[Use of ventriculojugulostomy and lumbosaphenostomy for the treatment of hydrocephalus of various etiology].

Analysed were the results of treatment of 85 patients with hydrocephaly of different genesis: 12 (14.1%) patients--occlusive, 73 (85.9%)--communicating hydrocephaly, including 31 (36.4%)--with normal cerebrospinal fluid pressure. Ventriculojugulostomy was performed in 55 patients, lumbosaphenostomy--in 30. Stabilization of hydrocephaly was achieved in 89.4% of the patients at the near postoperative period, in 81.5%--at the period of up to 17 years after the operation. Shunt revision was performed in 19 (22.3%) patients.

[The hemostatic system in traumatic subarachnoid hemorrhage in the Far North].

It was found that in conditions of the extreme North sanation of the cerebrospinal fluid occurs later in patients with traumatic subarachnoidal hemorrhage than in the central regions of the country. In connection with this the hemostasis system was studied in 20 patients with traumatic subarachnoidal hemorrhage and 10 patients with brain tumours (control group). Patients with traumatic subarachnoidal hemorrhage showed significant hypocoagulation changes.

[The clinical picture, diagnosis and treatment of chronic subdural hematomas].

A study is presented of 37 cases of chronic subdural hematomas. Initial manifestations: headache, epileptic seizures, mental disorders. Involvement of the III, V, VII cranial nerves, anisocoria almost in 50% on the contralateral side were noted. CSF xanthochromia is of certain diagnostic significance. Carotid angiography is the most informative method. Osteoplastic trephination with evacuation of the hematoma is the method of choice.

[Method for treating hydrocephalus by ventriculo-jugular shunting].

Pressure sustained by the venous valves was studied by means of a self-recording device; 47 valves of the great saphenous veins of the thigh were tests. A total of 51 valves taken from 10 cadavers were studied. The instruments for and the method of ventriculojugulostomy are described. It consists in diverting the CSF from the lateral ventricle of the brain into the internal jugular vein by means of ventricular silicone rubber catheter and a venous auto- or homograft. Despite the severe contingent of patients subjected to surgery, analysis of 12 operations allows the suggested method to be used for treating hydrocephalus both in adults and in children.

[The treatment of hydrocephalus by ventriculojugular shunting].

A total of 43 patients with hydrocephalus of various etiology were examined. Ventriculojugulostomy was performed in 20 patients, its modification in 23. Bypass surgery was made on the internal jugular vein in 9 patients, on the common facial vein in 9 and on the external jugular vein in 5. There was an improvement in 43 patients in the immediate postoperative period, which was followed by regression of global brain symptoms and cessation of hydrocephalus progression. Long-term results are known in 32 patients who were discharged having a well functioning drainage valvular system. The follow-ups lasted from 6 months to 17 years.
