[The active detection of tumors of the large intestine by using automated screening].

A system for automated screening for early detection of proctologic pathologies is described. Questionnaire-based data, formation of high risk groups using a computer and results of examination of 1660 office and professional workers of an industrial enterprise according to a given program are discussed. After filled questionnaires had been processed using a computer, 134 (8%) subjects were selected to group 1, 454 (27%) subjects-to group 2, 383 (23%) subjects-group 3 and 689 (42%) subjects-to group 4. Proctologic pathologies were detected in all persons of group 1, 83% of those in group 2 and in 30% cases of group 3. Asymptomatic proctologic pathology was established in 630 (37.8%) screenees of whom 10 revealed large bowel tumors, 5 early cancers included, whereas 49-large bowel polyps.

[Hemosorption in the complex treatment of rheumatic diseases].

The authors describe the results of the use of hemosorption in combined treatment of 42 patients with rheumatic diseases. The main group comprised patients with thromboangitis obliterans (23), nonspecific aortoarteritis was recognized in 6 cases, systemic scleroderma and systemic lupus erythematosus in 4, nodular periarteritis in 2. The functional skin tests were made with the aid of the KDU-3 unit, the level of the peripheral blood flow was measured with tetrapolar rheography and impedometry. The degree of redox processes and oxygen tension in the tissues was studied by the polarographic technique, the character of microcirculatory processes with the aid of biomicroscopy of the eyeball conjunctiva, the intensity of immunologic shifts by means of precipitation with polyethylene glycol at different dilutions. In addition, alterations in cellular and serum enzymes were also examined. The data obtained suggest that hemosorption is one of the effective additional methods of the treatment of patients with rheumatic diseases.

[Organization of the active detection of pretumorous and tumorous diseases of the stomach using primary automated screening and fiber gastroscopy].

The report discusses a computerized system of early detection of gastrointestinal diseases and risk group formation using special formalized questionnaires to be filled by industrial enterprise personnel. Endoscopic examination of 1,240 followed-up patients detected stomach cancer in 32 cases, gastric and duodenal ulcer--195, gastric polyps--37 and various forms of gastritis--in 766 cases. As a result of a computerized processing of the questionnaire-based data, 1,084 subjects were referred to group I. An endoscopic examination of this group carried out at a plant's medical station identified stomach cancer in 13 cases, gastric and duodenal ulcer--110, gastric polyp--46 and various forms of gastritis--in 803 cases.

[Ways of improving the therapeutic management of angiosurgical patients in a specialized vascular surgery department].

Many years of experience of the Yaroslavl Interregional Vascular Centre suggests that better therapeutic support at vascular surgery units may be one of approaches to improving current specialized angiosurgical care. Collaboration of angiosurgeons and therapeutists, and drug correction of possible disorders and complications before and after vascular reconstructive surgery is expected to contribute to better outcomes of treatment in patients with acute and chronic pathologic developments in arteries and veins.

[Errors, hazards and complications in treating acute obstruction of the aorta and major arteries of the extremities].

Errors made at various stages of the treatment of patients with acute thrombosis and embolism of the aorta and major arteries of the extremities were determined from the analysis of 476 clinical observations. Insufficient orientation of physicians in the problems of acute pathologic conditions of the vessels was the main cause of the errors at the prehospital stage. At the stage of specialized treatment, assessment of the degree of ischemia of the extremity, the severity of the patient's condition, and the viability of the extremity proved most difficult.
