Effect of ischaemia on the enzyme activity of the hepatic tissue.

The lactate and pyruvate levels, as well as acid and alkaline phosphatase, lactate dehydrogenase, glucose-6-phosphate dehydrogenase, glutaminic acid-oxalacetic acid transaminase and aldolase levels of rat liver homogenizates were measured at 24 degrees C and 38 degrees C during 120 min ischaemia from 0 to the 120th min. With the exception of transaminase and aldolase, the other enzymes were also enzyme-histochemically studied. The early lesions of the liver can be detected, both the quantitative laboratory tests and enzyme histochemical studies. The deviations from normal, observed at 24 degrees C between the 60th and 100th min, and at 38 degrees C between the 30th and 60th min, might be signs of irreversible lesions. Fractionated study of the liver homogenizate improves the assessability of enzyme determinations. In the course of "warm" ischaemia, the liver lysosomal lesions are early symptoms. Parallel to the breakdown of aerobic glycolysis lactic acid, fermentation, and an active pentose phosphate cycle can be detected. Quantitative testing of the liver homogenizate and enzyme histochemical observation of the hepatic tissue, might be a suitable method for the assessment of ischaemic liver lesions.

The effect of pindolol on plasma renin activity in patients with essential hypertension.

Twenty-two patients with essential hypertension were treated for 3 months with pindolol, and blood pressure and plasma renin activity were measured at rest and after stimulation (upright posture stimulation and insulin induced hypoglycaemia stimulation). Beta-receptor blockade produced a significant decrease in systolic and diastolic blood pressure. After treatment with pindolol the plasma renin activity was significantly lower. Under conditions of renin stimulation such as orthostasis and insulin produced hypoglycaemia, plasma renin activity was significantly lower in treated patients. There was no correlation between the fall of plasma renin activity and the decrease of blood pressure. Renin suppression is probably only one of the factors involved in the reduction in the blood pressure in these patients.

[Changes in blood sugar and ammonium levels after ligation of the afferent hepatic vessels].

Blood sugar and ammonia levels were studied after ligation of the afferent vessels of the liver. Isolated liver anoxia and isolated splanchnic stagnation were produced by means of a side-to-side porto-caval anastomosis in dogs. Isolated liver ischaemia for 45 minutes did not lead to a typical change in blood sugar and ammonia levels. In splanchnic stagnation with intact hepatic circulation, hyperglycaemia and a high blood ammonia level appeared in the portal venous blood. Thus, in the first 45 minutes afferent hepatic vessels are primarily due to splanchnic stagnation and not to hepatic ischaemia. When splanchnic stagnation and hepatic ischaemia occur simulatneously, the enhanced production of metabolites and the lack of compensation lead to a rapid lethal outcome.

Significance of 3alpha, 17 alpha-dihydroxy-5beta-pregnane-20-one in the diagnosis of congenital adrenal hyperplasia.

The 3alpha-17alpha-dihydroxy-5beta-pregnane-20-one (17-hydroxy-pregnanolone) excretion was found by gas chromatography to be as high as the pregnanetriol in urine of 4 virilising CAH female and 2 male patients. According to our results the significance of the determination of 17-hydroxy-pregnanolone is equal to that of the pregnanetriol in the diagnosis of adrenal hyperplasia. As the 17-hydroxy-pregnanolone is demonstrable after mild acid hydrolysis, too, it can be applied independently from the pregnanetriol determination. In five out of the six patients we found the excretion of the pregnanediol to be high, too. Its value reached, irrespective of sex, the characteristic quantity of the woman's luteal phase, so it undoubtedly originated from the adrenal cortex. We dealt shortly with the explanation of high pregnanediol excretion.

Steroid production by quartered and capsular rat adrenal gland in response to haemorrhage.

60 min after rapid bleeding (1.5--2.0 per cent of b. w.) both aldosterone and corticosterone production rate by quartered rat adrenals were found to be elevated. However, no difference was observed in the rate of aldosterone and corticosterone production by capsular adrenals of sham operated and hypovolaemic rats. Corticosterone production rate by decapsulated adrenals was much more higher after haemorrhage than in the control group. The same alterations could be observed incubating adrenal tissue with ACTH (0.3 mug per ml). Steroid production rate by quartered adrenals of sodium deficient rats was not affected by high in vitro concentration of angiotensin II (2.5 mug per ml). It is concluded that the effect of acute blood loss on corticosteroid biosynthesis of the rat is mediated by ACTH alone.

Influence of hormonal status of patients with cystic disease on the composition of cyst fluid and breast cancer risk.

The relationship between the composition of breast cyst fluid (BCF), the menstrual status and in addition some endocrine events in the history of patients (n = 131) with gross cystic breast disease was investigated. The dehydroepiandrosterone (DHA) levels in type II (K+/Na+ < 1) cysts of the follicular group were significantly higher compared to the type II cysts of the luteal or postmenopausal groups. For testosterone a significant difference existed between the type I (K+/Na+ > or = 1) follicular and type I postmenopausal groups. Estrone levels were significantly higher in type I BCF of patients in the luteal phase compared to both the follicular and postmenopausal type I cysts. Progesterone levels were lowest in the postmenopausal subgroups (both in type I and II cyst). Significant correlations were found between the number of pregnancies and the levels of DHA-sulfate and also progesterone in BCF. DHA levels were correlated with the period of lactation. The K+/Na+ ratios were the lowest in women who lactated for the longest period. The estrone was lowest in BCF of current oral contraceptive (o.c.) users while the estradiol was lowest in patients who had never used o.c. A history of previous o.c. use was associated with a significantly high mean DHA level. A significantly higher DHA and lower testosterone level were demonstrated in BCF of patients who had some previous gynecological interventions. The composition of BCF and the "life of cysts" and thus the rate of breast cancer risk may depend on hormonal status during the menstrual cycles or postmenopause and also on endocrine history of patients.

Lysosomal enzyme activities in frozen, non-cultured chorionic villi for prenatal diagnosis of enzymopathies.

Normal reference values of lysosomal enzyme activities (alpha-glucosidase, mannosidase, fucosidase and arylsulfatase-A) were determined in chorionic villi obtained from artificial abortion in the first trimester of normal pregnancies (gestational weeks 6 to 11). Villi were homogenized comparatively either in saline or in Triton X-100 detergent. The alpha-glucosidase, mannosidase and arylsulfatase-A enzyme activities significantly diminished if homogenization was done in saline instead of Triton-X while the difference in fucosidase activity was not significant. Significant correlation was detected between alpha-glucosidase activity and week of gestation. It is suggested that Triton X-100-homogenization should be used for the lysosomal enzyme determinations in chorionic villi because the solubilization of enzymes from the lysosomes is complete in this case than with homogenization in saline.

Normal lipid values (cholesterol, triglycerides and HDL-cholesterol) for children in a rural area of Hungary. A screening program.

Screening for serum total cholesterol (Ch), triglyceride (Tg) and HDL-Ch levels was performed in 3 Hungarian rural areas among 3-14-year-old children, divided into three groups: 3-6, 6-10 and 10-14-year-old, respectively. The mean levels as well as the 85, 90 and 95 percentile values of the investigated lipids are given. The serum HDL-Ch levels were very low in all three groups: 0.96 mmol/l in group I. (3-6 years), 0.99 mmol/l in group II. (6-10 years) and 1.04 mmol/l in the group III. (10-14 years). We did not found any correlation between the age and lipid parameters of the children. Significant positive correlation was detected between the serum Ch and HDL-Ch levels, and a significant negative correlation was observed between the serum Tg and HDL-Ch concentrations. Our data represent the normal lipid values (Ch, Tg, HDL-cholesterol) for 3-14-year-old children, living in rural area.
