[Evaluation of transcatheter arterial embolization with coaxial microcatheter and micro-coil for vascular lesions].

Transcatheter arterial embolization using a coaxial microcatheter and micro-coil was performed in eight patients with vascular lesions; one each with aneurysm of the basilar artery, cerebellar artery, and pancreatic artery, pseudoaneurysm of the common hepatic artery, gastroduodenal artery, and gluteal artery, carotid-cavernous fistula, and thoracic paraspinal arteriovenous malformation. Complete occlusion was achieved in five patients with aneurysm and pseudoaneurysm by occluding the aneurysmal cavity and/or the orifice. A patient with recurrent carotid-cavernous fistula was also completely embolized. A case of basilar artery aneurysm resulted in partial occlusion because the posterior cerebral artery originated from the aneurysm. The unsatisfactory result in a case of paraspinal AVM was due to its wide extension with multiple feeding arteries. No apparent complication was seen. In conclusion, super-selective arterial embolization therapy with coaxial microcatheter and micro-coils was found to be a useful method for vascular lesions that would have been technically difficult to embolize with the standard catheter and coils.

Histopathological study of balloon embolization: silicone versus latex.

Bilateral, symmetrical, experimental aneurysms were produced with anastomosed vein flap in the carotid arteries of 24 mongrel dogs. Aneurysms were occluded with latex or silicone balloons on each side and observed angiographically from 2 weeks to 2 months. A histopathological study was performed subsequently using light and scanning electron microscopy. Rupture after balloon embolization occurred in five aneurysms; all of which were incompletely occluded by a silicone balloon. On subsequent angiograms, four silicone balloons and one latex balloon were found to have migrated into the aneurysm, resulting in aneurysmal expansion. Parent artery occlusion was more common with latex balloons than silicone balloons. Histopathologically, residual fresh thrombi, decreased proliferation of fibroblasts within the aneurysmal cavity, and poor endothelialization were present around the silicone balloon. These results suggest that the intra-aneurysmal organization, as seen in the aneurysm occluded by the silicone balloon, will be delayed because the balloon is not fixed within the aneurysm, and that this free-floating and rotating balloon causes repeated trauma to the aneurysm wall, contributing to subsequent enlargement and rupture of the aneurysm. The superior antithrombogenic nature of silicone may be responsible for the bias of such phenomena toward the silicone balloon.

Primary choroid plexus papilloma located in the suprasellar region: case report.

A 34-year-old woman with primary choroid plexus papilloma occurring in the suprasellar region is reported. No connection with the ventricular system was found during intraoperative observations. The findings of pathological examinations such as hematoxylin and eosin staining, transthyretin (prealbumin) immunoreactivity, and electron microscopy were consistent with choroid plexus papilloma. Radiologically, it was extremely difficult to differentiate from tuberculum sellae meningioma. To our knowledge, this is the first case of primary choroid plexus papilloma in this location reported in the literature.

Vascular endoscopy for intravascular surgery--an experimental study.

A combination of endoscope and high-pressure saline irrigation system was tested in the occlusion of experimental aneurysms, and angioplasty and fibrinolysis of vascular occlusive lesions in canine femoral arteries. Endoscopy provided detailed information about the aneurysmal orifice and lumen and aided balloon placement and detachment. Endoscopy visualized the dynamic changes accompanying thrombolysis and the fine intimal injury due to angioplasty invisible on angiography. Vascular endoscopy is valuable for pre-, intra-, and postoperative evaluation of intravascular interventions.

Giant meningioma fed by the anterior choroidal artery: successful removal following embolization--case report.

A 53-year-old female was admitted with apathy, left sensory and motor disturbance, left homonymous hemianopsia, and dressing apraxia. Computed tomography and magnetic resonance imaging showed a huge mass markedly enhanced in and around the right trigone. Cerebral angiography revealed rich vascularity of the tumor mainly fed by the right anterior choroidal artery. The feeding vessel was embolized with microfibrillar collagen after superselective catheterization. Seven days later, the tumor was totally removed with low blood loss. The histological diagnosis was fibroblastic meningioma. She was discharged without neurological deficit, except for left homonymous hemianopsia.

Growth of microalgae in high CO2 gas and effects of SOX and NOX.

Growth and lipid production of microalgae were investigated, with attention to the feasibility of making use of flue gas CO2 as a carbon source. The effect of a high CO2 level in artificial seawater differed from strain to strain. Three algal strains from the Solar Energy Research Institute (Golden, CO) collection were selected as good fixers of CO2 when the level of CO2 in the sparging gas was high. These algae also accumulated large amounts of crude lipids. SOx and NOx inhibited algal growth, but a green alga, Nannochloris sp. NANNO2 grew after a lag period, even when it received NO gas at the concentration of 300 ppm.

Myeloma manifesting as a large jugular tumor: case report.

The authors report a case of cranial plasmacytoma with multiple myelomas and palsy of the lower cranial nerves. The osteolytic lesion adjacent to the jugular foramen was demonstrated by an angiogram to be exceedingly hypervascular, with arteriovenous shunting resembling that seen in paragangliomas. Forty-five cases of cranial and intracranial plasmacytoma from the literature were reviewed. The findings indicate that a cranial plasmacytoma commonly appears to be a hypervascular tumor, whereas most dural tumors or intraparenchymal tumors have poor vascularity.

[A case of sialolithiasis in a two-year-old girl].

Sialolithiasis occurs due to the calculous concretion in salivary ducts or glands, but it is rare in childhood. In this paper, a case of sialolithiasis with a submandibular duct calculus observed in a 2-year-old girl is reported. This case is one of the youngest, based on a review of the literature regarding sialolithiasis found in Japan. The retrospective survey was made in 30 cases of sialolithiasis in children under 10 years of age which were reported in the Japanese literature with clear descriptions of age, sex and location. The summaries are as follows: 1) Sex difference: The difference between males and females was in the ratio 16/14. There was no significant difference according to sex. 2) Location of the salivary calcul: In 27 cases they were found in the duct of the submandibular glands, and 3 cases in the parotid glands, and no cases in the sublingual glands. 3) Term before treatment: Most cases were treated within a month after the patients has noticed the symptom. 4) Removal method of salivary calculi: Salivary calculi were removed by means of intraoral incision in most cases. 5) Number of the removed calculi: In each of all cases, one calculus was removed. 6) Size of the removal calculi: The diameter of the calculi was less than 5.0 millimeters long in most cases.
