[Movement therapy. With special reference to functional disorders of the locomotor system].

Physical therapy by movements is of importance in several medical fields, in particular in rehabilitation. A great variety of possibilities for treatment by exercises and manual therapy exists. In a first part we outline a useful and problem-oriented nomenclature derived from actual applications of physical therapy by movements based on various concepts. The bases and applications of functional therapy by movements, by training or manipulative procedures as well as other therapeutic means are presented. The therapeutic climate and the relation between doctor and physiotherapist are briefly explored. In a second part we present the most important traditional concepts of treatment by movements in a more detailed way.

Exercise Therapy

[Defecography and manometry in the postoperative follow-up of patients treated with colo-anal anastomosis by the Parks technique for neoplasm stenosis of the rectum].

The Authors report 15 cases of patients with adenocarcinoma of the rectum who were treated using Park's resection. Morphofunctional parameters are evaluated 3 months after surgery using defecography and manometry before the closure of the neostoma and the reactivation of the ano-rectal canal. Opaque defecography enables the morphology and dimensions of the anal canal to be examined together with modifications to the area between the rectum and the anal canal, and the impression of the pubo-rectal sling. In conclusion these data confirm the importance of surgery, whereas the correlation between defecographic and manometric results allows the functional recovery "ad integrum" of the area to be checked.

Adenocarcinoma

[National Research Program. Part B: Chronicity of backache].

The main goal of part B of the National Research Program No 26 is to investigate the process leading to chronic low back pain. Starting from epidemiological facts the main risk factors are described from a systemic viewpoint. The rapid increase of disabling low back pain in the past decades makes it clear that factors outside the spine have to be made responsible for this process. There are changes in life-style and interpersonal relations, as well as in society and the health-care system. From these factors means of prevention are derived. Besides psychological factors general fitness and the training condition of the back muscles play an important role in effective prevention.

Back Pain

[Physical therapy of patients in general practice. Role of the main groups of physical therapy methods in acute and chronic problems of the locomotor system].

The choice of a treatment concept in physical therapy largely depends on whether the presenting functional disorder is of an acute or of a chronic nature. The classification acute or chronic has to be made according to two criteria: acute or chronic course in time/acute or chronic type of disorder. The main concern of physical therapy is prophylaxis of chronic disorders. Chronification of symptoms quite often leads to human tragedy and, moreover, to higher costs, since the total medical and social costs for the chronic cases are higher than those for acute cases. The general practitioner has an important function in the prophylaxis of chronic musculoskeletal disorders. Three to four weeks after the onset of the symptoms he should perform a special evaluation of every patient, thereby assessing the risk of chronification. When a chronic course is considered to be probable, an activating treatment concept should be initiated which in general comprises the following components: comprehensive information and instruction, active exercising (especially training therapy), self mobilization and soonest possible return to work within the limits of stress the condition can be exposed to.

Acute Disease

[Various radiological aspects of spontaneous biliary-digestive fistulas].

A number of significant radiological pictures of spontaneous biliodigestive fistula are presented. The aetiopathogenesis, pathological anatomy and clinical medicine of the condition are reviewed and the techniques available for the radiological investigation of biliodigestive short-circuits considered; the most frequent radiological signs and corresponding pictures are discussed and the complications and differential diagnosis with duodeno-biliar reflow examined.

The Leclercq test in diagnosis of tear in the rotator cuff.

The authors report their experience in using the Leclercq test to diagnose tear in the rotator cuff of the shoulder. When positive, this test consists in a radiographic demonstration of reduction in the subacromial space in opposing abduction of the shoulder. Twenty-five patients affected with chronic painful shoulder and suspected lesion of the rotator cuff were submitted to the Leclercq test and subsequently to arthrography in order to evaluate the conditions of the cuff. In all of the cases where the cuff was whole, and arthrography normal, the Leclercq test was negative. When lesion of the cuff was revealed by arthrography, the test was positive in 75% of the cases considered globally, and in 92% of the cases with complete tear in the rotator cuff.
