Programmed versus spontaneous and stimulated labour.

The present study indicates advantages of programmed labour compared with labour which started spontaneously on or before term day. Programmed labour was shorter and the condition of the newborn, assessed according to Apgar and acid-base values of cord-blood, was better than in spontaneous or stimulated labour groups. There was no appreciable difference in the frequency of other complications such as prematurity, hyperbilirubinemia, and neuropathological symptoms, among groups studied.

Largactil (chlorpromasine) and cervical dilatation in primiparae.

The effects of Largactil on cervical maturation and dilatation during induced labour in primiparous women were investigated. The obtained results suggest that Largactil has significant influence on the maturation and dilatation of the cervix in severely "neurotic" women relative to "non-neurotic" women.

The effects of lavour time on neonatal depression.

The effects of labour time on neonatal depression were investigated in 1072 cases. It was found that the everage Apgar score assessed at one minute post-parturition decreased significantly as a function of duration of labour. Additionally, the incidence of neonatal depression (Apgar indes of 7 or less) was found to increase significantly with increase significantly with increases in labour duration. Apgar indices taken five minutes after birth did not reveal any significant effects related to duration of labour. Further analysis of the labour conditions indicated that the effects of labour time on the condition of the infant was greater in spontaneius labours than induced or stimulated labours. In spotanious labours, the five-minute Apgar score continued to be influenced by duration of labour. After five minutes individual differences were clearly observable in all labours.

[Analysis of 290 breech presentations].

By analysing all breech presentations handled in the last three years (290 cases), the authors have found that perinatal mortality in such cases was much higher (8%) than the general perinatal mortality (1.8%) in the last years and that this increase mainly related to the perinatal mortality of children weighing less than 2500 g, owing to complications and diseases typical of that kind of children. Out of the children delivered with breech presentation, 40% were either premature babies (22%) or small-for-date babies (18%). The condition of children delivered with breech presentation is 5-6 times worse than in premature babies delivered with full or partial extraction. The authors could not establish if the condition of children with cesarean section was better. The results were obtained when the delivery was induced by using the maniotomy and Syntocinon. The authors point out that the problem of breech presentation lies in prematurity, that an excessive application of cesarean section does not solve the problem, that extractions should be avoided, and that all such deliveries should be managed on the principle of active management of labour.

[Breech presentation delivery in Slovenia].

A computer-supported analysis of 722 breech presentations was performed, and the results were compared to those of 18,533 vertex presentations. This covered all labors performed in 10 Slovene obstetric hospitals in 1986. It was found that the greatest risk existed in the group of preterm breech deliveries. This group had the highest percentage of perinatal mortality (256%), unfavourable infant postpartum condition (1 min Apgar 1-3 24.8%; 5 min Apgar 1-3 11.4%), malformations (14.8%), intracranial bleeding (4.7%), respiratory distress (11.4%), and infections (2.7%). In term breech deliveries the number of malformations was also higher than in term vertex deliveries (3.9%: 1.1%). The cesarean section was used in 30% of breech presentations (8% in vertex presentation). The postpartum condition of preterm infants was slightly better when they had been delivered with a cesarean section (1 min Apgar 1-3 42.2%; 5 min Apgar 1-3 0%) than if delivered vaginally (1 min Apgar 1-3 25.0%; 5 min Apgar 1-3 14.6%), but this was not the case with infants born at term. Here the postpartum condition is better in vaginal deliveries. Surprisingly, the fetal monitoring was the least satisfactory in the group at the greatest risk.

[Simultaneous interstitial and intrauterine pregnancy: successful completion of the intrauterine pregnancy].

The authors describe an extremely rare combination of simultaneous intrauterine and interstitial pregnancy. In spite of the rupture of interstitial pregnancy in the 13th week and cornual resection, it was possible to restrain the intrauterine pregnancy under careful control until the 38th week, when a healthy mature baby was delivered by elective Caesarian section. The cicatrix of the uterus after cornual resection held the pregnancy well. Severe anaemia as a consequence of the bleeding before and during operation was quickly corrected and did not harm the further development of the child.
