[Verification of a surgical technique using a new biotitanium cage in degenerative diseases of the lumbar spine--experimental study].

PURPOSE OF THE STUDY: Authors performed experimental PLIF operations on cadavers to verify appropriate shape and size of cage made from bioactive titan material. The approach for application into intervertebral space of the lumbar spine and appearance on X-ray and CT scans was also investigated. MATERIAL: In experiment on cadavers five implantations of interbody cages were performed, two of them in interbody space L3/L4 and three of them in L4/L5. Subtotal discectomy and end platec preparation was done before cage insertion. Our type of cage is made from bioactive titan material and fastened in the application instrument that we developed for this purpose. METHOD: To verify operational approach we used midline incision and opened spinal canal through laminectomy, partial hemilaminectomy or partial medial facetectomy. This phase was followed by discectomy and end plates preparation. Stability of inserted cages in intervertebral space is ensured by rotation and anchorage of cage wings in end plates. Operated motion segment was taken out and evaluated by X-rays and CT scans. RESULTS: The operational approach through partial hemilaminectomy and partial medial facetectomy was fully sufficient for cage application with respect to operated segment. All implantations were successful and position of cages satisfactory. Thanks to less robust gripping instrument the approach was more thrifty compared to glass-ceramic cage and comparable with commercially produced cages that we have experience with (Spine Tech, Stryker, Comesa). Due to cage radio opaque, proper position of cages is easy to control on X-ray and CT scans and allows good peroperative monitoring by X-ray magnifier. DISCUSSION: Usage of bioactive materials in spinal surgery started by introduction of glass ceramics. Based on our experience we determined suitable indications. To eliminate some of te disadvantages of glass ceramics (mainly mechanical properties), a cage made from bioactive material was developed. This type of cage does not require bone harvesting to fill the cage. This fact results in better comfort for patient in postoperative period and eliminates complications from bone graft harvesting. Bioactive surface of the cage with osseo conductive and osseo integrative features creates prerequisites for solid fusion without bone grafts. Titanium material guarantees mechanical strength and makes possible to produce a wide range of shapes and sizes. Strength of the material enables more advantageous gripping of cage in application instrument. The cage is well visible on X-ray and in MRI scans artifacts are considerably reduced. Operational approach and technique are similar to other commercially produced cages and the extent of destabilization is limited to minimum. CONCLUSION: Experimentally we repeatedly verified operational approach, suitable shape and applicability of bioactive titanium cage into intervertebral space. For clinical use bioactive titanium could be a possible way how to replace bone grafts. For spinal surgeons it represents a chemically and mechanically stable material capable of interaction in an environment where it is implanted. Even in difficult conditions the level of osseo integration is high.

[The prevalence of idiopathic proctocolitis].

During screening of colorectal cancer by the Haemoccult test (Röhm Pharma Co.) the authors investigated the incidence of idiopathic proctocolitis in the population of the Bruntál district. The group of tested subjects comprises 16.642 of the population aged 45 to 86 years. After examination of 380 positive subjects (2.28%) twelve cases of idiopathic proctocolitis were detected. Of these twelve seven were identified previously and a new diagnosis was made in five subjects. At twelve cases were identified from a population of 16.642, the assumed prevalence in 1989 in the given age group is 72/100.000. This suggests considerable underestimation of the prevalence of idiopathic proctocolitis in current epidemiological surveys.

[Problems in the early diagnosis and treatment of colorectal carcinoma and adenoma in the aged].

In 1985-1989 in the Bruntál district by means of the test for occult haemorrhage (Haemoccult, Röhm Pharma Co.) a total of 1084 subjects aged 65-86 years were examined. The test was positive in 74 subjects (6.83%). A subsequent coloscopic examination was made in 60 subjects, 81% of those with a positive test. Coloscopic examination revealed 8 patients with carcinoma of the large intestine and 45 patients with an adenomatous polyp. The oncological positivity is thus 4.89% of the tested subjects and 88.33% of those subjected to coloscopy. The carcinoma was evaluated in two cases in the stage Dukes A, in four instances in stage Dukes B, in one case in stage Dukes C and in one case in stage Dukes D. In two patients with carcinoma another adenomatous polyp was found in the large intestine. In 14 patients of 45 with an adenomatous polyp another synchronous adenomatous polyp was detected (31.1%). 43 patients were treated by coloscopic polypectomy. Contemporary possibilities of early diagnosis of colorectal carcinoma in conjunction with effective therapeutic endoscopy are perspective in particular in patients of advanced age.

[Classification of colorectal adenomas based on the histological picture in patients without symptoms].

During screening of colorectal carcinoma by the test of occult haemorrhage in 1985 to 1989 in the Bruntál district during coloscopic examination of positive subjects a total of 277 adenomatous polyps of the large intestine were revealed. Adenomatous polyps were detected in 227 patients where by in 180 solitary polyps were involved and in 47 cases synchronous polyps. 222 adenomatous polyps were in the rectum and left half of the colon (80.1%). Histological examination revealed: tubular adenoma 153 cases (55.23%), tubovillous adenoma 92 cases (33.21%), villous adenoma 13 cases (4.69%), adenoma with intramucous carcinoma 12 cases (4.43%) and adenoma with invasive carcinoma 7 cases (2.53%).

[Primary hemangioendothelioma of the bone marrow].

A case of malignant haemangioendothelioma in a 61-year-old female is described. The tumour infiltrated extensively medullary spaces in a number of bones without their destruction. Extraskeletal neoplastic foci were found in the intestine and in a lymph node, liver and especially spleen were involved more extensively. Several diagnostic features of the tumour are listed together with its position in some recent classification schemes.

[Neuroaxonal dystrophy with delayed onset and protracted course (author's transl)].

An unusual case of neuraxonal dystrophy in a 24-year-old man has been described. The condition started at the age of 10 years with motoric disturbances, later the patient developed spastic quadruparesis and deep dementia in last two years. The disease appears to be of genetic origin. The morphology is characterized by the generalized presence of spheroids in the central nervous system. Additional findings of status pigmentosus and dysmyelinisatus do not exclude the case from being labelled neuraxonal dystrophy.
