In vitro synthesis of pig pancreas ribonuclease.

From studies on the in vitro synthesis of the heavily glycosylated pig pancreas ribonuclease (molecular weight of the protein moiety is 13 786, on the basis of the amino acid composition), the following points emerge: (1) the enzyme is synthesized as a precursor having an apparent molecular weight about 7000 higher than that of the mature non-glycosylated protein; (2) the mRNA coding for the enzyme protein consists of about 950 nucleotides.

Comparison of propafenone and disopyramide for treatment of chronic ventricular arrhythmias: placebo-controlled, double-blind, randomized crossover study.

In a double blind, placebo-controlled study, the efficacy of propafenone, a new antiarrhythmic drug was compared to that of disopyramide. Sixteen patients with frequent and complex premature ventricular contractions (PVCs) were studied by serial 24-hour ambulatory monitoring, while they were receiving propafenone, 300 mg, and disopyramide, 200 mg, both every 8 hours. A reduction in the mean frequency of PVCs per hour, in comparison to the placebo period, from 574 +/- 535 to 100 +/- 130, was observed after propafenone (p less than 0.005) and from 629 +/- 455 to 231 +/- 280 after disopyramide (p less than 0.008). A greater than 70% reduction in PVCs in comparison to placebo was observed in 11 of 14 after propafenone and 9 of 15 after disopyramide (NS). A greater than or equal to 90% reduction in PVCs was observed in 9 of 16 with propafenone and in 4 of 15 with disopyramide (p less than 0.05). The suppression of complex PVCs (repetitive, polymorphic, or more than 5/min with bigeminism) was observed in 11 of 14 after propafenone and in 9 of 14 after disopyramide. The abolition of nonsustained ventricular tachycardia was observed in 6 of 6 and 3 of 5, respectively, after propafenone and disopyramide (p less than 0.05). A lower incidence of side effects, 4 of 16 vs 8 of 16, was observed during propafenone than during disopyramide treatment. We conclude that propafenone, in a dose of 900 mg daily, is more effective than disopyramide, in a dose of 600 mg daily, in the treatment of frequent and complex PVCs and nonsustained ventricular tachycardias. Propafenone also showed a lower incidence of side effects.

[Psychiatric problems and therapeutic measures in the office of the general practitioner].

The results of a survey of mental disorders identified from General Practitioners in their offices, in a quarter of Padua are presented. The findings emphasize the role of the General Practitioner in psychiatric care delivery (12% of the patients have been judged "psychiatric case" and these patients obtain treatment from General Practitioner in 70% of the cases). About half of "psychiatric patients" present a depressive illness. In 46% of the cases the General Practitioners have pointed out a close relationship between social difficulties and psychological problems. In 59% of the cases mental and somatic disorders are closely linked together. Implications of these findings in the prospect of a collaboration between psychiatric services and primary care then discussed.

Propafenone for refractory ventricular arrhythmias: correlation with drug plasma levels during long-term treatment.

The efficacy of propafenone, a new antiarrhythmic drug, was studied in 21 patients with ventricular arrhythmias refractory to previous antiarrhythmic medications. Group A included 10 patients with chronic ventricular premature complexes (VPCs), 6 of whom had nonsustained ventricular tachycardia (VT) and 4 of whom had recurrent, sustained VT; all received propafenone, 900 mg/day. Group B included 11 patients, all with chronic VPCs, 9 of whom had nonsustained VT and 5 of whom had sustained VT; all received propafenone, 450 mg/day. Drug efficacy was evaluated as a 70% or greater reduction in VPC frequency with complex VPC abolition in ambulatory monitoring and suppression of nonsustained VT and sustained VT during a follow-up period up to 154 +/- 58 days in group A and 96 +/- 42 days in group B. Drug plasma levels were measured during chronic therapy in pharmacologic steady state. In group A, propafenone reduced the frequency of chronic VPCs in 9 patients and abolished nonsustained VT in 4 of 6 and sustained VT in 3 of 4; in group B, propafenone reduced the frequency of chronic VPCs in 6 patients and abolished nonsustained VT in 6 of 9 and sustained VT in 3 of 5. Two patients with recurrences of sustained VT in this group were later successfully treated with propafenone, 900 mg/day; overall, 8 of 9 patients with recurrences of sustained VT were successfully treated with 900 mg/day.(ABSTRACT TRUNCATED AT 250 WORDS)

[Comparative study on the efficacy of disopyramide and propafenone in the prevention of the recurrence of ventricular tachycardia].

The purpose of our study was to assess the efficacy of Propafenon in comparison with Disopyramide in the long-term treatment of repetitive ventricular tachycardia. We studied 34 patients suffering from episodes of repetitive ventricular tachycardia of different etiology. Propafenon and Disopyramide was administered in doses of 300 mg and 200 mg three times a day respectively. The number of ventricular tachycardias has been evaluated during a period of 4 months of basal observation and during two periods of 4 months each of treatment respectively with Disopyramide and Propafenon. Moreover a dynamic electrocardiogram was recorded to assess the efficacy of Propafenon in suppressing ventricular ectopic beats in 17 patients. We conclude that Propafenon shows greater efficacy than Disopyramide in the treatment of repetitive ventricular tachycardias and a comparable efficacy in the control of ventricular ectopic beats. Moreover Propafenon shows also a minor incidence of side effects.

Bovine seminal ribonuclease precursor synthesized in vitro.

Native bovine seminal ribonuclease is a dimeric protein, whose identical subunits (Mr 14500), linked through two disulfide bridges, can be dissociated by a selective reduction procedure. Evidence is presented that the synthesis in vitro, under reducing conditions, of bovine seminal RNAase, directed by polyadenylated RNA isolated from bull seminal vesicles (where the enzyme is synthesized in vivo), occurs in the form of a precursor, 18000-Da polypeptide. The precursor nature of this translation product was deduced by two criteria: (1) its specific immunoprecipitation with anti-bovine seminal RNAase antibodies; (2) its processing by dog pancreas microsomal membranes to produce a protein with a molecular weight similar to that of the subunit(s) of bovine seminal RNAase. Moreover, evidence is offered that the precursor polypeptide is able to form in vitro a dimeric molecule under conditions where no exogenous reducing agents were added.

[Multiform ventricular tachycardias and Torsades de Pointe].

The term Torsade de Pointe (T.d.P.) was first introduced by Dessertenne (1966) to designate an unusual ventricular tachyarrhythmia characterized by paroxysms of V.T. at rates typically greater than 200 beats/m, in which the QRS morphology shows alternating polarity in a modulating pattern, so that the complexes appear to be twisting around the baseline. This arrhythmia, triggered by E.V.B. falling in the vulnerable period of a previous beat, occurs in the setting of a prolonged Q-T interval, is generally self-limiting and, occasionally, degenerates into V.F. Similar morphologic features may be observed in patients with a normal Q-T interval. The duration of Q-T interval has important therapeutic implications. The arrhythmia occurring in the setting of a prolonged Q-T requires strict avoidance of all drugs that may potentially further delay repolarization (including class I antiarrhythmic agents), whereas that occurring with a normal Q-T interval usually responds to conventional therapy (including administration of class I antiarrhythmic agents). Thus, it seems reasonable to reserve the term T.d.P. exclusively to the forms with prolonged Q-T interval and to define those with normal Q-T interval with the name of multiform (or polimorphous) ventricular tachycardia (M.V.T.). In this paper the Authors discuss the most important electrocardiographic, clinical, aetiological, electrogenetic features of both the T.d.P. and M.V.T., on the basis of their own experience.

[Verapamil in effort angina: a multi-centre study].

We have performed a multi-centre study with 47 outpatients in order to evaluate the efficacy of Verapamil (V) in the treatment of stable effort angina, and to compare the effect of two different doses of the drug (240 and 360 mg/die). The protocol consisted of a first period of Placebo, followed by the double-blind randomized cross-over administration of Placebo (P) and Verapamil (V) in doses of 240 and 360 mg/die. The symptomatology, the consumption of TNG, the ECG pattern at rest and during exercise, the maximum exercise tolerance during exercise and the rate of recovery were evaluated at the end of each 1 month period. V. provided a significant reduction of the number of angina attacks and of the consumption of TNG pills with improvement of symptomatology. The maximum exercise performance improved without changes in maximum rate pressure double product. A decrease of double product was observed at rest and during the recovery period. The higher dose of V. (360 mg/die) provides a better improvement in the number of angina attacks, in the symptomatology, in the double product at rest, and in the rate of recovery than the lower dose (240 mg/die). Thus these data indicate that V. provides anti-anginal efficacy by reducing myocardial oxygen demand, and increases exercise tolerance in effort angina patients.

Detection of invasiveness of mammalian cells by Escherichia coli: collaborative study.

In a collaborative study of 2 standard procedures (Procedure I, presented in J. Assoc. Off. Anal. Chem. 60, 546-562 (1977) and Procedure II in the Bacteriological Analytical Manual, 1978), 7 laboratories evaluated the invasive potential of Escherichia coli. Monolayers of HeLa cultures were infected with E. coli (infectivity ratio 100 bacteria/mammalian cell) suspended in 2 menstrua: heat-inactivated fetal bovine serum (Procedure I) and 0.2% bovine albumin Fraction V dissolved in Earle's buffered salts (Procedure II). After uptake of the bacteria, intracellular growth, and differential staining, the percentages of HeLa cells containing a minimum of 5 bacteria were determined microscopically. All laboratories correctly identified invasiveness of E. coli by Procedure I; 5 of 6 laboratories recognized invasiveness by Procedure II. Although Procedure I was more reproducible than Procedure II, repeatability did not differ significantly. Procedure I was adopted official first action for presumptive recognition of invasiveness in E. coli.

Comparison of the efficacy of propafenone versus disopyramide for complex premature ventricular contractions. Evaluation with long-term monitoring.

Interpreting the results of this study, propafenone (Rytmonorm), administered orally in a dose of 900 mg daily, was more effective than disopyramide in a dose of 600 mg daily, in the treatment of patients with complex premature ventricular contractions (PVCs). The study was performed in 12 patients, with several Holter ECG monitorings to obtain quantitative data of the effectiveness in the same patient of the new drug propafenone in comparison with disopyramide in a documented effective oral regimen. A quantitative analysis of PVCs was obtained with a computer assisted detection. Clinical effectiveness, considered as 70--80% reduction of total number of PVCs, was obtained in 8/12 patients for propafenone and 6/12 for disopyramide, or as 50% reduction with suppression of all complex PVCs (couplets, repetitive, multiform and bigeminy) was obtained in 11/12 for propafenone and 9/12 for disopyramide.

[Dynamic electrocardiography evaluation of the effectiveness of prajmalium bitartrate, disopyramide and procainamide in patients with stabilized ventricular extrasystole].

20 patients with chronic premature ventricular contractions (P.V.Cs) underwent several Holter ECG monitorings to assess clinical effectiveness of three antiarrhythmic drugs: Prajmalium Bitartrate (P.B.) 80 mg/daily, Disopyramide (D.) 600 mg/daily and Procainamide (P.) 2400 mg/daily, to assess the most effective antiarrhythmic medication in every patient. Clinical effectiveness was considered as 80% reduction of P.V.Cs or 50% reduction with suppression of all complex ventricular ectopy (repetitive, polymorph, bigeminy). These results were observed respectively, for 80% reduction, in 5/20 patients for P.B., in 9/20 for D., and in 3/19 for P; and for 50% reduction in 11/20 for P.B., in 18/20 for D., and in 9/19 for P. Comparison in the same patient, using Holter ECG monitoring with a computer assisted analysis, of the effects of different antiarrhythmic medications, is a rational procedure to assess clinical efficacy of new antiarrhythmic drugs and to choose the most effective in each case.

Niemann-Pick disease type B: prenatal diagnosis and enzymatic and chemical studies on fetal brain and liver.

Patients with Niemann-Pick disease type A have a severe neurovisceral disease caused by a deficiency of lysosomal sphingomyelinase activity in all tissues examined. The patients with the type B form have signs and symptoms related to storage of sphingomyelin in the spleen, liver, and lungs, while neurologically they remain normal. They also have a severe deficiency of lysosomal sphingomyelinase activity in all tissues previously examined. Here the brain and liver of a fetus with Niemann-Pick disease type B are examined for enzymatic anc chemical changes. Despite careful analysis, no measurable lysosomal sphingomyelinase could be measured in either organ. Lipid changes were comparable to those observed in fetuses aborted with Niemann-Pick disease type A. The affected child in this family is now age 3 and remains neurologically normal but continues to show organ enlargement and lung infiltration of lipids. It appears that the lack of neurological involvement in type B patients cannot be due to an obvious presence of significant lysosomal sphingomyelinase activity in brain.
