma].

Presentation of one case of embryonic paratesticular rhabdomyosarcoma in a 19-year old male. Following high inguinal radical orchiectomy, he required hemiscrotectomy, and it was found to be a foreign body granuloma. Prior to staging lymphadenectomy, therapy was complemented with radiotherapy and polychemotherapy according to a VAC protocol. The patient shows clinical remission at two and a half years after being diagnosed and one and a half years after completion of treatment.

[Trimethoprim-sulfamethoxazole++ in the treatment of a case of localized Wegener's granulomatosis].

A 50 year old man, affected by a Wegener's granulomatosis limited to lung, is presented. The indirect immunofluorescence test for "Anticytoplasmic Antibodies" (ACPA) was negative and we got the definitive diagnostic by the histological study from thoracotomy and biopsy samples. We started treatment with trimethoprim-sulfamethoxazole and a good evolution with an adequate clinical control, x-ray study and blood test was obtained. Some aspects of the value of the ACPA as diagnostic and activity markers of the disease and also the use of an antimicrobial agent on the treatment of this nosological entity are discussed.

Ultrastructure and light microscopical study of a Leydig cell tumor of the testis associated with bilateral gynaecomastia.

Light and electronmicroscopic study of a Leydig cell testicular tumor in an 18-year-old male is presented. Bilateral gynaecomastia and normal hormonal blood levels were found. Emphasis on the diagnostic value of electronmicroscopy is remarked upon, based on the following ultrastructural characteristics of the cells; 1) Ovoid shaped nuclei with ondulating contours and dispersed and homogeneous chromatin, 2) Rich agranular endoplasmic reticulum with frequent special modifications, such as membranous whorls with a central cytoplasmic mass or lipid droplets, 3) Numerous mitochondria with occasional tubular cristae, 4) Numerous lipid vacuoles. Other structures also identified in this tumor are Reinke crystalloids, cytoplasmic microbodies, myelin figures, gap-type junctional complexes and paracrystalline inclusions of Payer type E, which are less common.

[Pulmonary toxicity of amiodarone].

A case of lung disease due to amiodarone is described based on epidemiological, clinical and evolution criteria, bronchoalveolar lavage (BAL) analysis, X-ray and pathological study by optic and electron microscopy. Iodine on pulmonary tissue quantified spectrophotometrically was 2 p.p.m. This was not observed in normal lung tissue. The best diagnostic criteria for amiodarone pneumopathy are discussed highlighting lymphocytosis in the BAL and the presence of dense lamellar bodies in the ultrastructural study of the affected lung tissue.

[Eccrine poroma: its histogenesis and review of the literature].

It has been done an analysis of a case of eccrine porome, based on the morphology, considering the actual tecnical publications on the matter. The morphogenesis of the formation of this nevoid tumor is established by the continuity between the main body of the tumor and the underneath eccrine sweat glands. These glands present signs of hyperplasia, which in the highest levels present destructuration of the excretory duct and morphological modifications of the cells which are conditioned by the functional adaptation at different levels. In this way there is proliferation of structures, similar to the different parts of syringuium and acrosyringuium producing this hamartomatous neoformation.
