Endoscopic gastroduodenal polypectomy.

The authors performed a total of 288 successful endoscopic gastroduodenal polypectomy procedures in 129 patients. Drawing on literary data and the results of follow-up of their own patients, they regard endoscopic gastroduodenal polypectomy as a successful and relatively low-risk method for secondary prevention of gastric cancer. Recurrence of adenomas, with malignant degeneration in 6-75%, was noted, only in the first year after primary polypectomy, in 8% of patients. Four percent of patients after adenoma polypectomy developed gastric carcinoma. Removal of gastroduodenal adenomas by endoscopy represents a final therapeutic procedure significantly reducing the possibility of these originally benign variants transforming into malignant ones. Even polyps smaller than 10 mm in diameter may be adenomas possessing a rather high malignant potential. The authors regard endoscopic polypectomy of hyperplastic polyps as a justified procedure since the literary data compiled and their own experience suggest polyp susceptibility to development of dysplasia. While this tendency gets manifest only rarely, it would be medically wrong to dismiss it as negligible. In case of removal of an early polypoid gastric carcinoma, endoscopic polypectomy is a therapeutic procedure only in clearly defined and histologically well classified cases. In other cases involving prominence of the gastroduodenal mucosa, it is an invaluable diagnostic method whose benefit is that it removes an entire polyp for histologic examination.

The incidence of carcinoma in situ in postpubertal undescended testis.

In a retrospective study of 32 adult males undergoing "preventive" orchiectomy for unilateral undescended testis the average age of the patients at the time of orchiectomy was 28.3 years (range 16-63). Macroscopically, all the testes were atrophic and there was no evidence of tumour. In all surgically removed testes histological examination showed atrophied germinative epithelium and absence of spermatogenesis. In one patient, aged 16, an obvious carcinoma in situ was identified. In another 3 patients, aged 18, 19 and 29, atypical germ cells were found. Unilateral undescended testis in postpubertal patients should be treated by orchiectomy instead of orchiopexy.

Determination of cardiac output by the CO2 - rebreathing method in fifteen-year-old boys subjected to graduated loading.

The dynamics of cardiac output changes were studied in 23 15-year-old boys subjected to graduated loading up to maximum on an bicycle ergometer and relationships between cardiac output and other indexes of cardiorespiratory functions were determined. The correlation between cardiac output (Q in 1/min) and oxygen consumption (VO2 in 1/min) was Q = 6.84 + 5.64. VO2. CO2-rebreathing was found to be a suitable non-invasive method for determining cardiac output in loading tests in adolescents. Maximum cardiac output correlated with maximum oxygen consumption, maximum pulmonary ventilation, body weight, lean body mass and physical working capacity W 170 and did not correlate with systolic volume, the arteriovenous oxygen difference, the pulse rate and ballistocardiographic force. The absence of any statistically significant differences between the various functional indexes of the physical fitness of trained and untrained boys indicates that training was not very effective.

[Hip screening of newborn infants. Type classification, therapy and follow-up].

Between December 1982 and the end of January 1987, a total of 21,082 hip sonographies were performed at the authors' clinic to establish whether or not screening of newborns, including sonographic follow-up, was desirable. The incidence of sonographically detected neonatal dysplasia up to the eight week of life was 2.23%; the incidence of dislocation was 0.01%. Hip types IB and IIA have a tendency to deteriorate in the eighth and twelfth weeks. During the first few weeks of life more than one-half of all HD cases manifest no symptoms, either clinical or historical. A complete cure was achieved after an average of five months when suitable therapy was instituted post partum, and after eight months when it was instituted later.

[The nephrotic syndrome in renal biopsies].

Among 1,499 representative renal biopsies in 278 cases nephrotic syndrome was diagnosed. In the latter only in 201 glomerulonephritis was confirmed by biopsy and in 77 (more than one quarter) the bioptic finding was different. The authors attempt to analyze the essential feature of nephrotic syndrome focused on the glomerular basal membrane and surrounding structures.
