Studies on the prospect of bioprostheses by bovine aortic valve for human use.

A total of 21 heart specimens from indigenous cattle (Bos indicus) were studied. The measurement and observation of bovine heart and aortic valve in different age group is the preliminary finding of this research work. After harvesting, the bovine aortic valves were measured and the sizes ranged from 21 mm to 35 mm in orifice diameter. In comparison with implanted aortic and mitral valves at NICVD in 1989, it indicates that the selected bovine heart valves (specially aortic valves) were similar to human valve size and can be used in future for implantation as far as the size is concerned. Now further study regarding making these tissue valves implantation worthy for clinical use is to be undertaken. We strongly feel the prospect is bright.

HBsAg amongst unscreened operated patients.

Prevalence of asymptomatic Hepatitis B carrier state varies in different countries. Surgical procedures are an important mode of dissemination of Hepatitis B virus infection. Five hundred patients whose HBsAg status was not known and had undergone a major operative procedure in the Dhaka Medical College Hospital were screened for HBsAg. Of these, 43 (8.6%) were found to be HBsAg positive. Age, sex, and previous history of jaundice didn't show any relationship with the HBsAg status. But the HBsAg positivity was found to be positively related to the past history of blood transfusion, hospitalization and injections.

Enterically transmitted non-A, non-B hepatitis associated with an outbreak in Dhaka: epidemiology and public health implications.

Investigation of an outbreak of hepatitis in Dhaka implicated enterically transmitted non-A, non-B (ET-NANB) hepatitis as the etiologic agent. This observation was made by exclusion in 17 of 19 patients, and confirmed serologically in one randomly selected patient by a fluorescent antibody blocking assay specific for ET-NANB hepatitis. These findings reinforce earlier suspicions that ET-NANB may be an important cause of acute hepatitis in Bangladesh. The epidemiology of ET-NANB hepatitis is discussed, and public health recommendations are made.

Smoking habit among Bangladesh Secretariat staff.

The paper assesses the smoking behaviour among the Bangladesh Secretariat Staff, Dhaka. The findings are based on interviewing 2008 respondents of whom 775 (38.6%) were reported to be smokers. Amongst the three categories of the respondents selected for interview, the prevalence of smoking was the highest (49%) for the respondents belonging to the lowest socio-economic group. The mean age at commencement of smoking was in the neighbourhood of 18 years. Majority of the smokers (76.5%) were motivated by their friends. Advertisement did not appear to have any influence of smoking. Nearly 17% were self motivated. Respondents' current age, level of education and employment status were found to have strong influence on smoking habit.

[Comparative study of the effects of verapamil, ethmozin and ethacizine on provoked attacks of atrioventricular nodal reciprocal tachycardia].

27 patients underwent serial electrophysiological studies by using transesophageal atrial stimulation. A-V nodal reciprocal tachycardia was documented by intracardiac electrophysiological examinations. Sustained tachycardia was induced in all the patients before drug administration. On day 4 after oral verapamil, 320 mg/day, ethmosine, 800 mg/day, and ethacizine, 150 mg/day, the patients were subjected to transesophageal atrial stimulation. An antiarrhythmic effect was regarded to be reached if the authors failed to induced sustained tachycardias again. Verapamil, ethmosine, and ethacizine were found to be beneficial in 21 (78%), 13 (48%) and 21 (78%) patients, respectively. A comparative analysis demonstrated that ethacisine was not inferior to verapamil, but ethmosine produced less effects than verapamil and ethacizine. The crossover and individual efficacy shown by each drug suggests that it is necessary to use the technique of serial testing and to choose beneficial drugs from a possibly wide range of medicaments for each patient.

Contraceptive behaviour in the areas served by family welfare assistants and midwives in rural Bangladesh: a comparative analysis.

"The study is an attempt to investigate contraceptive behaviour in the areas served independently by FWAs [family welfare assistants] and Dais [midwives] in...selected areas of Bangladesh....[Findings] indicated that almost every woman (98.5 percent in FWA area and 99.3 percent in dai area) has a basic awareness of at least one method of contraception....The results...suggest that a basic awareness has developed...in both the areas....A significant increase in the response on the knowledge of specific contraceptive methods was observed after prompting had been adopted while interviewing in both the areas. This clearly reveals the influences of prompting...on measures of knowledge among the target population." Data are from surveys conducted in 1985.

Mortality in three selected villages of Bangladesh.

"The present study was undertaken to identify the levels and age patterns of mortality in one rural area of Bangladesh. Some of the most recently developed nontraditional methods were applied to the...data set for obtaining estimates of infant and child mortality and adult mortality. The validity of estimates was assessed by making a comparison with a few indicators of mortality available from various sources. Although the basic data from local studies might have been deficient, the application of nontraditional methods to such information on mortality could still provide consistent and plausible estimates both at regional and national levels." Data are from a survey conducted in 1986.

Prevalence of hepatitis B surface antigen (HBsAg) among prostitutes at Dhaka.

One hundred prostitutes were studied with an aim to acquire more insight on the role of sexual transmission of HBsAg and to detect carrier state among them. All subjects were interviewed and their serum samples were tested for HBsAg by reverse passive haemagglutination method (RPHA). Eleven (11%) of the prostitutes were found to be positive for HBsAg. Their mean age was 19.3 +/- 4.9 years. The mean duration in the profession was 2.5 +/- 1.6 years. Mean number of clients for each woman per 24 hours was 10.7 +/- 1.2. Most of the prostitutes (84%) were lacking knowledge about spread and contact of the disease.

A non-isotopic screening test for antibodies to TSH receptor: dot immunobinding assay.

A dot immunobinding assay (DIBA) for thyrotropin (TSH) receptor antibodies is described. The method depends on the detection of antibody binding to highly purified thyroid plasma membrane attached to nitrocellulose solid support by horse-radish peroxidase - conjugated anti-human IgG. The method can detect down to 0.75 mU LATS standard and 1/1000 dilution of Graves' serum or immunoglobulin fraction. The interaction is inhibited dose-dependently by bTSH but not by insulin or human chorionic gonadotropin. The DIBA results show close correlation to those of TRAK (TBII) but not to cyclic AMP generation assay. DIBA is reproducible when tested monthly for 4 months. Sera and immunoglobulins gave virtually the same results. The method has a sensitivity of 90%, validity of 90% and specificity of 80% for both. We have, thus developed a sensitive and reliable method for screening for TSH receptor antibodies which can be performed in routine clinical laboratories.

Prevalence of hepatitis B surface antigen (HBsAg) among parenteral drug abusers at Dhaka.

One hundred parenteral drug abusers were studied with an aim to detect HBsAg in their sera and to know carrier state among them. All subjects, were interviewed and their serum samples were tested for HBsAg by reverse passive haem-agglutination method (RPHA). Eight percent of the drug addicts were found to be positive for HBsAg. The mean age of addicts was 23.65 +/- 5.06 years. Ninety two percent were educated (above class VIII) and 59 percent were students. Forty three percent were influenced by friends to develop this habit and 34 percent became addicted out of frustration. Forty four percent addicts were using disposable syringes and needles and 32 percent were sharing their syringes and needles among themselves. Nineteen percent addicts had past history of jaundice. Fifty seven percent had extra-marital sexual life and 64 percent were lacking in knowledge of spread, or contraction of disease by their habits.

Use of anti-idiotypic antibodies as probes for the interaction of TSH subunits with its receptor.

TSH is a heterodimeric glycoprotein hormone, whose dissociated subunits are without biological activity. This has precluded the assessment of the relative contribution of each subunit to hormone action. We have raised anti-idiotypes to monoclonal antibodies specific, respectively, for the alpha and beta hTSH subunits. The anti-beta anti-idiotype inhibited 125I-hTSH binding to the beta subunit-specific monoclonal quantitatively, whereas 125I-hTSH binding to the alpha subunit-specific monoclonal was not inhibited by anti-alpha anti-idiotypes, suggesting that only the former is an "internal image" anti-idiotype. Neither of the two anti-idiotypes nor equimolar mixtures thereof inhibited 125I-bTSH binding to thyroid membranes, even though radiolabelled anti-idiotypes showed saturable binding to thyroid plasma membrane which was inhibited 41-65% by bTSH. Each anti-idiotype alone caused 9% inhibition (compared to 50% by NRIgG) of thyroid plasma membrane adenylate cyclase. Equimolar mixtures (125 micrograms/ml IgG of each anti-idiotype) induced enzyme activity equivalent to 85% of that of 250 mU/ml of TSH. The TSH-like action of the two anti-idiotypes was also reflected in their capacity to increase (450% by 250 micrograms/ml IgG compared to normal rabbit IgG) the uptake of 131I into isolated thyrocytes and to promote the organization of such cells into follicular structures. At 250 micrograms/ml, anti-beta anti-idiotype promoted the organization of small follicles and only at a concentration of 500 micrograms/ml did it enhance 131I uptake.

Congenital meningocele-encephalocele in an experimental swine herd.

Fourteen instances of meningocele-encephalocele in an experimental herd were recorded over a 5-year period. Thirteen of these affected animals were subjected to extensive pedigree analyses. Familial associations were strong, and clearly the meningocele-encephalocele in this herd was under genetic influence. However, additional studies are needed to clarify the mode of inheritance and, perhaps, to identify other factors that might be involved.
