The GnRH 10 micrograms test promotes a maximal gonadotropic response in normal women.

The authors have compared the results of gonadotropin stimulation with a gonadotropin releasing hormone (GnRH) intravenous bolus of 100 micrograms and of 10 micrograms in a group of eumenorrheic women. They conclude that there are no differences between the responses of the two dosages in normal women. Therefore the authors clarify that the dosage of 10 micrograms of GnRH as a bolus test may be used currently in clinical practice in order to assess gonadotropic function.

A G-string positive cis-regulatory element in the LpS1 promoter binds two distinct nuclear factors distributed non-uniformly in Lytechinus pictus embryos.

The LpS1 alpha and beta genes of Lytechinus pictus are activated at the late cleavage stage of embryogenesis, with LpS1 mRNAs accumulating only in lineages contributing to aboral ectoderm. We had shown previously that 762 bp of 5' flanking DNA from the LpS1 beta gene was sufficient for proper temporal and aboral ectoderm specific expression. In the present study, we identified a strong positive cis-regulatory element at -70 bp to -75 bp in the LpS1 beta promoter with the sequence (G)6 and a similar, more distal cis-element at -721 bp to -726 bp. The proximal 'G-string' element interacted with two nuclear factors, one specific to ectoderm and one to endoderm/mesoderm nuclear extracts, whereas the distal G-string element interacted only with the ectoderm factor. The ectoderm and endoderm/mesoderm G-string factors were distinct based on their migratory behavior in electrophoretic mobility shift assays, binding site specificities, salt optima and EDTA sensitivity. The proximal G-string element shared homology with a binding site for the mammalian transcription factor IF1, a protein that binds to negative cis-regulatory elements in the mouse alpha 1(I) and alpha 2(I) collagen gene promoters. Competition experiments using wild-type and mutant oligonucleotides indicated that the ectoderm G-string factor and IF1 have similar recognition sites. Partially purified IF1 specifically bound to an oligonucleotide containing the proximal G-string of LpS1 beta. From our results, we suggest that the ectoderm G-string factor, a member of the G-rich DNA-binding protein family, activates the LpS1 gene in aboral ectoderm cells by binding to the LpS1 promoter at the proximal G-string site.

[Ofloxacin in prophylactic therapy of oral infections].

86 patients (46 females, 40 males; mean age 37 years) were divided into 2 groups: one group of 40 patients with acute infectious odontostomatitis, and another group of 46 patients going to have their oral cavity surgically operated. The first group was treated with 300 mg oral tablets of ofloxacin 12 hourly for 5 days, the second group with the same posology for 3 days, for prophylactic purposes. 54 cases (24 in the first group and 30 in the second one) had Fans drugs integrating antibiotic therapy. At the end of the antibiotic treatment the first group showed a complete abatement of symptoms in 16 cases, a significant improvement in 19 cases, and better clinical conditions in 5 cases. The second group, examined again 4 and 7 days after surgical intervention, showed the prophylactic antibiotic treatment beneficial in 30 cases after 4 days, and in 14 cases after 7 days; for 2 cases it was necessary to replace the antibiotic drug by other. Side effects in both groups (14 cases) were slight and did not cause any treatment interruption. Given these results and the good tolerability of the drug, we conclude that orally administered ofloxacin is a valid alternative to the parenteral drug for the treatment of infectious odontostomatitis and for prophylaxis after oral surgery.

[Monoclonal antibody assessment of lymphocyte subpopulations in rheumatoid arthritis in adult patients in relation to stage and treatment].

Twenty patients with various stages of RA were examined. They included: 1) 7 cases in Steinbroker stage 1 (onset); 2) 13 cases in Steinbroker stage 2-3 (chronic). The group was divided into 16 treated and 4 untreated cases. Monoclonal antibodies were used to assay T3, T4, T8 and T4/T8 ratio and the results were compared with those in a control group. The results showed a T8 deficit and a consequent change in the T4/T8 ratio in untreated subjects with "onset" RA. The immune situation of the chronic and treated cases was very similar to that in the control group probably as a result of treatment.

[Significance of hemorheological parameters within the framework of chronic brain disease].

Blood filtration and viscosity were investigated in 32 patients with severe mental deterioration classified on the basis of standard criteria. The group included 17 patients with 'vascular' and 15 with 'cellular' pathogenesis. The results of the study were compared with the data from a large series of 'normal' subjects. Reduced erythrocyte filtration was observed in 70.5% of the patients with vascular pathogenesis and in only 6.6% of those with degenerative conditions. The data on blood viscosity showed less connection between pathological conditions and hemorheological alterations. It is suggested that hemorheological alterations might contribute to the pathogenesis of certain clinical forms of chronic cerebropathy. It is therefore proposed that hemorheological tests should be included among the diagnostic procedures applied to such patients and might also be used in order to establish the appropriate treatment.

[Effect of buflomedil on the behavior of platelet aggregation and its clinical evaluation in elderly patients with chronic cerebrovascular insufficiency].

An examination was made of the behaviour of platelet function after a single i.v. injection of 150 mg Buflomedil in 8 non-diabetic vasculopathic patients. The drug was followed by a significant reduction of the platelet aggregability induced by ADP and adrenaline, but not by collagen, and did not alter the percentage of reversible circulating platelet aggregates. Its action was limited in time, since no activity was observed after 24 hr. The drug was also clinically evaluated in 30 patients aged over 80 yr with chronic cerebrovascular insufficiency, following treatment with 100 mg/day i.v. for 20 days, and then 450 mg/day per os for 80 days. Significant improvements were noted in vertigo, tinnitus, lapses of memory, and the ability to remember numbers. The results were less impressive with regard to insomnia, migraine, and asthenia.

Angioma of the iris.

The clinical and anatomopathological features of angioma of the iris are described and the relevant literature is briefly reviewed. A personal case in a 47-year-old woman is presented. Reference is made to the fact that, unlike many of the cases in the literature, histological confirmation of the diagnosis was obtained. Mention is also made of the importance of fluorescein angiography in the differential diagnosis of other forms, particularly melanoma. Fluorescein angiography, for the first time applied to angioma of the iris in the present case, enabled a satisfactory result (follow-up of 8 months) to be obtained by conservative iridectomy, in line with the recent approach to the management of this form.

Combination chemotherapy with vinblastine, bleomycin and methotrexate in DTIC-resistant metastatic melanoma.

Fifteen patients with metastatic DTIC-resistant malignant melanoma were treated with vinblastine, bleomycin and methotrexate combination chemotherapy. Three patients showed an objective response (one complete response). The therapy was well tolerated and easy to administer. This combination appears to produce in DTIC-resistance patients a response rate similar to that obtained with DTIC.

Quantitative indices of clinical uremia.

The disabling, dialysis-responsive symptoms of clinical uremia primarily represent impaired functions of the nervous system. Accordingly, these studies used several quantitative electrophysiologic and cognition-dependent probes of nervous system function: peripheral nerve-conduction velocity, response latency and amplitude; electroencephalographic (EEG) power-spectrum analysis; visual evoked response latency; EEG responses to photic driving; and measures of integrated behavioral performance that stressed sustained and selective attention, reaction time, speed of decision-making, short-term memory, and mental manipulation of symbols. Probes of quanitified central nervous system function consistently revealed impairments that varied directly with the degree of renal failure, that improved following onset of maintenance dialysis, and that improved still further after successful renal transplantation. In contrast, measures of peripheral nerve function were generally unchanged. Neurobehavioral measures that are relevant to uremic symptomatology provide quantitative estimates of the clinically significant, whole-organism biologic effects of renal failure and its several treatments. Applications of such measures in studies of symptomatic states other than uremia are indicated also.

Quantitative assessment of the electroencephalogram in renal disease.

EEGs wre recorded from renal patients to determine if there are quantifiable characteristic changes in the EEG was quantified by calculating the percentage of spectral power in the bandwidth 3-7 c/sec referrred to a frequency range of 3-13 c/sec and by computing the mean frequency of the dominant rhythm in the EEG. Blood urea nitrogen and creatinine concentrations, as well as a self-assessment of the patient's clinical condition, were recorded. The general finding of this research is that EEG slowing, as evaluated by power spectral techniques, is correlated with uremia-associated variables. 1. In a non-dialyzed patient population with renal failure, slowing in the EEG was found to be directly corelated with increased creatinine concentrations. 2. Quantitative measures of slow wave activity computed using power spectral techniques were found to be highly corelated with an estimate of slowing made by an electroencephalographer. 3. Compared with undialyzed azotemic patients, malignant hypertensive patients with comparable serum creatinine concentrations typically displayed increased slow wave activity, while slowing was generally reduced in the dialyzed patient population. 4. A series of EEGs recorded from one patient during the first three dialyses of her life revealed that slow wave activity decreased during each successive dialysis. Survival rates of lung cancer according to histological type.

The post-operative survival in 554 lung carcinomata, classified according to the histological type, was calculated by the actuarial method. On the whole, squamous cell carcinoma was the most favourable and anaplastic small cell carcinoma the least favourable lesion. However, in tumours smaller than 4 cm, confined to the lung and with negative lymph nodes (stage I), small cell carcinoma had the highest percentage of 5 year survivors, followed by large cell carcinoma, squamous cell carcinoma and adenocarcinoma. When tumours had attained a larger size and/or spread to neighbouring structures and regional lymph nodes (stage II and III), the histological type was a much more determining factor in survival, squamous cell carcinoma being a significantly more favourable lesion. On the other hand, no difference in survival in relation to the histological type was found when distant metastases were probably present (stage IV). It was concluded that in assessing the role of histopathology in the prognosis of lung cancer, the mutual relationship to other pathological factors must be taken into account.
