Carcinoembryonic antigen (CEA) activity was assayed in plasma and in pure pancreatic juice from eight patients with carcinoma of the pancreas, 28 patients with pancreatitis, and 13 controls with no demonstrable pancreatic disease. Juice specimens were obtained via direct transduodenal cannulation of the pancreatic duct. The mean pancreatic juice CEA activities in controls, pancreatitis, and pancreatic carcinoma were 8.1 ng/ml, 18.6 ng/ml, and 309 ng/ml, respectively. Pancreatic juice CEA activity in patients with cancer of that organ was significantly higher than in those with pancreatitis or in controls. None of the 32 subjects with both pancreatic juice CEA activity less than 30 ng/ml and plasma CEA less than 2.5 ng/ml had pancreatic cancer. Three of the four patients with CEA elevations in both fluids above these levels harbored this malignancy. These findings suggest that combined measurement of CEA activity in plasma and pancreatic juice may help in diagnosing pancreatic disease.

A morphologically novel bacterium which ferments cellulose and produces butyric acid was isolated from a bovine rumen. It grows in long twisted chains that often form a double-helical pattern.

The present study was undertaken to define the role of ultrasonography (US) in screening and diagnosis of hepatic venous outflow tract obstruction. Forty-five consecutive patients clinically suspected to have hepatic venous outflow tract obstruction were included in the study for screening by US and for assessment of patency or block in the hepatic vein (HV) and/or inferior vena cava (IVC). Four patients were excluded from the study. Eleven patients had a diagnosis other than hepatic venous outflow tract obstruction and all these patients were found to have patent HV and IVC. Thirty patients were finally diagnosed to have hepatic venous outflow tract obstruction. Using US, as a screening test 27 (90%) out of 30 such cases were correctly identified as cases of hepatic venous outflow tract obstruction and in these cases the site of block in hepatic venous outflow tract (major HV and/or IVC) was correctly diagnosed in 90% of the cases. Our results indicate that US is a sensitive and accurate test and should be the initial investigation for screening and identifying the site of obstruction in patients with hepatic venous outflow tract obstruction.
A prospective double blind and randomized study was conducted to compare 4-aminosalicylic acid (4-ASA) and prednisolone-21-phosphate enemas in inducing remission in patients with acute distal ulcerative colitis. Patients with ulcerative colitis distal to the splenic flexure as assessed by flexible colonoscopy, barium enema and histology were included in the study. Of 40 consecutive patients, 20 were randomized to each of the two treatment groups. Clinical evaluation was done weekly; sigmoidoscopy and histology were performed at entry and at the end of 4 weeks. Therapy was discontinued in four patients treated with prednisolone enemas due to worsening of symptoms. The clinical improvement was significant in the remaining patients (P less than 0.001) and was similar in the two groups (P greater than 0.1). Sigmoidoscopic and histological improvement were better with 4-ASA than with prednisolone enemas. No adverse effects were observed in any of the patients treated. The present study suggests that 4-ASA is a safe and effective treatment for inducing remission in acute distal ulcerative colitis.
Alterations in the plasma levels of tissue plasminogen activator (t-PA) and plasminogen activator inhibitor (PAI-1) as observed in various liver disorders are attributed to deranged liver function. Status of these parameters in patients with Budd-Chiari syndrome (BCS) has not been reported so far. In the present study 27 patients with BCS with hepatic and/or inferior vena cava occlusion were investigated for any defect in the fibrinolytic system. Mild to moderately deranged liver function tests were observed in all (except 2) patients. Three patients each showed mildy increased levels of t-PA (3/27) and PAI-1 (3/27). The levels of t-PA and PAI-1 however did not show any correlation with liver function parameters.

BACKGROUND: Autonomic dysfunction has been associated with duodenal ulcer. We assessed autonomic reactivity in patients with duodenal ulcer. METHODS: Ten patients with non-bleeding active duodenal ulcers and ten age- and sex-matched healthy subjects were investigated for parasympathetic reactivity (heart rate response to deep breathing, Valsalva maneuver and head-up tilt test) and sympathetic reactivity (blood pressure response to hand grip, head-up tilt and cold pressor test). Anxiety status was measured by evaluating responses to a questionnaire. RESULTS: The duodenal ulcer patients showed normal parasympathetic reactivity, lowered sympathetic reactivity and high anxiety score. When compared to control subjects, they had significantly lower diastolic blood pressure change in response to hand grip (median [range]; difference in values 12 [4-16] mmHg vs 16 [10-22] mmHg) and head up tilt (1 [-6-4] vs 6 [2-10] mmHg). CONCLUSIONS: Patients with duodenal ulcer have lowered sympathetic reactivity; this may be involved in causation by decreasing mucosal protection.
