Effects of interleukin-1, lipopolysaccharide, and streptococci on procoagulant activity of cultured human cardiac valve endothelial and stromal cells.

Fibrin is the primary constituent of the vegetation in infective endocarditis, and tissue factor expression is a major mechanism of coagulation activation on infected valves. To determine which cells may participate in coagulation activation in this setting, expression of procoagulant activity (PCA; shown to be tissue factor) was studied in cultured endothelial and stromal cells derived from human cardiac valves. Endothelial cells had negligible PCA (99 +/- 50 mU/10(5) cells, mean +/- 1 standard deviation) unless stimulated by lipopolysaccharide or interleukin-1, which increased PCA to 5,592 +/- 1,482 and 5,901 +/- 1,497 mU/10(5) cells, respectively, in 6 h. Incubation of cells with viable enterococci or viridans streptococci or with an enterococcal cell wall preparation did not induce PCA. Cultured valve stromal cells constitutively expressed high levels of PCA (14,276 +/- 8,738 mU/10(5) cells) which was not changed with exposure to interleukin-1. PCAs of stromal or stimulated endothelial cells from valves of both right and left sides of the heart were comparable. The results suggest that endothelial cells may contribute to fibrin deposition during infection if stimulated, but PCA is not directly induced by bacteria. Stromal cells could contribute PCA if exposed to blood in the course of valve injury.

Staphylococcus aureus induces tissue factor expression in cultured human cardiac valve endothelium.

In vitro infection of cultured human cardiac valve endothelium (HCVE) with Staphylococcus aureus was used as a model to study potential mechanisms of vegetation formation in infective endocarditis. S. aureus was observed to adhere to and be ingested by HCVE. Infection for 8 h resulted in increased levels of procoagulant activity in HCVE, shown to be tissue factor by a specific assay. Mean activity in infected HCVE was 662 +/- 149 (mU/10(5) HCVE +/- 1 SD) versus 221 +/- 78 in control HCVE; surface-expressed activity was 57 +/- 25 in infected monolayers and undetectable (less than or equal to 10) in controls. Bacteria alone had no activity. These results suggest that endothelium may have a functional role in the pathogenesis of S. aureus endocarditis and may provide one potential mechanism for activating coagulation to initiate vegetation formation on a colonized cardiac valve.

Chronic myeloid leukemia presenting as a gynecological emergency.

An unusual case of chronic myeloid leukemia presenting to the gynecological unit as a case of septic abortion is presented. Though history and clinical examination suggested the possibility of double pathology of septic abortion and chronic myeloid leukemia, histology of the products removed from the uterus showed it to be leukemic deposits. The diagnosis of leukemia was confirmed by marrow aspiration (trephine biopsy). Careful clinical examination and systematic investigation helped in diagnosing the pathology and to treat her appropriately.

Some aspects of breast cancer in Singapore--a review of 306 cases.

A retrospective study of 306 cases of carcinoma breast in Singaporean patients managed at the University Department of Surgery, Singapore General Hospital was made, based on all available case notes, pathology report forms, operation record books, Cancer Registry Notification Forms, and data obtained from the Registry of Births and Deaths. These cases had their initial histological diagnosis made between 1970 to 1978. Basic data like age distribution, sex distribution and racial distribution of our cases are presented. Aspects of breast cancer like stage at time of diagnosis and 5- and 10-year survival figures are also presented. The crude five-year survival for all 306 cases was 56% and the crude 10-year survival for 223 cases was 22%. Survival for small lesions (T1, or less than 2 cm) was 90% at five years and 68% at ten years, a rate comparable with most centres in the world.

Streptococcal necrotizing myositis: a rare entity. A report of two cases.

Deep muscle abscess or bacterial myositis is not an uncommon disease in the tropics. The commonest pathogen involved is Staphylococcus aureus. However, streptococcal myositis is rare. Only a few cases have been reported in literature. Furthermore, streptococcal myositis with extensive necrosis of muscles has not been reported. In two cases of streptococcal myositis with extensive muscle necrosis, both developed severe acute infection with septicaemia. Histologic observations revealed massive necrosis of muscle tissue with acute inflammatory infiltration. This condition, which we designate streptococcal necrotizing myositis should be treated as a separate disease entity. It requires not just incision and drainage, as in bacterial myositis, but radical excision of all the necrotic muscles in addition to appropriate antibiotics. Unless it is recognized, the treatment will be inadequate and un-necessarily prolonged. Intramuscular pressure may contribute to the pathogenesis of muscle necrosis and may stimulate compartmental syndrome.

Acute polyhydramnios and cord presentation--complication of chorioangioma of the placenta--a case report.

Chorioangioma of the placenta is an uncommon benign tumor of the placenta. Large tumors are rare and they often cause increased maternal morbidity and fetal morbidity and mortality. A patient with continuous abdominal pain at 29 weeks gestation was clinically diagnosed to have acute polyhydramnios. Multiple pregnancy was suspected because of a tense uterus much larger than the dates and three palpable "fetal poles". An ultrasound examination revealed the third pole to be a large chorioangioma. She went into premature labor and a cesarean section was performed because of a cord presentation. Both mother and infant had an uneventful recovery. The complications of chorioangioma is discussed.

Assessment of vascularity of head of femur by bone scan and histopathology.

A prospective study of 17 patients with fracture neck of femur was undertaken to study the vascularity of the femoral head by bone scan using Technetium-99m MDP, as well as to study the viability of the femoral head by histopathology, and therefore determine correlation, if any, between these two methods. Treatment was by primary endoprosthetic replacement (Moore's Arthroplasty). In 16 of the 17 patients the bone scan showed decreased vascularity of the head, but histopathological examination could not confirm non-viability of the head. It is likely that, since the majority of the patients were operated on within 4 days, there was insufficient time for histopathological evidence of ischemia to develop, prior to removal of the femoral heads.

Colposcopic diagnosis of ovarian carcinoma spreading to cervix: a case report.

Secondary carcinoma of the cervix is uncommon. A patient with a history of ovarian carcinoma had an abnormal cervical smear and colposcopy was performed. Colposcopy and directed biopsy led to the diagnosis of ovarian cancer which had spread to the cervix. The unusual colposcopic features which allowed a clinical diagnosis are correlated with histopathology and the findings discussed.

Histological studies of vascularised nerve graft and conventional nerve graft.

Histological studies of vascularised nerve graft versus conventional nerve graft were performed in eighteen rat femoral nerves using microsurgical technique. This experiment showed that there was no difference in the degree of vascularisation, reticulin framework collapse, rate and extent of axonal regeneration and remyelination between the two groups.

Paraquat poisoning - management and prognosis.

Paraquat poisoning has become a significant clinical problem since the early 1960s. Its high mortality has posed a major challenge to clinicians treating these patients. Two patients who survived and one who did not are reported. The management of these patients and the possible factors affecting their outcome are discussed.

Benign Chondroblastoma.

Eight cases of benign chondroblastoma seen between 1957 and 1980 were reviewed. The majority presented between the age of 10 and 25. There was no male preponderance. All except one lesion occurred in the epiphysis but biansagression across epiphyseal plate into the metaphysis was not uncommon. In only one case the lesion was entirely in the metaphysis of the middle phalanx of a finger. Radiologically, the lesion appeared cystic with mottled calcification and a peripheral sclerotic margin. Microscopically, all lesions were benign with three characteristic features: round a polygonal chondroblast, areas of cell necrosis with intra and extra-cellular calcification and giant cells in response to the dead cells. Thorough curettage and bone grafting resulted in complete resolution in all cases without recurrence. For the lesions of the femoral head, an approach through the fovea of the head after division of the ligamentum teres and dislocation of the hip anteriorly was preferred. Adequate curettage could be dome without the risk of avascular necrosis or femoral neck fracture otherwise encountered with the transcervical approach.

Modification of the anterior commissure technique of partial laryngectomy.

The anterior commissure technique of partial laryngectomy is useful for treating "horseshoe" lesions of the larynx. As originally described, a McNaught keel is used after the excision to maintain an open glottic chink and to prevent webbing. After three weeks, this keel is removed as a surgical procedure. During the past four years, we have treated 12 patients using 0.020-gauge Silastic sheeting to replace the McNaught keel. The Silastic sheet is anchored above and below the glottis with externally tied 2-0 proline stutures. Two to three weeks later, a direct laryngoscopy is performed and the stent is removed. This modification avoids a second surgical procedure, decreases the hospital stay, and allows for inspection of the glottis.

Intake of selected nutrients compared with socioeconomic status in young women.

In this survey the aim was to investigate the relationship between the socioeconomic status of young women of child bearing age and their daily intake of calcium and other selected nutrients. Although no significant relationship was found there were other findings of nutritional interest. Further support is given to the theory that young women are a vulnerable group with respect to nutrition.
