Temporal and spatial localization of novel nuclear protein NP95 in mitotic and meiotic cells.

Expression of novel NP95 (nuclear protein, 95 kDa), which contains a leucine zipper motif, a zinc finger motif, a putative cyclin A/E-cyclin-dependent protein kinase 2 phosphorylation site, and retinoblastoma protein-binding motifs, is associated with S-phase progression of mouse cells. It is suppressed during G1 and G2/M phases in normal thymocytes but expressed at a constantly high level irrespective of cell stage in mouse T cell lymphoma cells. NP95 was shown previously to be expressed strongly only in proliferative tissues and cells. In this immunohistochemical study, we demonstrate that NP95 is localized in S-phase nuclei as dot-like foci. Double immunostaining of NP95 and proliferating cell nuclear antigen (PCNA) showed that NP95 was co-localized with PCNA. Construction of three-dimensional images indicated that NP95 was localized with PCNA in replication sites in a somewhat distinct temporal manner. During meiosis, NP95 was present not only in proliferating spermatogonia but also in meiotic spermatocytes and differentiating spermatids which were not proliferating. The possible role of NP95 in mitotic and meiotic cells is discussed.

An immunohistochemical study of cytokeratins in skin-limited amyloidosis.

The frequency of amyloid deposits in cases of seborrheic keratosis was investigated In addition, the origin of amyloid protein(s) in lichen amyloidosis, macular amyloidosis and seborrheic keratosis was studied by immunohistochemical staining using a panel of anti-cytokeratin (CK) monoclonal antibodies. Amyloid deposits were found in 41 of 327 specimens (12.5%) from 301 cases of seborrheic keratosis. Amyloid deposits in seborrheic keratosis reacted with 6 of 12 CK antibodies and in lichen and macular amyloidosis (20 specimens) reacted with 5 of 12 CK antibodies. In seborrheic keratosis, antibody DE-K10 (labeling CK10) reacted with amyloid in 17 of 36 cases, antibody 34betaE12 (labeling CK1, 5, 10, 14) reacted in 33 of 39 cases, and antibody MNF116 (labeling CK5, 6, 8, 17) reacted in 32 of 35 cases. Among 20 specimens from lichen and macular amyloidosis, the three antibodies reacted with amyloid in the following rates: 1 with antibody DE-K10, all 20 with antibody 34betaE12, and 6 with antibody MNF116. These results suggest that amyloid deposits in seborrheic keratosis and lichen and macular amyloidosis may derive from epidermal cytokeratins.

Hyperreactivity of pseudoxanthoma elasticum-affected dermis to vitamin D3.

A 68-year-old woman with pseudoxanthoma elasticum (PXE) was treated with oral vitamin D(3). After 2 weeks, new papules developed on the pre-existing plaques. Biopsy specimens of a fresh papule showed more abundant calcium deposition than before therapy. Electron microscopy revealed electron-lucent areas, which suggested unusual mineralization. From these findings, we propose that in patients with PXE, certain cutaneous tissues are highly sensitive to vitamin D(3), resulting in microdeposition of calcium salts.

[Critical paths and economical efficiency on cancer therapy].

The critical paths of cancer patient care are discussed. The number of cancer institutes which have introduced critical paths into patient care has been increasing in the past few years. The effect of critical paths is to standardize the patient care process, shorten the length of the hospital stay of cancer patients, reduce the cost of care, have a well-managed cancer care team, and satisfy patients with full information on the care schedule. Combined with clinical practice guidelines, protocols and algorithms, critical paths are a more powerful tool with which to improve the efficiency and quality of cancer patient management in hospital care.

[Modularization by the open standard. (II)].

In recent years, accompanied by the marvelous development and spread of Laboratory Automation System(LAS), the NCCLS is now proposing five international standards for laboratory automation. We have based our laboratory on these "NCCLS standards of laboratory automation", we take these standards ahead first, and we now propose an open standard called "Open LA 21", to establish more detailed standard replacing the NCCLS laboratory automation standards.

Concurrent chemoradiotherapy for esophageal carcinoma patients with malignant fistulae.

BACKGROUND: It remains controversial whether chemotherapy and/or radiotherapy are/is contraindicated for esophageal carcinoma patients with malignant fistulae. In some case reports, closure of fistulae by chemotherapy or radiotherapy has been reported. The current study investigated chemoradiotherapy for these patients using various primary treatments to manage the pulmonary complications. The aim of this study was to evaluate the efficacy and feasibility of chemoradiotherapy for patients with locally advanced esophageal carcinoma with malignant fistulae. METHODS: Patients with endoscopically or radiologically confirmed fistulae were treated with concomitant chemoradiotherapy. Closure of fistulae was assessed by esophagography or endoscopy. Oral food intake also was assessed before and after treatment. RESULTS: Of 202 esophageal carcinoma patients treated at National Cancer Center Hospital East between July 1992 and May 1998, 24 patients (11.9%) developed malignant fistulae. Twelve patients developed fistulae before treatment and the remaining patients developed fistulae during treatment. Closure of the fistulae after chemoradiotherapy was observed in 17 of these patients (70.8%), and 16 of these 17 patients (94.1%) had oral alimentation restored after successful treatment. The median survival time from the diagnosis of the fistula for all patients with fistulae was 198 days; in the patients whose fistulae were present before chemoradiotherapy, the median survival time was 238 days. CONCLUSIONS: These results suggest that the presence of malignant fistulae does not contraindicate chemoradiotherapy. Once the inflammation due to the fistula has been controlled, chemoradiotherapy should be utilized because it may provide the best chance for survival and palliation of severe dysphagia.

Nonclostridial gas gangrene due to Streptococcus anginosus in a diabetic patient.

Streptococcus anginosus was recently identified as a distinct species from the other members of Streptococcus milleri group (Streptococcus constellatus, Streptococcus intermedius). We report a rare case of nonclostridial gas gangrene caused by S. anginosus. A 62-year-old diabetic woman was admitted with gas gangrene of the perineal area. She had been taking her oral hypoglycemia medication regularly for 10 years, but the diabetes was inadequately controlled. She was treated with surgical debridement of the necrotic tissue, insulin injection, and antibiotic therapy, and had a satisfactory clinical course.

Biologic roles of gangliosides G(M3) and G(D3) in the attachment of human melanoma cells to extracellular matrix proteins.

The biologic functions of gangliosides G(M3) and G(D3) in the attachment of human melanoma cells to extracellular matrix proteins (type I and IV collagens, fibronectin, and laminin) were investigated by using the G(D3)-deficient mutant clone (SK-MEL-28-N1) and the parent cell line SK-MEL28. SK-MEL-28-N1 (N1) (high G(M3) expression: G(M3), 97.3%; G(D3), 0%) was selected by treating SK-MEL-28 (high G(D3) but low G(M3): G(M3), 6.5%, G(D3), 93.5%) with an anti-G(D3) monoclonal antibody (R24) and rabbit complement and subsequent subcloning of the surviving cells. The N1 clone showed significantly higher ability to adhere to type I and IV collagens and laminin than the parent clone SK-MEL-28. In the N1 clone, the expression of alpha2beta1 and alpha3beta1 integrin receptors was increased, whereas in SK-MEL-28, their expression was very low or undetectable. The treatment with monoclonal antibodies directed specifically to G(D3) expressed on SK-MEL-28 inhibited the cell attachment to type IV collagen (33% inhibition of control), fibronectin (59%), and laminin (71%). These findings suggest that gangliosides G(M3) (by influencing integrin receptor levels) and G(D3) (by interacting directly with matrix proteins) might play some functional roles in attachment to extracellular matrix proteins and thereby enhance the metastatic potency of melanoma cells.

Clinical significance of magnetic resonance cholangiopancreatography for the diagnosis of cystic tumor of the pancreas compared with endoscopic retrograde cholangiopancreatography and computed tomography.

BACKGROUND: Cystic tumor of the pancreas has been investigated by a variety of imaging techniques. Magnetic resonance cholangiopancreatography (MRCP) is being widely used as a non-invasive diagnostic modality for investigation of the biliary tree and pancreatic duct system. The purpose of this study was to compare MRCP images with those of endoscopic retrograde cholangiopancreatography (ERCP) and computed tomography (CT) in order to clarify the diagnostic efficacy of MRCP for cystic tumor of the pancreas. METHODS: We retrospectively studied 15 patients with cystic tumor of the pancreas that had been surgically resected and histopathologically confirmed. There were five cases of intraductal papillary adenocarcinoma, five of intraductal papillary adenoma, two of serous cyst adenoma, two of retention cyst associated with invasive ductal adenocarcinoma and one of solid cystic tumor. RESULTS: In all cases MRCP correctly identified the main pancreatic duct (MPD) and showed the entire cystic tumor and the communication between the tumor and the MPD. On the other hand, the detection rate by ERCP of the cystic tumor and the communication between the cystic tumor and the MPD was only 60%. Although the detection rates by CT for the septum and solid components inside the cystic tumor were 100 and 90.0%, respectively, those of MRCP for each were 58.3 and 20.0%. CONCLUSION: MRCP is capable of providing diagnostic information superior to ERCP for the diagnosis of cystic tumor of the pancreas. Although MRCP may provide complementary information about the whole lesion of interest, the characteristic internal features of cystic tumor of the pancrease should be carefully diagnosed in combination with CT.

Overexpression of p53 protein associated with proliferative activity and histological degree of malignancy in solar keratosis.

BACKGROUND: Overexpression of p53 protein has frequently been demonstrated in human epithelial neoplasms including squamous cell carcinoma (SCC) and solar keratosis (SK). SK is a precursor lesion of SCC. In SCC of the skin, we have already reported that overexpression of p53 protein is associated with the proliferative activity, which is indicated by the percentage of Ki-67-positive cells. In SK, however, the significance of p53 overexpression is still unclear. OBJECTIVE: The aim of the present study was to clarify the relationship between p53 overexpression and proliferative activity, and between p53 overexpression and histological degree of malignancy in SK. METHODS: Sixty-eight cases of SK were divided into four grades according to the proportion of atypical cells in the epidermis (in grade I, up to 25% of the epidermal cells are atypical, in grade II 25-50%, in grade III 50-75%, in grade IV more than 75%) and were immunohistochemically analyzed for the expression of p53 protein and Ki-67 antigen. RESULTS: p53 overexpression was detected in 58 out of the 68 cases of SK. The percentage of p53-protein-positive cells (DO-7 index) ranged from 0 to 78.8 (mean +/- SD = 20.6 +/- 21.6). Mean DO-7 indices were 7.9 in grade I, 26.7 in grade II and 38.3 in grade III. The differences in mean DO-7 index between grades I and II and between grades I and III were statistically significant (p < 0.001). The percentage of Ki-67-antigen-positive cells (MIB-1 index) ranged from 0.8 to 38.2 (mean +/- SD = 10.5 +/- 9.0). Mean MIB-1 indices were 5.9 in grade I, 10.7 in grade II and 20.5 in grade III. The differences in the mean MIB-1 index between grades I and II and between grades II and III were statistically significant (p < 0.01). Cases with higher DO-7 indices tended to show higher MIB-1 indices, and the relation was statistically significant [y = 0.272x + 4.8; correlation coefficient r = 0.59 > r (n - 2, 0.01) = 0.31]. CONCLUSION: Overexpression of p53 protein was related to cell proliferation as well as histological degree of malignancy in SK. Dysfunction of p53 protein might be implicated in skin carcinogenesis.

Definitive chemoradiotherapy for T4 and/or M1 lymph node squamous cell carcinoma of the esophagus.

PURPOSE: To investigate the efficacy and feasibility of concurrent chemoradiotherapy for locally advanced carcinoma of the esophagus. PATIENTS AND METHODS: Fifty-four patients with clinically T4 and/or M1 lymph node (LYM) squamous cell carcinoma of the esophagus were enrolled. Patients received protracted infusion of fluorouracil 400 mg/m(2)/24 hours on days 1 to 5 and 8 to 12, 2-hour infusion of cisplatin 40 mg/m(2) on days 1 and 8, and concurrent radiation therapy at a dose of 30 Gy in 15 fractions over 3 weeks. Filgrastim was prophylactically administered to 35 patients. This schedule was repeated twice every 5 weeks, for a total radiation dose of 60 Gy, followed by two courses of fluorouracil (800 mg/m(2)/24 hours for 5 days) and cisplatin (80 mg/m(2) on day 1). RESULTS: There were 21 patients with T4M0 disease, one with T2M1 LYM, 17 with T3M1 LYM, and 15 withT4M1 LYM. Forty-nine patients (91%) completed at least the chemoradiotherapy segment. The 18 patients (33%) who achieved a complete response included nine (25%) of the 36 with T4 disease and nine (50%) of the 18 with non-T4 disease. Major toxicities were leukocytopenia and esophagitis; there were four (7%) treatment-related deaths. Prophylactic filgrastim reduced the incidence of grade 3 or worse leukopenia without improving dose-intensity or response. With a median follow-up duration of 43 months, median survival time was 9 months. The 3-year survival rate was 23%. CONCLUSION: Despite its significant toxicity, this combined modality seemed to have curative potential even in cases of locally advanced carcinoma of the esophagus.

Smooth muscle cell proliferation in the ductus arteriosus and the descending aorta, and effects of enalapril on SMC proliferation in perinatal rats.

This study was carried out to determine the proliferation profile of the smooth muscle cells (SMC) in the media of the ductus arteriosus (DA) and the descending aorta (Ao), and to examine the effects of the angiotensin-converting enzyme inhibitor enalapril on the proliferation of these cells in perinatal rats. The proliferating cell nuclear antigen (PCNA) index of the DA peaked in 19-day-old fetuses at 75%, and the index significantly declined in 20-day-old fetuses. The PCNA index of the Ao showed a similar profile until pups reached 1 day of age; however, the index of the Ao then increased in 3-day-old pups. The PCNA indices of the DA and Ao decreased significantly after maternal oral treatment with enalapril (10 mg/kg for 7 days), with a more marked decline in the DA than in the Ao. The PCNA indices of these vessels in 20-day-old fetuses were not altered by maternal treatment with enalapril. These results indicate that the SMC proliferation rate in the DA was similar to that in the Ao until pups reached the age of 1 day, and that the inhibitory effect of enalapril on the SMC proliferation was age-dependent and more prominent in the DA than in the Ao.

[Computerized tomography features of intestinal infarction: 56 surgically treated patients of which 5 with reversible mesenteric ischemia].

PURPOSE: Bowel infarction is a rare and typical condition of the elderly; despite improvements in diagnostic imaging and vascular surgery, bowel infarction remains a major cause of acute abdomen, with mortality rates ranging 70-80%. Diagnosis is often late because clinical signs, laboratory data and radiologic findings are aspecific. We investigated radiographic and particularly CT patterns of intestinal infarction in 56 patients submitted to surgery within 12 hours of admission. We also report the CT findings of 5 of these patients who had reversible mesenteric ischemia. MATERIAL AND METHODS: We retrospectively reviewed 56 cases of bowel infarction. The patients were 29 men and 27 women ranging in age 46-84 years (mean: 63). All the patients were submitted to plain radiography of the abdomen in different projections; emergency CT was carried out with i.v. contrast agent injection. We considered the following CT patterns: dilatation of intestinal loops > 2.5-3 mm, wall thickening > 3-4 mm, intraperitoneal effusion, stuffing of mesenteric vessels with diameter > 3 mm, air-fluid levels. RESULTS: Patients in the 7th decade of life were most frequently affected (38 cases), with an overall mortality rate of 59% (33 deaths). Plain radiography showed distention of bowel loops with air-fluid levels in 91% of cases. CT proved to be an accurate technique with higher sensitivity than radiography in detecting mesenteric edema and hemorrhage (68%), abdominal and pelvic effusion (88%), parietal pneumatosis (9%), wall thickening (29%), intraportal gas (7%), and thrombosis of superior mesenteric artery (3.5%). CT patterns in the 5 patients with reversible intestinal ischemia were wall thickening (80%), peritoneal effusion (80%), meteoric dilatatation (40%), a blurred appearance of mesenteric fat (40%). CONCLUSIONS: Angiography is a valuable imaging and treatment technique permitting the diagnosis of vascular occlusion and the intraarterial infusion of vasodilators, but it can be carried out in emergency in few centers only. This makes conventional radiology, and particularly CT, the only tool providing useful information for early diagnosis and treatment of bowel infarction. CT is more sensitive than radiography and does not exhibit the limitations of angiography--i.e., invasiveness, radiation exposure and complex organization. Therefore CT can presently be considered the method of choice in patients with suspected bowel infarction.
