Study on endocrine disrupting chemicals in wastewater treatment plants.

From July 1998 to March 1999, a study was made of a total of 27 treatment plants for the principal purpose of understanding the actual condition of endocrine disrupting chemicals (EDCs) in sewage, and the behavior of EDCs in wastewater treatment plants. The results showed actual levels of influent and effluent concentrations of EDCs in sewage. Substances detected above the minimum limit of determination were 15 for wastewater influent and 6 for effluent. Similarly, nonyl phenol ethoxylate and 17 beta-estradiol, which are highlighted as pertinent substances, were detected. It was confirmed that the reduction ratio of EDCs in treatment plants was 90% or more for almost all substances. The behavior of EDCs in general in treatment plants was also studied. As a result, the EDCs reduction effect was recognized in both the primary setting tank and biological reaction tank, though the trend varies among substances.

[An approach for the quality control in clinical microbiology laboratory by antibiotic susceptibility].

It is important to develop a system for the prevention of various errors and the maintenance of the high quality level in the department of laboratory medicine. We developed a computer program to infer the species of microorganisms by assessing the minimal inhibitory concentration of respective microorganisms to various antibiotics, antibiotic susceptibility patterns(ASP). Retrospective analysis of 3,405 strains from 39 species using this program resulted in a sensitivity of 91.4% and specificity of 98.9%. Furthermore, in 1,660 strains isolated from September 1999 to August 2000, the efficacy of this program was evaluated using both this program and conventional identifying method such as VITEK, api and ATB. The discrepancy between this inferring method and conventional method was noted in 179 strains, revealing errors in 12 strains. We conclude that this ASP inferring program is useful for preventing errors and the maintenance of high quality examination by combining with conventional methods in the laboratory.

[Susceptibilities of bacteria isolated from patients with lower respiratory infectious diseases to antibiotics (1999)].

From October 1999 to September 2000, we collected the specimen from 430 patients with lower respiratory tract infections in 17 institutions in Japan, and investigated the susceptibilities of isolated bacteria to various antibacterial agents and antibiotics and patients' characteristics. Of 515 strains that were isolated from specimen (mainly from sputum) and assumed to be bacteria causing in inflammation, 506 strains were investigated. The breakdown of the isolated bacteria were: Staphylococcus aureus 78, Streptococcus pneumoniae 101, Haemophilus influenzae 104, Pseudomonas aeruginosa (non-mucoid) 58, P. aeruginosa (mucoid) 11, Moraxella subgenus Branhamella catarrhalis 41, Klebsiella pneumoniae 18, etc. Of 78 S. aureus strains, those with 4 micrograms/ml or above of MIC of oxacillin (methicillin-resistant S. aureus: MRSA) occupied 57.7%. Vancomycin and arbekacin showed the most potent activities against MRSA without detection of ABK-resistant strain (MIC: 64 micrograms/ml) and decrease of VCM-sensitive strains those were found in 1998. The frequency of S. pneumoniae exhibiting low sensitivity to penicillin (penicillin-intermediate S. pneumoniae: PISP + penicillin-resistant S. pneumoniae: PRSP) decreased to 34.7% from 46.0% in 1998. The frequency of PRSP was 3.0%, being the least number after 1991. Carbapenems showed strong activities against S. pneumoniae. Especially, panipenem inhibited the growth of all 101 strains with MIC of 0.063 microgram/ml. Generally, all drugs showed strong activities against H. influenzae with MIC80s of 4 micrograms/ml or below. MICs of ofloxacin ranged between 0.063 microgram/ml and 4 micrograms/ml in 1998, however, those were 0.125 microgram/ml or below in all H. influenzae in 1999 showing the strongest activity. Tobramycin and ciprofloxacin showed strong activities against P. aeruginosa (both mucoid and non-mucoid) with MIC80s of 1 microgram/ml. Number of isolated P. aeruginosa (mucoid) was little as 11, however, the susceptibilities to all drugs were better than P. aeruginosa (non-mucoid). K. pneumoniae showed good susceptibilities to all drugs except for ampicillin with decreasing of low-sensitive strains compared to those detected in 1998. Also, all drugs generally showed strong activities against M. (B.) catarrhalis. MIC80s of all drugs were 2 micrograms/ml or below. The drug which showed the strongest activity was imipenem inhibiting all 41 strains with MIC of 0.063 microgram/ml. On the patients' characteristics, the number of patients aged 80 years or older who had been increased was decreased in 1999 in the distribution by age. The percentage of the elderly patients aged 70 years or older was 47.0%, which occupied almost a half number of the total patients as in the last year. As for the incidence by disease, bacterial pneumonia and chronic bronchitis were the highest. They were noted in 37.9% and 30.5% of the patients, respectively. In 1999, bronchial asthma was frequently observed as compared in recent years. It was noted in about 10% of the patients which is the same % as in bronchiectasis. We examined the number of strains from these patients with infections before and after administration of antibiotics. In patients with bacterial pneumonia, the number of isolated strains was almost the same between those before and after administration. However, in patients with chronic bronchitis, the number of strains remarkably decreased to less than the half of the total after administration of antibiotics in the last year, but it decreased to 2/3 of the total in 1999. On the administration of antibiotics and isolated bacteria by the day of administration, the bacteria which were isolated more before administration were H. influenzae in 28.4%, S. pneumoniae in 25.7%, M. (B.) catarrhalis in 12.0% and S. aureus in 10.6%. The frequency of S. aureus after administration over 15 days was almost the same as that before administration, but the frequency of P. aeruginosa (both mucoid and non-mucoid) was 36.8% which was higher than that before administration. The frequency of isolated S. pneumoniae was decreased after administration and none of them was isolated after completion of administration. However, that of H. influenzae was decreased to 7.1% after administration within 3 days, and many H. influenzae were isolated after completion of administration as 21.4%.

[A case of primary pleural leiomyosarcoma].

A 70-year-old man was admitted to our hospital with complaints of chest pain and exertional dyspnea. Chest radiography and computed tomography (CT) revealed right pleural effusion and pleural thickening on admission. The pleural fluid was bloody. Microbiological and cytologic examinations of the fluid were negative. The chest CT revealed progress of pleural thickening after hospitalization. A thoracoscopic pleural biopsy was performed, and the histological finding of the excised specimen was leiomyosarcoma. Because no organ of origin of the leiomyosarcoma, other than the pleura, was detected, this case was diagnosed as a primary pleural leiomyosarcoma. It is thought that leiomyosarcoma originating from the pleura is rare.

[A case of ankylosing spondylitis associated with pulmonary aspergillosis and amyloidosis].

A 56-year-old man was admitted because of diarrhea, cough, weight loss, and disturbance of consciousness. He had been diagnosed as having ankylosing spondylitis at 18-years old. The spondylitis progressed until there was complete rigidity of the spine including the neck, hip and knee joints. Human leukocyte antigen (HLA) B27, which has been characteristic of ankylosing spondylitis, was also present in this case. A chest radiograph showed pleural thickness and a cavity in the right upper lobe; and a soft tissue mass and fluid level was found in the cavity. Aspergillus fumigatus was detected in the sputum and pulmonary aspergillosis was diagnosed. Biopsy of the colon revealed that a large interstitial amyloidosis. Despite the treatment of the patient's malnutrition and lung aspergillosis using amphotericin B, the clinical course was rapidly progressive and the patient died of respiratory failure due to lung aspergillosis. It is important to be aware of these rare complications, which are correlated with the prognosis in cases of ankylosing spondylitis.

Monitoring impact of in situ biostimulation treatment on groundwater bacterial community by DGGE.

Changes in bacterial diversity during the field experiment on biostimulation were monitored by denaturing gradient gel electrophoresis (DGGE) analysis of PCR-amplified 16S rDNA fragments. The results revealed that the bacterial community was disturbed after the start of treatment, continued to change for 45 days or 60 days and then formed a relatively stable community different from the original community structure. DGGE analysis of soluble methane monooxygenase (sMMO) hydroxylase gene fragments, mmoX, was performed to monitor the shifts in the numerically dominant sMMO-containing methanotrophs during the field experiment. Sequence analysis on the mmoX gene fragments from the DGGE bands implied that the biostimulation treatment caused a shift of potential dominant sMMO-containing methanotrophs from type I methanotrophs to type II methanotrophs.

Primary effusion lymphoma with B-cell phenotype.

We describe here a case of primary effusion lymphoma that occurred in a 78-year-old woman. She was successively treated with prednisolone but died 15 months after the diagnosis of primary effusion lymphoma. The immunohistochemistry revealed the neoplastic cells to be CD19(+), CD20(+), CD21(+), Sm-Ig(+), and HLA-DR(+). This patient exhibited clonal IgH and clonal kappa light chain gene rearrangement, indicating a B-cell origin. The present case was distinguished from the majority cases of HHV-8-positive primary effusion lymphoma. Here we present clinical details of response to therapy in this case.

Macrolides and clindamycin suppress the release of Shiga-like toxins from Escherichia coli O157:H7 in vitro.

We investigated the effects of antimicrobial agents fosfomycin (FOS), cefdinir (CDIN), levofloxacin (LEVX), rokitamycin (ROK), roxithromycin (ROX), and clindamycin (CLI) on the release of Shiga-like toxins (SLTs) by enterohaemorrhagic Escherichia coli (EHEC). EHEC was cultured for 14 h in the presence of ROX, ROK or CLI at sub-minimum inhibitory concentrations (subMICs) of 1.56-6.25 mg/l, followed by assay of the level of SLTs in the supernatants using cytotoxicity assay and reversed passive latex agglutination method. Exposure to ROX, ROK or CLI reduced the amount of released SLTs compared with untreated control cultures (P<0.05). These agents however, did not decrease the number of viable EHEC, indicating the importance of bactericidal agents. When the bacteria was exposed to CDIN, FOS or LEVX, the level of SLTs in the culture supernatant increased with the destruction of bacterial cells in the order of CDIN, FOS, LEVX. When 0.5 mg/l of LEVX was added to cultures with or without pretreatment using ROX, ROK, or CLI, the release of SLTs was reduced by this pretreatment (P<0.05). These results may have clinical implications for the treatment of EHEC infection.

Functional angioarchitectural comparison of the fungiform papillae of rat, rabbit, cat in scanning electron microscopic specimens.

The aim of this study was to compare the functional angioarchitectural fungiform papillae (FuP) of three kinds of mammalia, rat, rabbit and cat, with microvascular cast specimens (MVCS) utilizing the corrosive resin cast method in scanning electron microscopy (SEM). FuP distributed geometrically on the dorsal surface of the tongues of the three mammals with different eating habits were of various kinds of form based on their functional and microvascular structure. The basic form of the capillary loop of FuP in the three mammals consisted of the ascending and descending branches of the capillary bed. The basic form of the loop microstructure in rat FuP was a cylindrical or bamboo-basket-like loop structure. In the rabbit, FuPs consisted of a carnation-like structure and in the cat FuP consisted of a half oval fishnet ball-like structure. It can be said conclusively that from this detailed investigation of the intracapillary microvascular architecture of the three mammals, FuP are effective as a sensory organ receiving the taste of foods and liquids by the expansion of the surface area in various functional and microvascular formations in the rat (cylindrical or bamboo-basket-like loop structure), rabbit (carnation-like) and cat (half oval fishnet-ball-like) respectively.

Angioarchitectural form, functional distributive pattern and classification of the filiform papillae on the crossbred Japanese cat tongue anterodorsal surface in scanning electron microscopic specimens.

The purpose of this study was to undertake a three-dimensional comparative observation of the angioarchitectural form, functional distributive pattern and classification of the filiform papillae (FiP) as they appear on the entire dorsal surface of the front of the Japanese cat tongue using microvascular cast specimens (MVCS). By means of the corrosive resin casting technique, the MVSC of the FiP of the cat tongue were prepared and examined in detail under the scanning electron microscope (SEM). On the frontal half of the anterodorsal surface of the anterior tongue, types I-V of the FiP are arranged in the form of a A with the point in the direction of the apex and in an oblique line running from the antero-central to both postero-peripheral regions. In the rear half of the anterodorsal surface of the anterior part of the tongue, types I-V of the FiP are arranged in the form of a V with the point in the direction of the root and in an oblique line running from both antero-peripheral regions to the postero-median region or towards the pharynx on the anterior centro-dorsal surface of the tongue. The FiP, arranged in an oblique line running fron the central to the apical part of both the periphery and the pharyngeal region of the cat tongue, can be classified into five types (Types I-V) according to the shape and size of the main process (MP), numbers of the accessory processes (AP) and regional position of the lozenge arrangement. FiP Types I-III consisted of an MP which contained a large spoon-shaped and concave network process, and the AP contained a bundle of spin-like processes arranged radially at the anterior basal margin of the MP. FiP Types IV and V consisted only of MP. It was conjectured that the lozenge arrangement of the A and V form FiPs, classified into five types (Types I-V) from the frontal portion of the anterodorsal surface toward the pharynx on the front of the tongue, play a functionally assistant role in the mastication of food and sucking of liquid, including milk.

Angioarchitectural form, functional distributive pattern and classification of the fungiform papillae on the crossbred Japanese cat tongue anterodorsal surface in scanning electron microscopic specimens.

The purpose of this study was to undertake a three-dimensional comparative observation of the angioarchitectural form, functional distributive pattern, and to classify the fungiform papillae (FuP) found sporadically and geometrically on the oblique lines of the filiform papillae (FiP) on the entire dorsal surface of the crossbred Japanese cat foretongue in microvascular cast specimens (MVCS). By means of the corrosive resin casting technique, the MVCS of the FuP of the cat tongue were prepared and examined in detail under the scanning electron microscope (SEM). With regard to the arrangement of FiPs and FuPs on the frontal half of the antero-dorsal surface of the cat foretongue, FiP types I-V and FuP types I-IV are arranged a A form with the point in the direction of the apex and in an oblique line running from the antero-central portion to both postero-peripheral portions. In the rear half of the antero-dorsal surface of the foretongue, FiP types I-V and types FuP I-IV are arranged in a V form with the point in the direction of the radix and in an oblique line running from both antero-peripheral portions to the postero-median portion or towards the pharynx on the anterior centro-dorsal surface of the cat foretongue. With the FuP arranged in the medial zone (M) and peripheral zone (P) running from the central to the apical parts, both the peripheral parts and the pharyngeal can be classified into four types (FuP types I-IV) according to the shape and size of the main process (MP), the number of the accessory processes (AP) and the regional position of the lozenge arrangement. FuP Types I-III consisted of a MP containing a large half oval, network-like process, and the AP contained a bundle of spine-like processes arranged radially in a circle at the anterior basal margin of the MP. The FuP Type IV was made up of only one MP. In the central zone (C) of a lozenge part arranged in the A and V forms, there was no FuP as a taste organ. It was assumed that in the lozenge arrangement of the A and V forms, except for the C zone, FuPs could be classified into four types (FuP types I-IV) from the frontal portion of the antero-dorsal surface toward the pharynx on the foretongue, and would play an assistant functional role in receiving the sense of taste of foods and liquids.

Allogeneic bone marrow transplantation-related transmission of human T lymphotropic virus type I (HTLV-I).

We report here the first case report of bone marrow transplantation (BMT)-related transmission of human T lymphotropic virus type-I (HTLV-I). Antibodies against HTLV-I-associated antigens (anti-HTLV-I) were detected in the serum from the BMT recipient 12 days post BMT. IgG against gag core proteins (anti-p19 and anti-p24) appeared earlier than IgM against gag and env proteins (anti-p19, anti-p24 and anti-gp46) during seroconversion. The data presented here differs from blood transfusion-related seroconversion. This phenomenon may be due to the engraftment of anti-HTLV-I producing cells from the donor.

Viability of Escherichia coli O157:H7 in natural river water determined by the use of flow cytometry.

The enzymatic activity and viability of Escherichia coli O157:H7 in natural river water was determined by flow cytometry. River water was collected at two sites (an agricultural area and an industrial area) on the Aigawa River (Osaka, Japan). To facilitate estimation of the physiology of E. coli O157 in natural river water, bacterial cells in the water were stained with 6-carboxyfluorescein diacetate (6CFDA) and propidium iodide (PI). The cells were sorted into two populations, using a flow cytometer, based on their esterase activity. Each population was stained with E. coli O157:H7 fluorescent antibody (FA), and E. coli O157:H7 cells were observed in the esterase-active population. River water samples collected at the same points were incubated with yeast extract containing antibiotics to prevent cell division, and bacterial cells in the incubated samples were stained with PI and FA. Escherichia coli O157:H7 existed in both the viable (elongated and/or fattened) and inactive bacterial population determined by flow cytometry. These results indicate that E. coli O157:H7 may retain metabolic activity and growth potential in the natural aquatic environment.

Ovarian leiomyosarcoma: an autopsy case report.

Primary non-specific sarcoma of the ovary is extremely rare, and only 22 reported cases of pure leiomyosarcoma (LMS) are known to the authors. We present an autopsy case of a primary ovarian leiomyosarcoma in a 73-year-old woman. She had noticed an abdominal mass after difficulty in defecating for several months. The excision of tumor with bilateral salpingo-oophorectomy and hysterectomy was carried out. A diagnosis of pure leiomyosarcoma of the left ovary was made on pathological examination with immunohistochemistry. Adjuvant radio-chemotherapy was not given. At 18 months' follow up, abdomino-pelvic sonography revealed an abdominal tumor and hepatic metastasis. The patient died 3.5 years after the initial surgery. The post-mortem examination revealed a peritoneal recurrent tumor and extensive distant metastases of the liver, lungs, pancreas, gastric mucosa, muscle and skin. The prognosis of the ovarian LMS is poor from the pertinent literature. Several prognostic indicators on histology including mitotic activity, proliferative activity and p53 status of the tumor are discussed.

[A case of tuberculous peritonitis diagnosed by ultrasonography-guide peritoneal biopsy].

The diagnosis of tuberculous peritonitis is quite difficult because the symptoms are not specific for the disease and the incidence of occurrence are relatively rare. We report a case of tuberculous peritonitis diagnosed by ultrasonography-guided peritoneal biopsy. A 64-year-old male was admitted to our hospital because of fever, dyspnea and abdominal pain. Laboratory findings revealed an elevated ESR (53 mm/1 hr.) and positive CRP. The tuberculin skin test was negative. The chest radiograph revealed bilateral pleural effusion. Abdominal ultrasonographic examination and computed tomography showed ascitic fluid, thickening of the mesentery and peritoneum, and inflammatory pseudotumor of the omentum. Ascitic fluid was exudate with a high lymphocyte count and elevated ADA (184 IU/l). Microbiological studies with the fluid were negative. Peritoneal biopsy guided by ultrasonography was performed, and the specimens showed central caseous necrosis surrounded by epitheloid cells and acid-fast bacilli were demonstrated. The size of the pseudotumor, pleural effusion and ascites decreased after antituberculous chemotherapy with corticosteroid was given. Diagnosis of tuberculous peritonitis has often been made by laparotomy or laparoscopy. In a case of this kind, percutaneous peritoneal biopsy guided by ultrasonography is safe and useful.

Improved direct viable count procedure for quantitative estimation of bacterial viability in freshwater environments.

A direct viable count (DVC) procedure was developed which clearly and easily discriminates the viability of bacterial cells. In this quantitative DVC (qDVC) procedure, viable cells are selectively lysed by spheroplast formation caused by incubation with antibiotics and glycine. This glycine effect leads to swollen cells with a very loose cell wall. The viable cells then are lysed easily by a single freeze-thaw treatment. The number of viable cells was obtained by subtracting the number of remaining cells after the qDVC procedure from the total cell number before the qDVC incubation. This improved procedure should provide useful information about the metabolic potential of natural bacterial communities.

Experimental model of progressive disseminated trichosporonosis in mice with latent trichosporonemia.

Trichosporon asahii and Trichosporon mucoides are the most common strains of fungi that cause disseminated trichosporonosis, a severe opportunistic infection in immunocompromised hosts. We have previously established a nested PCR assay using serum samples for detection of both strains. Here we describe a new experimental animal model for investigating the underlying mechanisms of disseminated trichosporonosis. T. asahii (OMU239, a clinical isolate from a patient with acute myelogenous leukemia) and 8-week-old ICR male mice were used in all experiments. A suspension of T. asahii (3 x 10(6) CFU/animal) was injected into the caudal vein of each mouse after immunosuppression with cyclophosphamide (200 mg/kg of body weight/day for 2 days) and prednisolone (30 mg/kg/day for 1 day). Mice were then divided into four subgroups (R0, R1, R2, and R3) based on the time of reimmunosuppression. The latter was performed using the same drugs 1 week (group R1), 2 weeks (group R2), and 3 weeks (group R3) after fungal infection. Reimmunosuppression was not performed in group R0. The 5-week-survival rates of mice after T. asahii infection were 0% for group R1, 50% for group R2, 80% for group R3, and 80% for group R0. There was a significant difference in the survival rates between group R1 and either group R0 or R3 (P < 0.05). Fungal clearance in peripheral blood and various organs of group R1 and R2 was delayed relative to that of group R0 but was similar to the control in group R3 in spite of reimmunosuppression. Our results suggest that the critical period for the development of disseminated trichosporonosis in our model is shorter than 3 weeks after T. asahii infection. We concluded that mice during this critical period were in a state of latent trichosporonemia. Comparison of the survival rates suggests that the nested PCR assay was more useful than blood culture and glucuronoxylomannan antigen assay in the detection of this latent trichosporonemia.
