[Effect of the antirheumatic drug Sulindac on thrombocyte function (author's transl)].

The effect of a treatment with the antirheumatic drug Sulindac (from 200 to 400 mg/day over 4 weeks) on thrombocyte function (bleeding time, platelet retention, platelet aggregation induced with suitable concentrations of ADP, epinephrine, collagen and Ristocetin) on thrombocyte count and thromboplastin time was investigated in 20 patients. In 10 healthy volunteers collagen-induced platelet aggregation was measured after oral application of 200 mg Sulindac. Analysis of the data obtained by the tests revealed no unwanted or harmful effects of Sulindac on platelet functions, platelet count and prothrombin time. No significant difference between the data before and during treatment was found. No medical drop out was registrated.

[Evaluation of storage-iron distribution in serum and reticuloendothelial system in patients with rheumatoid arthritis (author's transl)].

Serum ferritin, transferrin and the stainable non-heme iron, were evaluated in 20 patients with rheumatoid arthritis in various stages and with different medications. We found a correlation between the serum ferritin values and the amount of stainable non-heme iron in the bone marrow, which was independent to the stage of the disease and the medication. The pathogenesis of the anemia in rheumatoid arthritis is discussed, together with the reported results in the literature.

[Extravascular lung water in traumatic shock of the dog (author's transl)].

Fourteen dogs underwent standardized traumatic hemorrhagic shock. Periodically, measurement of extravascular lung water (EVLW) was taken by thermo-green-dye dilution technique and related to microvascular pressure (MVP) values. An initial drop in EVLW, corresponding to MVP was seen. During shock time the EVLW rose again, while the MVP remained at low levels. This shows a permeability leak during shock time.

[Characterisation of activity of a biphasic monocomponent insulin (author's transl)].

13 automated continuous measurements of blood glucose over a period of 24 hours were registered in 10 diabetics with two daily doses of highly purified biphasic monocomponent insulin Rapitard (Novo) using an automated analysis system based on the autoanalyser II (Technicon). It was shown that the action of MC Rapitard insulin starts within 30 minutes after subcutaneous injection. There is a pronounced initial activity which is clearly superior to that of MC Monotard insulin. The maximum of activity is after 2-4 hours and lasts at least for 8 and sometimes 11 hours after application. Not counting two men with brittle diabetes MC Rapitard insulin was able to keep postprandial blood glucose changes within narrow limits. Thus there is a good effectiveness against hyperglycaemic peaks. Despite the b.i.d. application of this relatively long acting intermediary insulin there were no undesirable effects of an overlap. As expected the effectiveness of MC Rapitard insulin does not differ from the earlier non-chromatographically purified Rapitard insulin preparation.

Stimulation of human prolactin secretion by mescaline.

Prolactin (PRL) and Growth Hormone (GH) secretions were studied in human serum after the oral administration of 5 mg/kg mescaline (3,4,5-trimethoxy-beta-phenylethylamine) or 2,3,4-trimethoxy-beta-phenylethylamine (2,3,4-TMPEA) respectively. Mescaline stimulated the secretion of PRL more than four-fold above base-line levels. Peak concentrations were found 90--120 min after drug intake. Five hours later serum PRL was still markedly increased. Mescaline also triggered GH secretion. There was no alteration of serum PRL and GH concentrations after intake of the non-hallucinogenic 2,3,4-TMPEA.

[Transferrin and serum ferritin in tumor patients].

Serum iron, an iron absorption test, transferrin and serumferritin were investigated in patients with malignant disease. A positive correlation was found between increased serumferritin and decreased transferrin and the clinical tumor staging in single cases of acute leukemia and in advanced tumor stages.

[Serum ferritin and cytochemical storage iron in erythropoiesis and the R.E.S. in iron deficiency and faulty iron metabolism].

In 60 patients with iron deficiency anemia of chronic disorders serum iron, an iron absorption test, serum transferrin, serumferritin and stainable non-heme iron in erythropoiesis and reticuloendothelial system, were evaluated. Intercorrelations of these parameters were studied in different diagnostic groups. Pathogenetic principles and the clinical value of the applied diagnostic methods are discussed.

[Haemodialysis in the treatment of biguanide-induced lactate acidosis (author's transl)].

Severe lactate acidosis developed in nine diabetics on biguanide. When lactate acidosis was diagnosed all patients had reduced renal function, six being oligoanuric. Pre-existing chronic renal failure as a factor in the development of lactic acidosis was excluded in five patients, normal renal function being restored later. All patients were dialysed, seven surviving. This mortality rate is lower than that reported by others for biguanide-induced lactate acidosis. Rapid biguanide elimination by dialysis was demonstrated both in vitro and in vivo. The in vivo clearance of buformin was 83 +/- 43 ml/min (mean +/- SD, n = 4), that of phenformin 68 +/- 33 ml/min (n = 7). The main advantages of haemodialysis in the treatment of biguanide-induced lactic acidosis are rapid removal of toxic biguanides and excess lactate and the ability to administer sodium bicarbonate adequately without risking hypernatraemia and fluid overload.

Treatment of cryptorchidism with pernasal gonadotropin-releasing hormone therapy.

Twenty-five boys between 1 and 10 years of age with unilateral or bilateral cryptorchidism were treated with 200 microgram of gonadotropin-releasing hormone (GnRH) pernasally six times daily until descensus was completed, or for 10 weeks at most. Complete descent of the tests occurred in 16 patients, usually after 2 to 5 weeks of treatment. No adverse side effects have been observed. Radioimmunologic measurements of serum luteinizing hormone (LH), follicle-stimulating hormone (FSH), testosterone, and prolactin, carried out before treatment, at the end of treatment, and 6 months after treatment, showed a transitory increase of the LH responsiveness to GnRH in only four of the patients and in the group as a whole a slight but significant decrease of FSH responsiveness. There were no signs of precocious puberty. GnRH antibodies were not found.

Group B streptococci in the female genital tract and nosocomial colonization of newborns.

In a long-term study, rates for group B streptococcus prevalence in the vagina and/or anus of nonpregnant women and pregnant women in the first trimester and in the second trimester were found as follows: 7.1%, 7.3% and 5.4% respectively. At delivery, positivity rates of 4.8% and 3.8% were found for mothers and newborns, respectively. Each pair of strains isolated from mother and newborn was of the same serotype. In 13.6% of newborns nosocomial colonization of the upper respiratory tract occurred during their stay in the maternity home. The sources were other positive newborns sharing the same boxes, the nosocomial spread proceeded in clusters. Detailed type-antigen analysis of isolates allowed identification of index cases and their direct contacts. Types Ib, Ic, II/R, III + Ic predominated. Sexual transmission was demonstrated by findings of identical serotypes in both partners: 23% of male partners of nonpregnant positive women and 9.1% of partners of pregnant positive women had positive urine samples. A control male group consisting of psychiatric sanatorium patients without sexual contact repeatedly exhibited a positivity rate of 2%.

Investigations on the polymorphism of sperm diaphorase in man. Evidence for a third common allele, SD.

The polymorphism of sperm diaphorase (SD) was investigated in 141 unrelated persons from Hessen, Germany, by high voltage thin-layer agarose gel electrophoresis (Age) and thin-layer isoelectric focusing on polyacrylamide gel (Pagif). In addition to the three known common phenotypes SD 1, 2-1, and 2, two further phenotypes with the preliminary designation SD 3-1 and SD 3-2 were discovered. This polymorphism can thus be explained in terms of three alleles, SD1, SD2, and SD3 segregating at an autosomal locus. The allele frequencies calculated from the five different phenotypes SD 1, 2, 2-1, 3-1, and 3-2 are: SD1 = 0.7553, SD2 = 0.2234, and SD3 = 0.0213. As we also found SD activity in female reproductive tract tissues (ovaries, oviducts, uterus), the term 'gonadal diaphorase' (GD) appears to be applicable.
