[Cystic lymphangiomas of the mesentery and the epiploon. About two cases].

Two cases of cystic lymphangiomas of the mesentery and the epiploon are hereby related. The authors underline the following points: first, the rarity of these tumors along with the absence of significant clinical features makes the differential diagnosis with ovary tumors a difficult task. Second, although abdominal ultra sonography, completed by CT scan, allows the hypothesis of lymphangioma, only histological examination will confirm the diagnosis. There is no other treatment but complete exeresis, with a good prognosis and exceptional recurrences.

[Pelvic lymphocele: report of a case and review of the literature].

Lymphadenectomy for pelvic cancer can lead to complications, particularly lymphocele. We report a case of pelvic lymphocele, which occurred in a patient who underwent surgery for stage IIa cervical carcinoma after preoperative radiotherapy. The intervention consisted in colpohysterectomy, with lymphadenectomy without peritonisation. Five months later she developed dysuria and pelvic pain. Ultrasound and computed tomography showed a pelvic lymphocele complicated by renal insufficiency. Kidney function was re-established after intraperitoneal marsupialisation. One year later the patient was in good clinical condition with no disorder of kidney function.

[Contribution of ultrasonography in thyroid diseases. Apropos of 100 cases].

Thyroid disease is a very common disease, essentially affecting women, and raises diagnostic and therapeutic problems. The objective of this study was to specify the value of ultrasonography in the diagnosis and aetiological orientation of these lesions, based on a retrospective study of 100 patients. The results of ultrasonography were compared to the histological results. The aetiologies detected were dominated by benign lesions, particularly adenomas (56 cases), dystrophic goitres (32 cases) and cancers (7 cases). Ultrasonography allowed the detection of clinically impalpable nodules with no isotope scan signs in 11% of cases. The ultrasonographic appearance of benign nodules was variable. Solitary nodules were detected in 63 cases. They were hyperechoic in 15 cases, isoechoic in 8 cases, cystic in 9 cases, and mixed in 31 cases. A peripheral clear halo was revealed in 25 cases and macrocalcifications were present in 17 cases. Malignant lesions were visualized in 7 cases and were solitary in 5 cases. Solitary lesions had a hypoechoic echostructure in 1 case, an isoechoic echostructure in 2 cases and a mixed echostructure in 2 cases. The margins were poorly demarcated in 3 cases, and circumscribed without peripheral halo in 2 cases. Cervical lymphadenopathy was detected in 2 cases. Graves' disease, diagnosed in 2 patients, showed a diffuse hypoechoic appearance of the entire thyroid gland. Ultrasonography is a sensitive morphological method for the diagnosis of thyroid lesions. A detailed and precise analysis of the ultrasound signs of the lesion can suggest that benign or malignant nature of the lesion, which can be completed by Doppler-ultrasound and especially ultrasound-guided needle biopsies.

Modulation of IgE production in the mouse by beta 2-adrenoceptor agonist.

The present study examined the in vitro and in vivo effect of salbutamol on IgE production in the mouse. The present results show that salbutamol potentiates the in vitro interleukin 4 (IL-4)-induced IgE production from lipopolysaccharide-activated murine B lymphocytes. This effect is dose-dependent and is observed at concentrations above 10 nM. In vivo, when ovalbumin (OA)-sensitized BALB/c mice were treated with a daily injection of salbutamol, an increase of the anti-OA IgE levels in the serum was observed as compared to sensitized animals. Such an effect was observed at doses above 1 microgram/kg and was maximal at 10 micrograms/kg. Treatment of sensitized mice with salbutamol increased the ex vivo production of IL-4, IL-5, IL-6 and IL-10 from concanavalin A-activated splenocytes whereas no modification of IFN-gamma synthesis was noticed as compared to nontreated sensitized control mice. These results demonstrate that beta 2-adrenoceptor agonist stimulation results in an increase in IgE production both in vitro and in vivo in the mice. At least in vivo, they also suggest that the effect of this drug could be explained by an increase of the production of Th2-type lymphokines.

[Krukenberg's tumor: still a somber prognosis. 4 cases].

The authors report four cases of Krukenberg's tumour. They point out its rarity and that the diagnosis can come as a surprise on histological examination. The tumour always has Signet ring cells. How the ovary comes to be involved is not known as yet. Treatment is mainly surgical with added chemotherapy but the prognosis is still very poor indeed.

[Pelvic echinococcosis in women. Two case reports].

The purpose of this report of two cases of pelvic hydatid disease is to recall the aetiology and pathogenesis of this endemic condition in Morocco. We point out that because it gives rise to many different clinical signs it is difficult to diagnose accurately but if the appropriate blood tests are carried out in every case with a pelvic mass the diagnosis could not be missed. It is important to treat effectively this condition which can compromise both the gynaecological and obstetrical future of the patient and that can have serious repercussions extending to the upper urinary tract. The authors, too, show how important it is to follow up these cases after operation in order to spot recurrences.

[Pyloroduodenal stenosis of ulcer origin. Apropos of 260 cases].

This study is about 260 cases of ulcerous pyloroduodenal stenosis operated on in 10 years among 1209 ulcers (21.5%). Males are by far dominating with a rate of 83.55%, the average of age was 45 years. 20% of our patients have presented a complication anterior or contemporary to the stenosis (perforation or hemorrhage). Clinical context is obvious and upper gastro intestinal series has made diagnosis in 92% of cases fibroscopy has been done in only 30.8% of cases. Surgical treatment has consisted in a troncular vagotomy in 252 cases. (98.2%), transduction de "dont" 34.5% of whom underwent a gastroduodenostomy 25.8%, gastrojejunostomy, 18.2% a pyloroplasty and in 21.4% an inferior polar gastrectomy. There were 3 fundic vagotomies (1.17%) associated to a gastro duodenostomy in 2 cases and to dilatation in one case e deplored 3 deaths related to surgery (1.1%) long follow up was marked by 3 ulcerous relapses and 2 stenosis.

[Our experience of surgery for common bile duct lithiasis. Apropos of 370 cases].

We report a series of 370 common bile duct stones in which the diagnosis was made preoperatively in 56% of cases. We performed: choledochotomy with Kehr drainage in 70% of cases, biblio-intestinal anastomosis in 20%, transcystic extraction with drainage via the same route in 7%, surgical sphincterotomy in 2%. The global mortality was 2.7% related to preexisting deficiency states. The residual stone rate was 5.6%.

[Ovarian struma: 2 cases].

The authors report two cases of ovarian goiters; they insist upon their rarity and the difficulty of their pré-operatory diagnosis. Only histologic examination can affirm their goitrous origin. The treatment of ovarian goiters is surgical. The evolution is favorable except their exceptional malignant transformation.

[Hydatid cysts of the hepatic dome. Apropos of 124 cases].

The authors report a series of 124 hydatic cyst of hepatic dome. The discovery circumstances are represented by evolutive complication: infections in 58%, angiocholitis 45%, vascular compression 7.2% and bilio-bronchic fistulas in 6.4%. The treatment realised was the resection of the bulging dome in 87%, of cases parietal perikystectomy was realised in 9.6% and total perikystectomy in 3.2%. They deplore 12 deaths (9.8%).

[Peritoneal echinococcosis. Diagnostic and therapeutic problems. Apropos of 34 cases].

By the light of retrospective study of a series of 34 cases of peritoneal echinococcosis the authors are reminding epidemiological, etiopathologenical, diagnostic difficulties and treatment of this endemic affection in their country. In their study they noted a frequency of 4.5%. With a female predominance (73.5%), the main age was 47 years 80% of the patients were noted to have a rural origin. The treatment was conservative for the primitive kyste as for the peritoneal locations. The post operative course was uneventful in 76% of cases and complicated in 24% (subphrenic abscess in 5 cases and cholerragy in 2 cases). They deplored 3 deaths the follow up was assured clinically; echographically and biologically; they have reoperated 9 relapses.

[For a conservative treatment of hydatid cyst of the spleen].

A retrospective study of 45 cases of spleen hydatic cyst. Operated on between January 1980, and December 1989 permitted the following commentary. Splenic hydatic cyst is characterized by its rarity and latency. It's diagnosis gives no rise to any problems. Echography data are essential in asymptomatic froms associated to other localisations. Surgical treatment can not be summarised to splenectomy; conservative methods are used as long as possible.
