High density lipoprotein cholesterol low serum level as the only risk factor in male patients with coronary heart disease.

Forty-three male patients with myocardial infarction, severe angiographic coronary lesions, high or normal low density lipoprotein (LDL) cholesterol serum levels, and without other risk factors for coronary heart disease were selected. In all patients high density lipoprotein (HDL) cholesterol and HDL-cholesterol/total cholesterol ratio were significantly lower than in the control group; in particular, the ratio was below 0.240 which was the median value of normal subjects. Six patients with total cholesterol and LDL-cholesterol below 5.16 and 3.35 mmol/l respectively, had low serum levels of HDL-cholesterol; in 4 of them the value of this risk factor was below 0.9 mmol/l; 2 of 6 patients had a lipoprotein (a) serum concentration above 0.3 g/l but not a premature myocardial infarction or a clinical history of coronary heart disease. Our data confirm that HDL-cholesterol/total cholesterol ratio could be a better marker of coronary heart disease than HDL-cholesterol, total cholesterol or LDL-cholesterol and suggest the importance to check HDL-cholesterol serum levels also in subjects without risk factors for atherosclerotic disease.

[Effects of simvastatin on atherogenic blood lipid pattern].

The effect of a 6-month treatment with simvastatin, an HMG CoA-reductase inhibitor, on serum lipoprotein pattern was investigated in patients affected by primary hypercholesterolemia. After 4-6 weeks of lipid lowering diet, 46 patients showing serum cholesterol greater than 250 mg/dl and triglycerides less than 200 mg/dl were included in the study and treated with simvastatin (10-30 mg/day) plus diet. Serum total cholesterol, LDL-cholesterol, apolipoprotein B and triglycerides levels significantly decreased at the end of the follow-up (-27%, -33%, -22%, -26% respectively). Apolipoprotein A1 showed a significant increase as well as HDL-cholesterol, HDL3-cholesterol and apolipoprotein A1/B ratio. This study provides further findings on the simvastatin efficacy to normalize the lipoprotein pattern in primary hypercholesterolemia.

[Evaluation of the clinical effectiveness of biofeedback EMG on muscular relaxation and painful symptomatology].

The therapeutic effectiveness of EMG biofeedback on muscular relaxation and pain symptomatology has been analysed in a group of 24 dysfunctional patients presenting consecutively at the Prosthesis Department of the Odontostomatological Clinic. A good level of muscular relaxation was achieved in 22 cases; pain symptomatology disappeared totally in 19 and reduced considerably in 3 of 24 patients examined. Therapy failed in 2 cases. The method thus presents unquestionable therapeutic value in dysfunctional patients, particularly in cases with symptomatology that is mainly due to muscular hyperactivity.

[Electromyographic evaluation of muscular tension in dysfunctional subjects in relation to normal subjects].

Electromyography has been used to analyse the myoelectric activity of the masticatory muscles in 130 dysfunctional patients in comparison with normal subjects. Of the muscles examined, those presenting greater basal tension were also identified. Myoelectric activity proved greater, at postural values, in dysfunctional patients; by contrast, subjects without craniomandibular symptomatology presented normally low initial values. As for the analysis of the muscular group presenting greater tension, analysis of the results did not produce statistically significant percentages pointing to the prevalence of one muscular group over another.

[Considerations on a case of hemolytic microangiopathic anemia in a patient with adenocarcinoma of the gallbladder].

The authors report a case of hemolytic microangiopathic anemia (HMA) in a patient with gallbladder carcinoma with metastases to the liver, lymph nodes of the hepatic hilum, omentum, iliac bone and its bone marrow. After an overview of the literature on HMA and particularly in the peculiar mechanism of the small vessel damage, the clinical and laboratory picture is described. It is also underlined that this is the first case described in which the association of gallbladder adenocarcinoma and HMA is present. The progressive and fatal course of the disease is tied to the severe erythrocytic lysis and mechanical damage by the massive microembolization of small vessel walls by neoplastic cells. This also explains the complete uselessness of blood-transfusion therapy.

[Epidemiology of the cardiovascular malformations. II. Follow-up of 337 newborn in 1975-1980 at the Maternity of Florence (author's transl)].

337 newborn, in whom a cardiovascular malformation was diagnosed at birth by criteria previously reported, were followed for variable period of time from January 1975 to December 1980. They came from a population of 32,561 live births which occurred at the Maternity of Florence in the same period. The diagnosis was confirmed by angiography, surgical procedure or autopsy in the 15 per cent of the whole group. Fifty-nine babies (18 per cent) with one exception died within the first year of age. Signs and symptoms referable to the cardiovascular malformation at birth disappeared during the follow-up period in 33.8 per cent of cases. Total incidence lowered from 10.3 per thousand at birth to 6.8 at the end of the follow-up. The percentage of normalized babies correspond to the frequency with which ventricular septal defects close spontaneously in the first period of age and accounts for the disappearance of this malformation from the first place of relative incidence of cardiac malformations in adults.

[A common pool of adenylate cyclase for different types of afferent receptors].

The authors investigate whether adenylate cyclase, that is a component of different receptors, i.e. the beta-adrenergic and H2-Histaminic ones of the heart, either plays a specific role for the activation of each receptor of forms the same pool for two types of receptors. On the mechanogram of guinea pig ventricle, stimulated by electric stimuli, an analysis of interferences among isoprenaline, propranolol, histamine and noradrenaline shows that isoprenaline inhibits histamine; in presence of propranolol the isoprenaline doesn't block histamine. Therefore the AA. conclude that adenylate cyclase is common to both receptors.

[An epidemiological study of congenital heart disease. I. Incidence of cardiovascular malformations in 11,584 live-born babies in the District of Florence (Italy) (author's transl)].

11,584 consecutive live-born babies in the Ostetric Clinic of Florence in the years 1975 and 1976 were examined for the presence of congenital heart disease. Diagnosis was made by necropsy, cardiac catheterization and operation in 20% of cases and by clinical observations in 80%. The 74% of babies who were diagnosed as affected by congenital heart disease at birth were eventually controlled. The incidence of congenital heart disease was 7.1% in 10,789 live-born of weight over 2500 g while it was 37% in 795 under such weight. Global incidence was 9.2%. The first essentials in epidemiological study of congenital heart malformations is to define the method which is used for diagnosis since this is the primary factor responsible for wide variations in incidence. It is suggested that the institution of a Regional Register of malformations would allow more accurate estimate of overall incidence of congenital cardiopathies.

Amikacin in obstetric, gynecologic, and neonatal infections: laboratory and clinical studies.

Based on the proportion of resistant, moderately sensitive, and sensitive strains, the descending order of activity of amikacin against clinical isolates of urinary pathogens was Salmonella, Klebsiella, Enterobacter, Escherichia coli, Staphylococcus aureus, Citrobacter, Proteus species, and Pseudomonas aeruginosa. However, amikacin was the most active of the antibiotics tested (including gentamicin and tobramycin) against 100 strains of P. aeruginosa. The calculated half-life of amikacin was substantially longer in patients with compromised renal function than in normal subjects. Immaturity of renal function, characteristic of the newborn, similarly slowed the rate of excretion of amikacin. The cure rate (complete clinical remission and eradication of the pathogen) was 91% in 22 patients with urinary tract infection (including 16 with chronic pyelonephritis) treated with 500 mg of amikacin every 8 or 12 hr for eight to 17 days. After single injections of 7.5 mg/kg 2-3 hr before delivery, appreciable amounts of the drug were recovered from the cord blood. No local or systemic intolerance or laboratory abnormalities were observed in a total of 42 patients (including eight infants) treated for a maximum of two weeks. No ototoxicity was demonstrable in any of the 12 patients subjected to audiometry; nystagmography revealed slight vestibular dysfunction in two elderly patients.
