Local chemotherapy in bladder cancer treatment.

Intravesical chemotherapy can be employed either in a therapeutic aim or as an adjuvant prophylactic treatment after TUR. In this paper a discussion is presented about various controversial points that need to be clarified with regard to topical chemotherapy, with special reference to the selection of the drug and to various modalities of treatment. Some results are presented from the literature and from the author's personal experience, with emphasis on the randomized clinical trials performed by EORTC and by other workers or groups.

EORTC protocols in prostatic cancer. An interim report.

Two parallel prospective randomized studies have been undertaken by the EORTC Urological Group in previously untreated patients with prostatic cancer in order to compare low dose Stilboestrol versus Cyproterone acetate versus Medroxyprogesterone acetate in the first trial, and Stilboestrol versus Estracyt in the second trial. Although the follow up is still short, no superiority of the other drugs over Stilboestrol had appeared so far with regard to either objective response or significant side effects apart from gynaecomastia. In the third trial, patients with advanced disease no longer responsive to hormonal treatment were randomized to either Adriamycin or Procarbazine. Toxicity and early death were particularly frequent in Procarbazine treated patients, whereas most patients progressed in both treatment groups.

[Per-urethral coitus. One case followed by pregnancy].

Description of a case of intravesical coitus via the urethra in a woman with vaginal atresia with a punctate orifice, initially unnoticed. Pregnancy may occur by external insemination, despite this anatomical situation. Incontinence was worsened by hymenotomy and by the pregnancy. Incontinence without cystitis is the fact noted most frequently in the 18 cases found in the literature.

Adjuvant therapy of T1 bladder carcinoma: preliminary results of an EORTC randomized study.

This paper reports the preliminary results of an ongoing clinical trial in patients with category T1 bladder cancer who are randomized after transurethral resection to receive either thiotepa, VM-26, or no treatment. While there are no significant differences between the three treatment groups with respect to the time until first recurrence, thiotepa has significantly reduced the recurrence rate as compared to either VM-26 (P = 0.03) or no treatment (P = 0.04) among the 215 patients for whom follow-up information is currently available.

Permeability of the bladder mucosa to thiotepa, adriamycin, and daunomycin in men and rabbits.

The permeability of the bladder mucosa to thiotepa and to the anthraquinonic antibiotics, adriamycin and daunomycin, was investigated both in humans and in experimental animals. Instillations in rabbits were performed either in intact males or in animals with ligated ureters. Absorption of thiotepa was significantly higher than that of the antibiotics both in men and in rabbits. Furthermore, a qualitative difference was observed in rabbits in relation to time and with regard to fixation to vesical tissues. In man, absorption was highest after transurethral surgery. It was also increased in cases with extensive anaplastic tumours or in the presence of acute inflammatory reactions.

Preliminary evaluation of VM-26: a new epipodophyllotoxin derivative, in the treatment of urogenital tumours.

VM-26, a podophyllin derivative, is cytotoxic in vitro against cell cultures of bladder tumours. Intravenous therapy of vesical carcinoma produces only occasional responses. One total regression was observed in our series. Intravesical instillations are more effective. Topical treatment of papillomatosis led to total or partial regression in one half of cases. A significant reduction of recurrence rate following transurethral surgery was obtained in patients treated intravesically with VM-26 at weekly intervals.
