Blood pressure monitoring from a biostatistical and clinical viewpoint for predicting fetal distress in preeclamptic pregnancy.

With technological progress, blood pressure monitoring has become a procedure largely applied in medical practice but not adequately systematized under a biostatistical viewpoint. This paper shows the complexity and importance of the biomathematical approach and its clinical implications in preeclamptic pregnancy. Data series were analyzed by means of noninferential and inferential procedures of biostatistics. A progressive gain of information is obtained, the inferential procedures showing an analytical resolution able to discriminate fetal distress in preeclamptic pregnancy.

Cholinergic nerves in mouse uterus.

The topographical distribution of cholinergic nerve fibres in the uterus was studied in normal as well as chemically sympathectomized mice. Our analysis was performed using both transverse sections and whole mounts, in order to analyse the distribution of cholinergic nerves within the various layers of the uterus and to study the morphology of nerve plexuses that supply the organ. Cholinergic nerve fibres were found chiefly around the uterine artery and its primary ramifications. Nerve fibres coming from arterial plexuses are distributed to the myometrium and more infrequently to the endometrium. Groups of acetylcholinesterase positive ganglion cells were located in the cervix. The distribution pattern of the cholinergic innervation of mouse uterus shows important topographical differences. In the tubal end of uterine horn the innervation is poor and cholinergic nerve fibres are organized in a sparse plexus. The number and the density of uterine nerves increase gradually proceeding toward the cervix. The cervix shows the richest innervation. The findings that observed nerve fibres are unaltered by chemical sympathectomy and are revealed using short incubation times suggest their parasympathetic nature.

[Injectable progesterone in hormone replacement therapy in menopause].

PURPOSE: To evaluate activity and tolerability of intramuscular administration of 17-alpha-hydroxyprogesterone caproate (Lentogest-Amsa) in hormone replacement therapy in menopause. PATIENTS AND METHODS: Intramuscular slow releasing 17-alpha-hydroxyprogesterone caproate was given to 30 postmenopausal women. Patients were divided into two groups according to the number of monthly doses (1 or 2). RESULTS: In both groups of patients a regular monthly bleeding was obtained. Echographic assessment did not show abnormal endometrial thickness. CONCLUSIONS: The results of this study have pointed out the efficacy of intramuscular progesterone-caproate administration with a better compliance in the single dose group.

[Lifestyle and drinking habits during pregnancy].

BACKGROUND: It has been reported that a high consumption of alcohol during pregnancy is correlated to fetal alcohol syndrome and other perinatal problems. Less is known about the effects linked to moderate and limited drinking during pregnancy. METHODS: A study was carried out in 140 pregnant women: the mean age was 33.05 +/- 5.22, whereas the gestational age was 28.56 +/- 12.1 weeks (range 5-42). Each woman was asked to compile a form giving personal details and details of psychosocial behaviour, a lifestyle questionnaire (CAGE) and a form entitled "How much do you drink" based on SQQ. RESULTS: 22.2% of women reported that they had drunk alcohol during the month prior to the interview. Of these, 15% had not altered their habits, 0.8% had increased their alcohol consumption and 6.4% had reduced it. Wine was the form of alcohol most frequently consumed. Only one woman was positive to the SQQ test, and only one pregnant woman was positive to CAGE. CONCLUSIONS: This study shows that dietary behaviour was correct in the majority of cases.

[Urinary incontinence in pregnancy and prevention of perineal complications of labor].

BACKGROUND: Female urinary incontinence often manifests itself for the first time during pregnancy in a great number of women (23-50%). In many cases it is transitory in nature, the result of the stress induced by the pregnancy on the pelvic floor. In this study the authors discuss the frequency of urinary incontinence on the basis of a questionnaire administered to a limited sample group of obstetrics patients in order to identify the potential risk factors and to plan protective and rehabilitative strategies for the pelvic floor both during pregnancy and before and after delivery. METHODS: From November 1998 until June 1999 at the Obstetrics and Gynecology Clinic of the University of L'Aquila (Italy) 150 women were surveyed by written questionnaire both before delivery and on the second day following delivery. Those who were identified as having urinary incontinence during pregnancy were readministered the questionnaire at three months following delivery. RESULTS: Urinary incontinence was reported during pregnancy by 48% of the women under examination, which appeared, for the most part, during the third trimester of the pregnancy (in 43% of the women). No differences related to age were discovered, while a cesarean section played a protective role. For women with preexisting IUS the new pregnancy was an aggravating factor. Of the women with IUS during pregnancy who did not demonstrate preexisting symptoms only 9.4% reported persistent urinary incontinence at three months following delivery. Among women pregnant for the first time with persistent symptoms all had received episiotomies by the third month following delivery. CONCLUSIONS: The authors of this study wish to direct their attention to the identification of women with IUS during pregnancy so that damage from delivery can be reduced to a minimum and rehabilitation strategies for the functioning of the pelvic floor can be imitated as soon as possible.

[Effects of estradiol on in vitro cultures of human fibroblasts].

This paper assesses the effect of estradiol on the in vitro growth of fibroblast taken from human breast skin. It was observed that estrogens stimulate the proliferation of phenotypically adult fibroblasts; that there is a threshold increase of hormonal concentration which can trigger off this process; and that the size of the single dose is more important than the number of doses of hormone received.

[Vulvar lesions caused by Papillomavirus (HPV): effectiveness of thymopentin (Syntomoduline). First results and experiences].

The study included two different sized groups of patients suffering from various degrees of valvular microcondylomatosis. A group of 40 patients was treated using combined therapy while a second group of 20 patients received immunomodulating therapy alone with Sintomodulin. In particular, possible changes in the T-cell line of the immune system and correlations between the viral lesion and variations in serum levels of lymphocyte subpopulations were assessed. Results were analysed up to one year after the end of therapy.

Determination of cervicovaginal microorganisms in women with abnormal cervical cytology: the role of Ureaplasma urealyticum.

OBJECTIVE: To evaluate the existence of an association between cervicovaginal infections and precancerous lesions of the uterine cervix, through determination of prevalent cervicovaginal micro-organisms, alone and in association with human papillomavirus (HPV), in patients with abnormal and normal vaginal cytology. PATIENTS AND METHODS: Patients with abnormal vaginal cytology were divided into three study groups according to cytological findings: ASC-US, L-SIL and H-SIL. All patients underwent colposcopic examination and exoendocervical and vaginal sampling for microbiological and molecular analysis for detection of HPV-DNA, Ureaplasma urealyticum, Chlamydia trachomatis, Trichomonas vaginalis, mycetes and common bacteria. Results were compared with the patient group asymptomatic for cervicovaginal inflammation with negative vaginal cytology and colposcopy. RESULTS: A high association between Ureaplasma urealyticum infection and the grade of cytological cervical lesion (27% for ASC-US, 35% for L-SIL and 45% for H-SIL) was found. Furthermore, 19% of the control group samplings were positive for Ureaplasma urealyticum, significantly less than that observed in the positive cytology groups. An interesting association of HPV with Ureaplasma urealyticum in patients with H-SIL vaginal cytology (83%), much higher than that observed in patients with slightly abnormal or normal vaginal cytology (56% for ASC-US, 49% for L-SIL, 40% for normal cytology) was also identified. In contrast, the association between Papillomavirus and multiple microorganisms seemed to decrease with the level of cellular dysplasia in 30% of controls, 33% of ASC-US, 32% of L-SIL and 17% of H-SIL. CONCLUSION: The presence of a high Ureaplasma urealyticum level seems to be a cofactor of HPV infection, a necessary cause of precancerous lesions of the uterine cervix. The presence of Ureaplasma urealyticum may play a role both in initiating viral cellular anomalies and in viral persistence. It can be hypothesized that these initial processes are helped by a state of cervical inflammation, also supported by multiple microorganisms. It would, thus, be suggested for all patients who present with an abnormal PAP test to undergo a cervicovaginal microbiological examination to detect potentially pathogenic microbes for correct diagnosis and treatment, as well as a more complete follow-up of cervical cytological lesions.

[Evaluation of the endometrium by vaginal ultrasonography].

A group of 131 women aged between 35 and 70 years old were examined using endo-vaginal ultrasonography. Of the patients examined, 45 had been in menopause for at least 2 years and 86 were pre-menopause. 48.9% of the post-menopause patients reported loss of blood and 69.8% of pre-menopause patients reported alterations in the menstrual cycle. Ultrasonography was performed using an endo-vaginal 5 MHz (Ansaldo 600) probe with the patient in a dorsosacral position. During the course of ultrasonography the appearance of the endometrium, in particular its thickness, dyshomogeneity and, in pre-menopause women, its correspondence with the stage of the ovarian cycle, were examined. Taking these parameters into account, the type of endometrial echo patterns were classified, separating the cases into pre- and post-menopause groups. Of the women in menopause, 22 underwent curettage due to symptoms, independently of endometrial thickness which was over 4 mm in 50% of cases, 3 women underwent curettage following ultrasonographic indications, whereas the remaining 20 underwent hysterectomy due to other pathologies. Of the pre-menopausal group, 10 underwent curettage following ultrasonographic indications and 42 due to symptoms; the remaining patients underwent hysterectomy due to pathologies not related to the endometrium. A clearly defined role of endovaginal ultrasonography in the diagnosis of endometrial pathologies emerges if ultrasonographic findings are correlated with histological ones. In the group of post-menopausal women (45), all those cases which were histologically classified as endometrial carcinoma (4) were associated with an endometrial thickness over or equal to 4 mm.(ABSTRACT TRUNCATED AT 250 WORDS)

[Colposcopic monitoring of primary chemotherapy in patients with locally advanced cancer of the uterine cervix].

OBJECTIVE: To evaluate colposcopy in the monitoring of patients affected by locally advanced cervix cancer treated with primary chemotherapy. MATERIALS AND METHODS: Eight patients (42-54 years old) affected by squamous cervical cancer locally advanced (stage FIGO IIa "bulky"/IIb/III) were treated with primary chemotherapy (cisplatin 50 mg/m2 vincristin 1 mg/m2 bleomicin 30 UI in 24 hours continuous infusion q 7 days for 3 cycles). Colposcopy was performed before chemotherapy, after the second cycle and at the end of chemotherapy. RESULTS: Two pathological complete responses, 5 partial responses and one case of stable disease were observed. Patients with complete and partial response were treated with radical hysterectomy. In responding patients we observed a typical sequence of colposcopic findings. Regression of distinctive malignancy feature, reduction of atypical vessels, gradual disappearance of peripheral lesions and centripetal reepilization of the cervix were sequentially registered. At the end of treatment the cervix appeared with a colposcopy imaging as well as a centripetal squamous complete or incomplete metaplasia, open--for most part--glandular exit and weak acid-iodic positivity. CONCLUSION: In responding patients we observed a typical sequence of colposcopy finding, useful in order to evaluate the effects of chemotherapy and to programme the surgical approach.
