[The acoustic distortion products application in noise pathology].

We study a total of 40 individuals exposed to noise in 3 forms of presentation: continuous (metal-workers), impact (hunters and policemen) and acute (bombs or fireworks). All them underwent a study with otoscopy, pure-tone audiometry, speech audiometry, tympanometry and recording of DP 2f1-f2 in the DPgram form. The results showed an early damage in DP (3 to 6 kHz). That are demonstrated by a decrease in recording in direct relation with increased audiometric threshold. Being obvious at 6 kHz among subjects exposed to continuous noise, though the smaller extent data are expressed in the records of 3 and 4 kHz, independently of noise form of exposure.

[The usefulness of provoked acoustic otoemissions in the study of olivo-cochlear reflex].

The paper reports the experimental task done by the AA, in order to probe objectively the being of a feed-back circuit of cochlear interconnexion. This loop is basically represented by the medial efferent olivo-cochlear system. With that purpose they have analyzed the variations produced in the provoked acoustic otoemissions by clicks registered in one ear when the contralateral ear is stimulated with a white noise of 80 dB HL. The results shows that after stimulation of the contralateral cochlea with a white noise a decrease of the global amplitude of the PAOE is observed and also a decrease of the several amplitudes composing the frequency spectrum, but retaining the same morphological pattern.

[Eosinophils in nasal secretion. Technique and clinical utility].

The microscopic study of nasal secretions to identify eosinophils is a classic test in the diagnosis of allergic rhinitis. Modern immunologic diagnostic alternatives have reduced the frequency of its practice, but the information that it provides, although complementary, is quick, inexpensive and specific. Panoptic stains easily detect eosinophils, although the absence of these cells does not rule out any diagnosis.

[Monitoring drug ototoxicity with distortion products].

Distortion products are currently considered an objective method for rapidly evaluating the functional status of the cochlea, which is interesting for the monitoring of drug ototoxicity. We studied 40 ears from patients who had received an ototoxic drug, tobramycin. Two consecutive measurements were made using distortion products 2f1-f2, before and after taking the ototoxic agent. A significant reduction was observed in the amplitudes of the DP-grams. At the same time, we evaluated the influence of previous hearing loss on the DP-grams and divided the patients into two groups: normal hearing and sensorineural hearing loss. Patients were evaluated at the same time by conventional audiometry to compare the effect of tobramycin on both tests. To conclude, tobramycin had little effect on audiometric thresholds, but produced an evident change in the amplitude of the distortion products.

[Clinical study on the ototoxicity of cisplatin with distortion products].

Distortion products are an objective method of hearing assessment that provides a rapid way to evaluate the functional status of the cochlea, so they are interesting for monitoring drug ototoxicity. We studied 50 ears from patients who received an ototoxic drug, cisplatin. Two consecutive measurements were made using distortion products 2f1-f2, before and after taking the ototoxic agent. We observed a significant reduction in the amplitudes of the DP-grams. At the same time, we evaluated the influence of previous sensorineural hearing loss on DP-grams, dividing the patients into two groups: normal hearing and sensorineural hearing loss. All these patients were evaluated at the same time by conventional audiometry in order to compare the effect of cisplatin on both tests. We conclude that cisplatin produced little effect on audiometric thresholds and an evident effect on the amplitude of distortion products.

[Auditory neuropathies in infants].

Auditory neuropathy is a sensorineural disorder characterized by absent or abnormal auditory evoked potentials, and normal cochlear outer hair cell function. It is believed that a variety of processes are involved in its pathophysiology and their influence on hearing can differ. We describe the diagnostic sequence and management of two new cases of auditory neuropathy in infants. The first case was a girl with no risk factors for hearing loss. Her absence of response to sounds was compared with her twin's reactions. Otoacoustic emissions were present but no auditory evoked potential response was detected. Her evolution was characteristic of deep sensorineural hearing loss without a hearing aid. At present she is awaiting a cochlear implant. The second case was a boy who underwent hearing loss screening for hyperbilirubinemia. Examinations were repeated three months later because the mother suspected hearing loss. Objective tests showed a pattern of auditory neuropathy. The boy's evolution was different: in spite of the presence of destructured auditory evoked potentials; it was evident that the boy conserved some hearing and had thresholds suggestive of moderate hearing loss.

[Epithelioma of the pyriform sinus: retrospective study and survival analysis after 5 years].

We have studied 27 clinical records of patients with piriform sinus epidermoid carcinoma diagnosed and treated in our department during a period of 6 years. All the patients underwent surgery and radiotherapy. We have analyzed the survival and recurrence rates together with other parameters that may have some influence in them. We have tried to show the factors that might be in relation with the prognosis of pyriform sinus carcinoma. The statistical analysis show a survival rate of 29.6% after 5 years follow up, being the node invasion, wither clinical or histological, the main fact to have a prognostic value.

[Influence of the contralateral acoustic stimulation on the acoustic otoemissions: response modulation by the efferent olivocochlear system].

We have correlated in 16 patients (32 ears), the modifications de EOEs before and after sound stimulation of the contralateral ear. The recording of the EOEs has been performed in a sound proof room with ILO 88 from Otodynamics Ltd., the analysis has been made with the ILO 88 V.3.5 software. We have tried to study the modulator rol of the medial efferent coclear system on the external ciliated cells. In the results we observe a decrease of the amplitude in the EOEs when the contralateralmears is stimulated a white noise, in relation with the nonstimulation situation. When pures tones are used contralateral stimulation the decrease in the EOEs amplitude takes place in the same frequency spectrum as the pure tone used.

[Myringoplasty using formaldehyde formed fascia (Perkins' foot) in 188 cases: anatomical and functional results at 5 years].

Herein we explain the experiences we have had, in the ORL department of the HCU of Valencia, al along a period of seven years, by using the formaldehyde formed fascia graft, exposing the anatomical and functional results obtained after a minimum pursuit of five years on a sequence of 188 myringoplasties. With such method we got to close the tympanic membrane, and that they remain entire after five years, on the 72.87% of the cases, getting improvement of audition determined by a reduction of the air-bone gap greater than 15 dB, in the 62.77% of the cases. Such results are obtained with a very slight number of complications. This all makes us to consider this technique as the proper one in cases where perforation is greater than 50% of tympanic membrane.

[Usefulness of preoperative coagulation tests in the prevention of post-tonsillectomy hemorrhage in children].

A retrospective study was made of 266 children undergoing tonsillectomy to determine the usefulness of preoperative coagulation tests for predictin-haemorrhagic complications. A short questionnaire about haemorrhagic history identified 11 children with a positive background but no complications. Eleven children, 4.13%, had post-tonsillectomy bleeding that required a new admission or lengthened the hospital stay only one of which coagulation screening disorders. The determination of coagulation tests in every child had allowed us to diagnose 2 non-clinical cases of Von Willebrand's disease and 4 of lupus anticoagulant but did not identify high risk patients for bleeding, in spite of their medical history. Indiscriminate use of these tests brings no benefit to the patient because they don't give surgeon new information and they increase costs and delay surgery.

[The study of otoacoustic emissions in diabetes mellitus].

Cochlear activity was evaluated by means of otoacoustic emission in patients with normal hearing and insulin-dependent diabetes mellitus. Twenty diabetic subjects were studied by recording click evoked otoacoustic emissions (TEOAE) and distortion products otoacoustic emissions (DPOAE) 2fl-f2 and comparing the results with those obtained in a control group of non-diabetic with normal hearing. The results obtained in the diabetic group showed a statistically significant decrease in mean amplitude, in the amplitude of every click evoked otoacoustic emissions and in the mean amplitudes of various distortion products compared with the control group. These findings suggest the presence of cochlear disorders in diabetic patients, probably produced by impaired functional properties of the outer hair cells.

[Value of auditory evoked potentials of the cerebral brainstem in the diagnosis of retrocochlear tumors].

The use of auditory brainstem response (ABR) in the diagnosis of retrocochlear tumoral pathology has been established for years, but the development of magnetic resonance imaging for the same purpose calls for a review of the value of this method. We studied 200 ABR records made to exclude pathology. The results indicate that ABR is useful as a screening method in this type of pathology when carried out in a population with any auditory symptom, although it was not very suggestive. In most positive tests, the presence of pathology is not confirmed.
