In 118 patients suffering from thyrotoxicosis before and after the operation by the method of rheography a study was made of the effect of prednisolone on the stroke and minute volume of the heart, hydraulic resistance of peripheral vessels and the tone of the vessels of extremities. Brief courses of prednisolone treatment (medium doses) before the operation and also after it promoted a more rapid normalization of the mentioned indices. High doses of glucocorticoids prescribed under conditions of a thyrotoxic crisis also influenced the state of the systemic circulation favourably. The data obtained showed that it was expedient to include short courses of low and medium doses of prednisolone into the complex treatment of patients with thyrotoxicosis and in case of developed thyrotoxic crises, to carry out massive hormonal therapy.

The method of acceleration cinecardiography was applied to the study of phasic structure of the systole of the left and right cardiac ventricles in 22 patients suffering from Itsenko-Cushing's disease. In the majority of patients with severe and prolonged (over 5-year duration) sickness changes in the duration of the systolic phases of both cardiac ventricles were characteristic of phasic hypodynamia syndrome, i.e. there was a reduction of the contractile activity of the myocardium. In 6 patients with marked arterial hypertension the structure of the left ventricle systole testified to the presence of a phasic syndrome of high systolic pressure. Intensification of cardiac contraction (phasic hyperdynamia syndrome) was less frequent. This chiefly occurred in case of mild course of the disease of less than 5 years duration.
Clinical investigations were carried out in the endocrine surgery department. Of 1127 patients with different pathology of the thyroid gland operated on carcinoma was revealed in 179 cases (15.88%). Of this number 14 patients were operated on for clinically manifest carcinoma and its relapse, and in 165 cases carcinoma was revealed during the operation and after it. Among 609 patients operated on for euthyroid goiter carcinoma was revealed in 116 (19.04%). In solitary nodes (450 cases) its incidence constituted 21.77%, in mixed goiter (123 patients)--10.56%, and in autoimmune thyroiditis (101 cases)--15.84%. Of 394 patients with toxic goiter operated on carcinoma was revealed in 33 (8.37%). Of 287 patients with diffuse toxic goiter operated on--in 17 (5.92%) patients. The results obtained suggest an oncological risk in such pathology.
Among different thyroid abnormalities thyroid cancer was seen in 179 (15.8%) of 1127 operated patients. Cancer mainly occurred in patients with nodoseeuthyroid or combined goiter: in 111 (19.3%) of 573 patients. In 69 subjects cancer developed in the unaffected thyroid gland, 20 of them had multifocal lesions; 42 subjects had cancer coupled with hyperplastic process, multifocal tumor being diagnosed in 12 persons. The data obtained indicate high cancer incidence in the unaffected thyroid gland. Cancer-hyperplasia complex must be regarded as a specific pathological variety being the most difficult for diagnosis and treatment.
Distribution of the HLA system Class 1 specificities was studied in 97 patients with thyroid carcinoma, 101 ones with nodular goiter and 500 donors in the Central Urals. B15, B18 antigens were very frequent and All antigens quite rare in the patients with thyroid carcinoma; in patients with nodular goiter the incidence of Aw19, A28, B18 antigens was the highest, whereas the incidence of A11 antigen and the A2-B35 haplotype was low. The groups of patients with thyroid carcinoma and nodular goiter differed only by the incidence of A28 antigen and A2-B35 haplotype, this fact indicating the immunogenetic similarity of the diseases in question. Similar pathogenetic mechanisms of the formation of mammary carcinoma and of nodular goiter in the thyroid may be supposed, this giving grounds for the radical approach to the treatment of nodular formations of the thyroid.
