A nosocomial parvovirus B19 infection-induced transient aplastic crisis in a patient with chronic renal failure.

A male patient aged 67 years with chronic renal failure (CRF), who had undergone hemodialysis since June 3, complained of dyspnea while walking on June 23, 1998. Rapidly progressive anemia and severe reticulocytopenia were noted. Serological tests showed that parvovirus B19- (B19) specific IgM antibody, but not IgG antibody, was present in the patient's serum. B19 DNA was detected in the patient's serum by the polymerase chain reaction (PCR). Therefore, a definite diagnosis of transient aplastic crisis induced by B19 was made. On June 10, prior to the appearance of this case, a female nurse aged 27 years working in our hemodialysis center, complained of cough, fever and arthralgia. Another female nurse, aged 35 years, developed similar symptoms on July 3. Both nurses had a positive IgM titer against B19, but were negative for IgG, indicating an acute B19 infection. These findings led us to suspect that this series of B19 infection was spread by nosocomial transmission. Although some cases of B19 infection have been reported to occur in laboratory staffs, the B19 nosocomial infection has not been described in the literature. We also suggest that a transient aplastic crisis due to B19 infection could lead to severe anemia in cases of CRF whose erythropoiesis is maintained by a recombinant human erythropoietin.

Structure of a loop-deleted variant of 3-isopropylmalate dehydrogenase from Thermus thermophilus: an internal reprieve tolerance mechanism.

A loop-deleted mutant form of 3-isopropylmalate dehydrogenase from Thermus thermophilus was constructed to investigate the relationship between the flexibility of the structure and the thermostability of the enzyme. The structure of the mutant enzyme was determined by X-ray crystallography and was found to be almost the same as that of the native enzyme with a reduced temperature factor. Although the mutant protein had lost the flexible loop, its function and thermostability had remained unchanged. This phenomenon can be explained by an internal reprieve tolerance mechanism.

Crystallization and preliminary X-ray studies of isocitrate dehydrogenase from Thermus thermophilus HB8.

Isocitrate dehydrogenase from the thermophilic bacterium, Thermus thermophilus HB8, was crystallized by the vapor diffusion hanging drop method with polyethylene glycol 6000 as the precipitant. Pillar-like crystals of about 0.6 x 0.6 x 0.3 mm3 were obtained. Analysis of a series of oscillation photographs indicated that the orthorhombic crystals belonged to the I222 or I2(1)2(1)2(1) space group with unit cell dimensions of a = 100.1 A, b = 150.4 A, and c = 87.4 A. Intensity data were collected up to 2.5 A resolution.

Polyamine inhibition of gastric ulceration and secretion in rats.

The effect of polyamines on gastric ulceration and secretion in rats was studied. Stress-induced gastric ulceration and ulceration in pylorus-ligated rats were inhibited by subcutaneous or oral administration of spermine; spermidine's inhibitory effect was somewhat less. Histamine-induced ulceration was also inhibited by the subcutaneous injection of spermine. In addition, the daily oral administration of spermine for 10 days was therapeutic against an acetic acid-induced ulcer (chronic ulcer). Gastric secretion in pylorus-ligated rats and in rats with fistulae and stimulated by histamine injection was decreased by the subcutaneous injection of spermine.

Hippocampal kindling and effects of antiepileptic drugs.

The kindling effect of the hippocampus was investigated in rats. The development of behavioral manifestations in hippocampal kindling was similar to that seen in the case of amygdaloid kindling, but many more stimulations were required to evoke behavioral convulsions in the former. The transfer phenomenon from amygdala to hippocampus was evident. There were no differences in effects of drugs on the seizure-discharge and the behavioral convulsions between the amygdaloid and the hippocampal kindled rats. The present results suggest that the amygdala plays an important role in the formation of hippocampal kindling, especially in the manifestation of behavioral changes. The hippocampal kindled rat, as well as amygdaloid kindled rat, is useful animal model for evaluating the antiepileptic effect of various drugs.
