[Impact of smoking and alcohol consumption on mortality in the population of Barcelona [Spain]: 1983-1998].

OBJECTIVES: To analyze the mortality attributable to smoking and alcohol consumption in the city of Barcelona from 1983 to 1998. METHODS: All deaths among residents of the city of Barcelona from 1983 to 1998 were included. Population data were obtained from the city inhabitants register. The mortality attributable to smoking and alcohol consumption was calculated by population attributable fractions using relative risks from previous studies in the US population. RESULTS: In 1998, 2,205 deaths were attributable to smoking, representing 13.8% of all deaths among the population aged 35 years or older and a decrease of 9.6% compared with deaths in 1983. From 1983 to 1998 there was an increase in smoking-attributable adjusted mortality rates for lung cancer (155.84/100,000 inhabitants in men and 9.39/100,000 in women in 1998) and chronic obstructive pulmonary disease (COPD) (95.89/100,000 in men and 11.29/ 100,000 in women in 1998). In 1998, deaths attributed to alcohol consumption accounted for 4.3% of total mortality, representing a reduction of 26% since 1983. Among men, the primary cause of alcohol-attributable mortality was liver cirrhosis (17.1%), although its relative importance decreased (accounting for 17.1% of alcohol-attributable mortality in 1998 compared with 24.3% in 1983). CONCLUSIONS: The mortality attributable to smoking and alcohol consumption decreased in the city of Barcelona during the study period. Smoking-attributable mortality from lung cancer and COPD increased in both sexes. Alcohol consumption-attributable mortality from liver cirrhosis decreased in men.

[Trends in tobacco, alcohol and cannabis consumption among secondary school pupils in Barcelona, Spain (1987-1999)].

OBJECTIVE: To describe trends in tobacco, alcohol, and cannabis consumption among secondary school pupils in Barcelona between 1987 and 1999. METHODS: Cross-sectional surveys administered to representative samples of pupils in the second year of secondary school between 1987 and 1999. The questionnaires were anonymous and self-completed. We present data from 5013 secondary school pupils from Barcelona who participated in one of the five surveys. RESULTS: Regular smoking (daily and weekly) showed a uniform decrease between 1987 and 1996. The results of the last survey (1999) showed an increase over those the 1996 survey, ranging from 13.4% for regular smoking to 38.7% for experimental smoking. Between 1987 and 1999, the percentage of schoolchildren who reported drinking at least half a glass of alcohol at some time showed an overall decrease of 14.0%. No differences in sex were found in problematic alcohol consumption, while a higher proportion of girls (14.0%) than boys (10.5%) reported getting drunk at some time. Whereas in 1996, 6.9% of the population studied had smoked cannabis at some time, in 1999 this proportion was 7.3%, with no differences between sexes. CONCLUSIONS: During the study period regular smoking increased, alcohol consumption decreased and cannabis consumption tended to level-off. Differences between boys and girls tended to disappear.

[Adolescent pregnancy in Barcelona: its distribution, antecedents and consequences].

This paper describes the distribution of fertility and abortion rates in the city of Barcelona (Spain) in adolescent in 1990. The adolescents' socio-demographic background and characteristics of their family unit obtained from the statistically bulletin of birth are analyzed. In this year, there were 248 births and 195 abortions to women between 15 and 19 years-old, yielding a rate of 3.9 and 3.1 per thousand, respectively, so that at least 7 per 1000 were pregnant. Fertility rates varied by district between 0.8 in Sarrià-Sant Gervasi and 14.6 in the inner city district of Ciutat Vella, with a high positive correlation of these rates and a district social problem index (r = 0.96) and a high negative correlation with an index of family economic wealth (r = 0.93). Around two thirds (63%) of the adolescent mothers reported to be living with their parents after childbirth, and their companion joins this unit in four out of ten cases. In 14.5% of births, the father's age under 20, although the most of them (39.1%) were between 20 and 24 years-old. Although teenage fertility rates have been declining during the last years in Barcelona, the strong correlation with socio-economic indicators encourages the design of specific programs addressing socially disadvantaged groups. In addition, several risk factors can be used to identify adolescent mothers at risk when designing specific interventions such as the lack of recognized paternity by the father, dropping out of school, or lacking family's support.

[Inequalities in maternal-child health: impact of an intervention].

We present the evaluation of the population impact of a social and health maternal and child intervention, implemented in a low income urban area (Ciutat Vella) in Barcelona, Spain. Intervention was based on increasing access to health and social services for pregnant women and for the children. A quasiexperimental design with a nonequivalent control group and multiple measurements was used. We compared infant and perinatal cumulative mortality rates between Ciutat Vella District and the rest of the city, before (1983-86) and after the intervention (1987-89, 1990-92). Results showed that the significant differences found previously to the program between the mortality rates disappeared with the development of the program. Infant mortality in Ciutat Vella was 17.7/.000 births in 1983-86, and became 13.1/1.000 births in 1987-89, and 13.4 in 1990-92; in the rest of the city, infant mortality was 9.5/1.000 births in 1983-86, and became 8.8/ and 7.7/1000 births respectively. A comprehensive social and health care program implemented in small low income areas may contribute to reduce inequalities in maternal and child health.

[Social support and perceived health status in non-institutionalized population over 60 years of age].

OBJECTIVES: The purpose of this study is to describe the degree of social support in a sample of non-institutionalized population aged 60 and over and to explore possible relations between social support and perceived health. METHODS: We analyse data coming from Barcelona 1992 Health Survey. Sociodemographic and perceived health variables are analysed together with several social support variables based on the presence and number of available people for instrumental and emotional support necessity and perceived aid need for personal care and daily needs. RESULTS: In the study period, 1156 subjects aged 60 and over were interviewed at home. 20% of the sample (28.9% were women) lived alone and 28.2% (41.7% were women) were widowed. 43% had an extensive social support and 51% had a good emotional support. The men with less emotional support had a worse perceived health (OR = 2.7, IC 95% = 1.3-5.5). Only aid need for daily works in women was associated with worse perceived health (OR = 6.3, IC 95% = 3.2-12.4). CONCLUSIONS: Instrumental and emotional support availability weren't associated with perceived health in women in spite of they lived alone in a greatest proportion. In men, only emotional support absence was associated with worse perceived health.

[Smoking among nursing students in Catalonia: knowledge, attitudes and practice].

OBJECTIVE: Determine the prevalence and characteristics of tobaccoism, as well as some attitudes and their knowledge about tobaccoism in nursing university students in Catalonia. METHODS: A descriptive study with transversal section has been done. A self-filling anonymous questionnaire was designed following the guidelines of the European Regional Office of the W.H.O. We selected a sample of conglomerates of classrooms at random which was stratified according to levels in all the university schools in Catalonia. The field work was performed during the first term of the academic year 1994-1995 handing the questionnaires individually to the students and collecting them once filled. In the data analysis we used the ji squared test to Pearson with the Yates' correction and the lineal tendency test of Mantel-Haenszel. RESULTS: 904 students answered the questionnaire. The global prevalence of smokers is 38.7% (IC 95%: 35.8-41.6). There were no significant differences when considering levels or sex. The prevalence of tobaccoism in students over 24 years of age is 13% higher (p < 0.01) than in the group between 18 and 24 years of age. The group of daily smokers consumes an average of 12 cigarettes per day (DE: 5.9 cig/day). Significant differences were observed between smokers and nonsmokers in all the attitude variables analysed towards tobaccoism. 32% of third year students consider their knowledge is not enough in order to give advice against tobacco. CONCLUSIONS: A prevalence higher than 30% of the established objective in the Health Plan in Catalonia has been observed for the nursing professionals in the year 2000.

[Habitual physical activity in a population residing in the city of Barcelona].

OBJECTIVES: To describe the sociodemographic distribution of habitual physical activity and to analyse its relationship with self-perceived health status and occupational status, among the population older that 16 years of age in the city of Barcelona, Spain. METHODS: A sample of 1,885 adult men and 2,196 women answered the Health Interview Survey of Barcelona in 1992. Those whose habitual activity required high physical exertion or walking were considered as active. Bivariate and multivariate analyses adjusting logistic regression models were used to study the relationship between habitual physical activity and the rest of variables, for each occupational situation. RESULTS: Fifty six percent of adults reported being physically active. Physical activity was lower among workers than non workers, although workers reported more physical exertion (14.5% in men and 8.0% in women). Physical activity was associated with occupation and educational level among male workers, but only with occupation among female workers. In the non working population, physical activity was lower among those who perceived their health status as fair or poor, compared with those that described it as good or very good, (ORa in men was 0.2, 95% confidence interval = 0.1-0.5; ORa in women was 0.5, 95% confidence interval = 0.3-0.8). CONCLUSIONS: More than half of the Barcelona population were physically active in an habitual manner. Among workers, this activity was mainly determined by the occupation. Non-workers with a poor self-perceived health status did less physical activity.

[Health-related behaviors in secondary-school students: sexual relations and tobacco, alcohol and cannabis consumption].

BACKGROUND AND OBJECTIVES: The main premature mortality causes among youngsters are related to risk behaviors, usually initiated in adolescence. The study objective is to describe substance use and sexual behaviours among 10th-grade Barcelona students in 1996 (last year of compulsory education), as well as the interrelations between these variables and several sociodemographic variables. METHODS: Cross sectional study. Random sample including 35 10th grade classrooms (958 students whose mean age is 15.8 years old) stratified by academic or vocational studies, public or private school, school neighbourhood socioeconomical level and school size. Measurement instrument is a previously validated questionnaire. Tobacco, alcohol, and cannabis consumption, sexual intercourse as well as gender, age, weekly available money, parental instruction and type of studies are studied. Bivariate chi 2 analysis and multivariate gender specific log-linear model are performed. RESULTS: 27% of the students smoke daily and 31% drink alcohol weekly. 15% have had sexual intercourse, and among those 79% use always or almost always condoms. Among girls tobacco consumption is related to alcohol (OR = 4.2), to cannabis (OR = 5.9) and sexual intercourse (OR = 3.9), and, less strongly, with age, available money and vocational studies. Alcohol is associated with tobacco and cannabis use (OR = 4.2). Having had sexual intercourse is related to tobacco use, age (OR = 3.4), vocational studies (OR = 2.4) and cannabis experience (OR = 2.8). Among boys tobacco consumption is related to alcohol, (OR = 2.7), to cannabis (OR = 7.6) and sexual intercourse (OR = 4.4), and, less strongly, to available money and type of studies. Alcohol consumption is associated with tobacco and cannabis use (OR = 5.5). Sexual intercourse is related to tobacco use and age (OR = 2.5). DISCUSSION: Risk behaviors among final year secondary school students are strongly and significantly interrelated, both among boys and among girls, and are more frequent among older students, those with more money available and those in vocational curricula.

[Evaluation of a community intervention program ("Quit and Win") for smoking cessation].

OBJECTIVES: In recent years community intervention plans for motivating smokers to stop smoking have been developed. One of these programs is the < > initiative of the World Health Organization. Our study evaluated the impact of this intervention in a sample of persons who participated in the 1996 edition of the program in Barcelona, Spain. METHODS: One year after the contest, a telephone follow-up survey was carried out in one-third of the participants. Subjects were questioned about their current smoking status and their participation in the contest, including motivation and whether they had remained abstinent throughout the contest. RESULTS: A total of 941 valid cards were received from Barcelona residents, equivalent of 0.23% of all smokers. A selection was made of 307 persons for follow-up, 18 of which were excluded because they were former smokers. From the remaining 289 participants, we obtained valid responses from 196, 82 (41.8%) of which were abstinent when the survey was made. Participating in the contest with the intention to stop smoking permanently and continued abstinence throughout the contest period (one month) were significantly associated with abstention one year later. The overall agreement between participants' statements and the statements of a subsample (n=18) was 81.3%. CONCLUSIONS: One year after the intervention the proportion of abstinent smokers was higher than in earlier studies. However, self-selection by the participation who responded to the follow-up questionnaire could have contributed to the final result. Overall, such initiatives could be useful in motivating predisposed persons to stop smoking.

[Factors associated with reported vaccination coverage in early infancy: results of a telephone survey].

BACKGROUND: Barcelona's Continuing Immunization Plan affords the possibility Of monitoring the immunization coverage of the population by means of the voluntary family postal notification system. Prior studies have revealed that some families fail to provide notification while being correctly vaccinated, which can lead to actual coverage being underestimated. The objectives of this study are to estimate the early childhood immunization coverage of the population and to ascertain the factors associated with failure to provide notification of immunization. METHODS: A phone survey was conducted on a sample of 500 children regarding whom there was no record of any notification of the first three childhood vaccine doses (diphtheria, tetanus, whooping cough and oral polio), in addition to a sample of 500 children who were on record as having been immunized. To estimate the actual immunization coverage, all children were considered to have been properly immunized when their family members did provide notification. As regards those who failed to reply, it was considered in the worst of cases that these were cases of children who had not be immunized. In the best of cases scenario, a coverage similar to those of the responses was assumed. RESULTS: The response to the questionnaire was higher among those who had previously provided notification of immunization by way of the postal notification system (79.1%) than among those who had failed to provide notification of immunization (67%). The leading factors associated with failure to report immunization status were the size of the families, the use of private health care services and the place of birth of the parents. Solely six (6) cases of those who had failed to report immunization admitted to not having immunized their children, totaling 1.9% of the responses. The immunization coverage of the population in question would total 99.7% in the best of cases and 93.7% in the worst of cases scenario. CONCLUSIONS: Immunization coverage of the population in question is quite high. The results underline the importance of promoting immunization notification among health care professionals, especially in the private sector.

[Factors associated with smoking and the intention to smoke in secondary school pupils: results of the ESFA project in Barcelona, Spain].

OBJECTIVE: To describe the factors asssociated with smoking and intention to smoke among a representative sample of first-grade pupils of secondary education (12-13 years old) in Barcelona. SUBJECTS AND METHODS: 37 secondary schools were randomly selected to evaluate the impact of a European-wide smoking prevention program (European Smoking prevention Frame Approach, ESFA project). In these schools, 1041 pupils of first grade of secondary education responded to a questionnaire designed at the University of Maastricht to study life-style attitudes and behaviours, mainly smoking. RESULTS: 9.8% of boys and 12.6% of girls declared to smoke either regulary or ocasionally. Among the no-smokers, 61.4% of the boys and 73.3% of the girls reported to have the intention to smoke in the future. Among the boys, factors associated with smoking included social norms (odds ratio [OR] = 2.5; 95% confidence interval [CI]: 1.2-5.2), smoking by siblings (OR = 2.4; 95% CI, 1.1-5.2), attitudes against smoking (OR = 0.3; 95% CI, 0.1-0.6), practicing some sports in the leisure time (OR = 0.3; 95% CI, 0.1-0.7) and having more available pocket money (OR = 3.2; 95% CI, 1.5-6.8). Intention to smoke was only related to attitudes (OR = 0.3; 95% CI, 0.2-0.5). Among girls, smoking was associated to perceived pressure to smoke (OR = 2.5; 95% CI, 1.2-5.0), consumption by friends (OR = 6.0; 95% CI, 2.4-15.4) and attitudes against smoking (OR = 0.2; 95% CI, 0.1-0.4), while intention was only associated to attitudes (OR = 0.4; 95% CI, 0.2-0.6) and hanging out in the street in the leisure time (OR = 2.2; 95% CI, 0.3-3.5). CONCLUSIONS: The results stress the need to deal simultaneoulsy with the different factors associated to smoking initiation and attitude shaping, including cognitive factors, environmental factors, and patterns of leisure time utilization.

[Dietary behavior disorders: opinions and expectancies on prevention and treatment strategies from the perspective of the several social actors].

AIMS: The prevalence increase of Eating Disorders in the last decades emphasizes the necessity to develop strategies from the public healthcare system which include the perspective of the principal implicated social actors. The present study aims to know their opinions and expectations regarding the prevention and treatment of Eating Disorders. METHODS: Realization of 5 focus groups (with relatives, adolescents, professionals from the health and educational sector, representatives from institutions and support groups) and 14 semi-structured interviews (with Eating Disorder patients and relatives) about strengths and weaknesses of actual prevention and treatment programs, expectations of change and suggestions for improvement. Analysis of contents and triangulation of results. RESULTS: The absence of resources and knowledge about Eating Disorders, the scarcity of transversal programs and insufficient coordination were named as the main weaknesses whereas interdisciplinary pilot programs were positively valued. As principal demands, major endowment of human and material resources, increased coverage of specialized services, promotion of specific training and improvement of coordination was asked for. Regarding prevention, preference toward an unspecific approach to the subject in order to hinder imitation between young people was highlighted. The care in Day Hospitals was considered one of the most appropriate treatment modalities for the characteristics of this disease being valued the potentially therapeutic effect of contact between Eating Disorder patients. The support network, the education field and the family were named as other important pillars in a multidisciplinary approach to the disease. There was also a demand for the development of combined programs. CONCLUSIONS: The discussion of different psychotherapeutic approaches, the role of support groups in the caring network as well as the post-hospital follow-up were identified as issues open to future research.

[The prevalence of caries and associated factors in 12-year-old schoolchildren of Barcelona].

OBJECTIVE: To find the prevalence of caries in the 12-year-old school population of the city of Barcelona. DESIGN: An epidemiological crossover survey of dental caries in a representative sample of children in Barcelona. PARTICIPANTS: 739 children in the 7th year of basic education, who were twelve years old at the moment of the examination and came from 35 schools in the city of Barcelona, were examined. MEASUREMENTS AND MAIN RESULTS: Prevalence of caries was 52% (60% in girls and 45% in boys) and the CAOD index (teeth with caries, absent because of caries or permanent teeth with fillings) was 1.37 (1.70 for girls and 1.12 for boys). 51% of the permanent teeth affected by caries had been filled. The distribution of the CAOD index showed that 23% (almost a quarter of the population studied) of 12-year old schoolchildren in Barcelona had 3 teeth or more affected by caries. In the logistical regression analysis, gender (being a girl, OR = 1.69) and the absence of a regular programme of fluoride mouthwashes (OR = 1.69) were associated with having a higher caries index (CAOD equal to or above 3). CONCLUSIONS: The results show caries indices which are generally low and a tendency to stability. The programmes of fluoride mouthwashes seem to be effective in decreasing the incidence of caries.

[Leisure time physical activity and its association with demographic variables and other behaviors related with health].

OBJECTIVES: To describe leisure-time physical activity at the levels considered more beneficial for health and to analyze the association with sociodemographic variables and other health behaviors, among the population older than 14 years in Barcelona, Spain. METHODS: A sample of 4,171 adults answered the Health Interview Survey of Barcelona in 1992. Those who had participated less than three times (20 minutes at least) in moderate and/or intense exercise in the week before to the interview were considered inactive. Bivariate and multivariate analyse using logistic regression were used to study the association of leisure-time physical activity and the study variables. RESULTS: 80.7% of population older than 14 years in Barcelona participated less than three times in moderate and/or intense leisure-time physical activity and 20% did not participate in any exercise. Inactivity was higher in women, and increased significantly with age and in people with a low socioeconomic level. Men and women with incomplete primary studies were more likely to be inactive than people with graduate studies (ORa-adjusted odds ratio-1.8, 95% confidence interval-CI-: 1.1-2.9 and 1.5, 95% CI: 1.0-2.4 respectively). For men working eight or more hours with interruption per day the ORa for inactivity was 1.9 (95% CI: 1.2-2.9) with respect to men working less than eight hours without interruption. Smoking men were more inactive than non smokers. CONCLUSIONS: These result suggest that health promotion programmes to promote physical activity for adult in Barcelona should facilitate the incorporation of older people, women, people with low socioeconomic level and the working population, besides reinforcing sports habits among the youngest population.
