How we do it: coblation tonsillectomy complication rates from a single ENT department compared with the National Prospective Tonsillectomy Audit.

Coblation tonsillectomy is a relatively new technique, the results of which need auditing within practising units, to justify its continued usage. * The National Prospective Tonsillectomy Audit provides an excellent source of data for individual units to compare their results to. * This retrospective audit of 391 coblation tonsillectomies shows that our units haemorrhage and return to theatre rates are similar to the National rates for cold steel & ties data. * Resolution of training issues and patient selection may lead to increased use of this technique.

The surgical arrest of post-tonsillectomy haemorrhage: hospital episode statistics.

INTRODUCTION: In the consent of patients for tonsillectomy, the risk of returning to theatre for control of postoperative bleeding may be determined from data provided on Hospital Episode Statistics (HES) provided by the Department of Health website. METHODS: HES data for England from 1998-2002 were used. The numbers of tonsillectomies and of surgical arrest of post-tonsillectomy haemorrhage were considered for children and adults. RESULTS: Of 220,497 tonsillectomies performed over the 4-year period, 1804 (0.82%) returned to theatre for control of bleeding. However, this was 3.87 times more likely in adults than children (P <0.0001). CONCLUSION: Adults are nearly four times more likely than children to require surgery for the arrest of post-tonsillectomy haemorrhage.

A rare case of Crohn's disease in head and neck surgery.

Oral manifestations of Crohn's disease may present to the Otolaryngologist in the form of ulceration, glossitis and odynophagia, but rarer examples of presentation are also recognized. In this case a 24-year-old lady presented with cervical lymphadenopathy, the subsequent investigation of which resulted in the identification of the disease both in this node and in the tonsils. It is noted that these lesions may precede the classical intestinal manifestations and so the Otolaryngologist could aid in the initial diagnosis of the disease. The pathogenic possibilities of how Crohn's disease might have arisen in these and other extra-intestinal regions, are discussed.

Drain placement after thyroid surgery: the bra-strap line.

In cases of thyroid surgery that require a drain post-operatively, the authors suggest the subcutaneous tunnelling of the drain to exit in the bra-strap line. This simple but effective technique hides the potentially unsightly resultant scars that may be of concern to women wearing clothes that expose the upper chest and lower neck. The bra-strap line can be easily found in the patient and the authors have found no additional complications with exiting the drain at this site.
