Immunohistochemical staining techniques of intraocular tumors.

The peroxidase-antiperoxidase staining technique for the antigens S-100, neuron-specific enolase, and alpha 1-antichymotrypsin (alpha 1ACT) was applied to 57 intraocular tumors: 46 malignant melanomas of the uvea, seven retinoblastomas, and four tumors metastasizing to the eye. The staining characteristics of the different intraocular tumors were compared. Staining for S-100 in a fine-needle aspiration biopsy sample taken from a malignant melanoma of the choroid before enucleation of the globe was attempted. The positive staining of a few cells thus obtained suggested that this technique may be helpful in the diagnosis of melanomas. The alpha 1ACT stain used in this study has not been used previously in ophthalmology to our knowledge. We found 60% of malignant melanomas of the choroid stained positively. Another finding was the staining of the retinal pigment epithelium with alpha 1ACT in 30% of eyes with malignant melanoma of the uvea.

An immunofluorescence study of primary anetoderma.

Primary anetoderma (PA) has occasionally been described in association with lupus erythematosus (LE). The present study was performed to elucidate a possible causal link between PA and LE by the use of direct and indirect immunofluorescence (IF) methods. Two patients with PA were studied. Biopsy specimens were obtained from early inflammatory and atrophic anetoderma lesions and from the exposed and unexposed uninvolved skin of each patient. The pattern of immune deposits observed in one patient was indistinguishable from that which is often seen in systemic LE, and in the other patient from that which may be observed in chronic cutaneous LE. The direct IF study also showed fibrillar immune deposits in the dermis that resembled elastic fibres morphologically. The indirect IF study, however, failed to demonstrate anti-elastic fibre antibodies in the patients' sera. The results of this study and a review of the literature suggest that some cases of PA have direct IF findings similar to those of either chronic cutaneous or systemic LE. However, these findings, along with the serological findings, are insufficient to establish a diagnosis of LE in most of these PA cases.

A comparative histopathologic study of generalized and localized granuloma annulare.

We have reviewed the morphologic findings in 41 histologic slides of granuloma annulare (GA), 15 from localized (LGA) and 26 from generalized granuloma annulare (GGA). The most common pattern was the histiocytic infiltrative type, more so in cases with LGA than with GGA. The palisading granuloma pattern was present in 21.9% of patients and its prevalence was almost equal in both clinical types. The least prevalent histologic type in all patients taken together was the epithelioid nodule type. However, the prevalence of this type in LGA was equal to that of the palisading type, in contrast to that in GGA where it accounted for only 11.5%. In addition, a mixed histologic pattern was found in greater than 25% of sections from GGA and in only one section from LGA. Periodic acid-Schiff stain revealed hyalinized capillary basement membrane in most of GGA and only in approximately 50% of LGA sections. The variation in prevalence of the different histologic patterns between sections from LGA and GGA, and the difference between our findings and those observed in other series, are discussed.

Massive haematemesis--presenting symptoms of cystadenocarcinoma of the pancreas.

A 43 year old woman presented with attacks of abdominal pain, haematemesis and hyperamylasaemia. Gastrointestinal X-rays and repeated upper gastrointestinal endoscopy failed to reveal the source of bleeding. Ultrasound and computed tomographic scan demonstrated a calcified mass in the tail of the pancreas. Surgical exploration revealed a solitary mass in the pancreas and histological examination showed cystadenocarcinoma. The patient died 2 years later because of local recurrence, but haematemesis and melaena did not recur. This case presents an unusual manifestation of cystadenocarcinoma of the pancreas with massive bleeding from the tumour via the pancreatic duct and associated pancreatitis. Other possible reasons for bleeding with cystadenocarcinoma of the pancreas are discussed.

Autogenous skin and fascia grafts as topical hemostatic agents in splenic injuries.

Splenic salvage techniques were developed since the immunologic importance of the spleen has been recognized. Various synthetic products, such as Avitan, Collastat Gel foam, Superstat, Thrombostat, were used and lately even pig skin was tested for its hemostatic ability. In this study, a canine splenic bleeding model was used to test autologous split-thickness skin-graft hemostatic effect, compared to pig skin, human skin and canine fascia. Lyophilized pig skin was tested on 12 splenic wounds, lyophilized human skin on 10, canine skin on 10, canine fascia on 10 and simple pad gauze on 10 other splenic wounds. Each animal served as its own control. Pig skin was more effective than canine skin (p less than 0.01), but the canine skin was more effective than human skin (p less than 0.01) and canine fascia (p less than 0.05). Long-term implantation of the canine skin graft caused fibrosis and epidermoid cyst formation, but they were of no clinical significance in the dog. In conclusion, autologous split thickness graft, always at hand, was found to be an effective hemostatic procedure and proved to be safe in the dog.

Villous tumors of the stomach.

Four cases of gastric villous tumors are presented. The radiologic features of these rare tumors in the stomach are described. Their high propensity for malignancy as compared to villous adenoma of the colon is emphasized. In 2 of 4 of our patients the villous tumors were malignant. The radiologic picture of villous tumors of the stomach is characteristic enough to suggest the diagnosis.

Hypersensitivity vasculitis induced by terbutaline sulfate.

Hypersensitivity vasculitis developed in a patient six days after therapy with terbutaline sulfate was initiated. The casual association between the drug and hypersensitivity vasculitis is based on the temporal relationship, a positive lymphocyte transformation test with terbutaline sulfate, and exclusion of other causes. This is, to the best of our knowledge, the first report of a leukocytoclastic vasculitic reaction to terbutaline sulfate.

The contribution of anterior mediastinotomy in the diagnosis and evaluation of diseases of the mediastinum and lung.

Our experience with 62 consecutive patients who underwent anterior mediastinotomy is presented. A wide range of histological diagnoses was achieved. The highest yield was achieved for mediastinal masses. The overall diagnostic specificity was 64.5% and the diagnostic sensitivity was 98%. However, patient morbidity and mortality were 16.1% and 1.6%, respectively. Therefore, specific guidelines for the procedure are presented.

Effect of diclofenac sodium (Voltaren) and prostaglandin E2 on spermatogenesis in mature dogs.

The effect of the prostaglandin-synthesis inhibitor, diclofenac sodium (Voltaren), and of prostaglandin E2 (PGE2) on spermatogenesis in mature dogs has been studied. Diclofenac sodium at 2.0 mg/kg was given subcutaneously once a day for 42 days. The testes were biopsied every 7 days, and spermatogenesis was quantified showing a marked and statistically significant increase (p less than 0.001). Administration of PGE2 in doses of 2.0 mg/kg subcutaneously, once a day for 15 days, produced a marked and statistically significant decrease in spermatogenesis (p less than 0.001), and an increase in the numbers of exfoliated immature germ cells in the seminiferous tubule lumens.

Extreme unilateral hydronephrosis with normal glomerular filtration rate: physiological studies in a case of obstructive uropathy.

A 30-year-old man presented with massive left hydronephrosis caused by congenital stricture of the ureteropelvic junction. Glomerular filtration rate of the post-released kidney (inulin clearance and creatinine clearance) was preserved completely. Simultaneous individual kidney function tests showed the following values expressed as the ratio of post-obstructed to normal kidney: inulin clearance 142 per cent, urine flow rate 475 per cent, sodium clearance 221 per cent and urinary excretion of prostaglandin E2 617 per cent. Urinary osmolality was 257 and 791 mOsm./kg. in the obstructed and normal kidneys, respectively. Administration of indomethacin, a cyclo-oxygenase inhibitor, caused decreased urinary excretion of prostaglandin E2 in both kidneys, with drastic antinatriuresis manifested predominantly in the normal kidney. We conclude that the glomerular filtration rate may be preserved despite huge hydronephrosis, which suggests that the human kidney is salvageable even after a prolonged period of unilateral ureteral obstruction, and the contralateral normal kidney mitigates the volume losses from the obstructed kidney.

Changes in gastric parietal cells during prolonged intermittent corticosteroid treatment.

Immature A/J mice were treated for up to 7 weeks with intermittent doses of triamcinolone hexacetonide and were thereafter allowed to recover for 7 weeks. Structural examinations and morphological measurements were performed on the parietal cells in the gastric mucosa. By the 3rd injection a significant decrease was noted in the number of the above cells - a feature that lasted throughout the experimental period. In contrast, the diameter of the parietal cells increased. However, following recovery, the latter returned to their normal size.

Xanthogranulomatous appendicitis--an incidental finding of localized pathology.

The clinical, histopathological, and electron microscopic features of an unusual case of xanthogranulomatous appendicitis are reported. The patient, a 37-year-old female, presented with typical signs of acute appendicitis and the appendix appeared slightly dilated at laparatomy. The histopathological sections showed numerous xanthoma cells mixed with inspissated fecaliths. Electron microscopy disclosed the presence of xanthoma cells filled with electron-lucent lipid droplets of variable size. The ultrastructural characteristics of these cells enabled the distinction of two types of lipid-laden histiocytes, in relationship to the size of the lipid droplets. Since the lipid droplets were seen also in cells other than histiocytes, it appears that these changes are secondary to a common mechanism, comprising factors such as obstruction, hemorrhage, inflammation, and local hypoxia.

Localized amyloidosis in nasopharyngeal carcinoma diagnosed by fine needle aspiration and electron microscopy. A case report.

BACKGROUND: The association of amyloidosis and tumors is well known, but only rarely has it been found in the nasopharynx. Moreover, only a single case of tumor-associated amyloidosis in the nasopharynx has been diagnosed by exfoliative cytology and reported in the English-language literature. We describe a case of localized amyloidosis in metastatic nasopharyngeal carcinoma diagnosed by fine needle aspiration and confirmed by histology and electron microscopy. CASE: Bilateral neck enlargement appeared six months before consultation in a 57-year-old man. Both masses were resected, and during exploration of the nasopharynx, several blind biopsies were taken. Smears from the fine needle aspirate showed malignant epithelial cells, consistent with metastatic carcinoma, and Congo red stain showed the presence of amyloid. Histologic examination of the resected cervical masses and nasopharyngeal biopsies confirmed the diagnosis of nasopharyngeal carcinoma, nonkeratinizing variant with amyloid, and metastases in two lymph nodes. Electron microscopic examination of a lymph node showed extracellular, ribbon-like amyloid deposits as well as masses of amorphous amyloid, apparently intracellular. CONCLUSION: The origin of this form of amyloidosis is unclear in this patient since he had no other known etiologic factors for the condition. Increased awareness of the potential association of these two conditions and the use of fine needle aspiration may reveal a higher frequency than hitherto reported.

The lipofuscin-iron association in pigmentosis tubae.

Pigmentosis tubae (PT) is a rare condition characterized by the presence of numerous lipofuscin-laden macrophages in the lamina propria of the fallopian tube. Two women, who also had endometriotic ovarian cysts, showed polypoid pigmented tubal mucosae. In addition to lipofuscin, occasional cells showed spotty positivity for iron. Ultrastructural examination of the tubal mucosa showed the lipofuscin-containing bodies, which were similar to lipofuscin-containing lysosomes found in other pigmented conditions. Cytoplasmatic ferritin and hemosiderin in siderosomes were observed in macrophages and endothelial cells of the lamina propria. The present study is the first to demonstrate the presence of iron-containing particles and lipofuscin in the residual bodies of PT. The origin of the excess iron is not clear, but erythrophagocytosis and an abnormal tubal environment could play a role. Iron-promoted lipid peroxidation may alter the lysosomal membranes and contribute to the excessive accumulation of lipofuscin in these cells.
