Use of tranexamic acid mouthwash to prevent postoperative bleeding in oral surgery patients on oral anticoagulant medication.

The clinical hemostatic effect of tranexamic acid mouthwash after oral surgery was evaluated in 47 patients receiving oral anticoagulant therapy. Surgery was performed after the anticoagulant medication was reduced in 15 patients (control group) and with no change in anticoagulant therapy in 32 patients (test group). The only statistical difference between the two treatment groups at baseline was the level of anticoagulation, which was significantly higher in the test group. There was no significant difference between the two treatment groups in the incidence of bleeding after oral surgery. The results indicated that a combination of local antifibrinolytic therapy and a local hemostatic agent is effective in preventing postoperative bleeding after oral surgery in patients treated with anticoagulants.

[Cervical necrotizing fasciitis].

Necrotizing fasciitis of the head and neck is a rare infection caused by a mixed bacterial flora with anaerobic predominance. Mortality is due to misdiagnosis which results in late and inadequate treatment. The natural course of this serious infection involves spreading necrosis of the soft tissues of the neck with erosion of major blood vessels. A 33-year-old man and a 66-year-old woman are presented to demonstrate the usual etiological factors of cervical necrotizing fasciitis, mainly dental and pharyngeal infections. Diagnosis is by bacteriological culture and CT-scan; a high degree of clinical suspicion is necessary. Treatment included aggressive surgery, appropriate antibiotic coverage and hyperbaric oxygen, and resulted in successful outcomes in our 2 cases.

Low lead level in teeth in Israel.

A total of 180 sound teeth extracted from people living mainly in Northern Israel and some in the center of the country, aged 14-75 years, were analysed by graphite furnace atomic absorption spectrophotometer. The average lead level in teeth roots ranged between 1.61-25.72 micrograms/g dry root, according to the donor's age. This low level was measured in all the regions which were checked, with no significant differences between them, despite differences in levels of motor traffic rates. Based on the low lead level in teeth, we assume that no further reduction in the lead level in gasoline is needed, as long as motorization rate or gasoline consumption does not change significantly.

Conservative approach to unerupted teeth within cystic lesions in Gorlin's syndrome.

The unerupted maxillary canine is the tooth that most commonly requires surgical exposure and guided eruption. The orthodontic specialist must be aware that delayed eruption is caused, sometimes, by serious pathologic conditions such as cystic or neoplastic malformation. A case of a 12-year-old girl who suffered from Gorlin's syndrome (nevoid basal cell carcinoma syndrome) is presented. Her symptoms were multiple jaw keratocysts, skeletal anomalies, and cleft lip and palate. Some authors advocate an aggressive surgical approach to jaw keratocysts because of their tendency to recur, thus sacrificing those teeth associated with them. Our view is that in young patients with Gorlin's syndrome, an attempt should be made to preserve permanent teeth, especially those vital for a normal growth pattern of the jaws. Therefore we recommend a conservative surgical approach, enabling guided eruption of unerupted teeth associated with cystic lesions in patients with Gorlin's syndrome.

Treatment of mandibular fractures by means of compression osteosynthesis.

This report is a retrospective study of 76 patients with mandibular fractures treated by compression osteosynthesis. Good results were achieved in 83% of the cases. The complications encountered were either soft tissue infections or minor malocclusions. It was concluded that the relatively low rate of complications can be further reduced by careful selection of patients and meticulous performance of the technique.

Allopurinol-induced toxic epidermal necrolysis.

An erythematous eruption with itching developed in an 84-year-old man 4 days after therapy with allopurinol was initiated. The diagnosis of toxic epidermal necrolysis, suspected when separation of the epidermis was noted, was confirmed by skin biopsy. This is the third reported case that can be attributed exclusively to allopurinol and the first patient who did not die.

Intestinal loss of vitamin D3 metabolites in rats with cirrhosis of the liver.

The metabolism of cholecalciferol (vitamin D3) was studied in cirrhotic and in control rats that were either depleted of or supplemented with vitamin D3. The vitamin was supplemented by s.c. injections of radiolabeled cholecalciferol at doses which meat the daily nutritional requirements of rats. Excretion of vitamin D3 metabolites in the feces of the cirrhotic rats was greater than that in the control rats. Fecal excretion consisted mainly of vitamin D3 ester the unchanged vitamin D3 and 25-hydroxyvitamin D3. It is suggested that one of the causes for vitamin D3 depletion in cirrhosis of the liver is fecal loss.

Changes in semen quality and fertility in response to endocrine treatment of subfertile men.

786 subfertile men received hormonal treatment in order to improve their semen quality and concentration. Most of the patient's semen was characterised as idiopathic oligo or asthenozoospermia. No cases of hypothalamohypopituitary-hypogonadism were included in this study. 117 men received human chorionic gonadotropin (hCG); 47% showed improvement in semen quality mainly in motility and morphology: 10 pregnancies were recorded among 44 couples (23%). 37 received human menopausal gonadotropin (hMG) + hCG; 46% improvement in semen quality, mainly in the motility and concentration was seen: 3 pregnancies were recorded among 20 couples (15%). 60 men received clomiphene citrate; 47% showed improvement in semen quality mainly in the concentration and motility: 2 pregnancies were recorded among 33 couples (6%). 270 men received androgen, usually in small doses; 61% had improvement in semen quality, mainly in the motility and morphology: 24 pregnancies were recorded among 58 couples (41%). 262 men received pregnant mare serum (PMS; Gestyl) and testosterone; 71% had improvement in semen quality in all the parameters measured especially in motility and vitality, but only 24 pregnancies were recorded among 87 couples (28%). 40 men received treatment for improvement of potency which included androgen, yohimbine and strychnine; 60% improvement in sexual potency, 65% showed a significant improvement in semen quality: 4 pregnancies were recorded among 15 couples (27%). The parameters for improvement of sperm quality were motility, concentration, morphology and vitality. Improvement in semen quality was not necessarily correlated with impregnation of the wives.

Metalloproteinases (MMPs -2, -3) are involved in TGF-beta and IGF-1-induced bone defect healing in 20-month-old female rats.

Matrix metalloproteinases are important in the physiological and pathological degradation of extracellular matrix including that of bone and cartilage. The process of bone defect healing is associated with formation of cartilage callus and cancelous bone. With maturation and aging, the response of skeletal tissues to injury is limited. The ability of growth factors to enhance bone defect healing in aged rats was studied. Partial bone defects were induced in femurs of aged rats. A single dose of IGF-1, TGF-beta+IGF-1 or saline was inserted in the defect and bones were examined after 2 and 4 weeks. Morphology revealed that after 2 weeks of treatment with TGF-beta the defects were filled with mesenchyme-like tissue and delicate bone trabeculae. Positive staining for metalloproteinase-2 (MMP-2) was shown at the sites of new bone formation. In defects treated with IGF-1 or TGF-beta+IGF-1 nodules of cartilage and fine bone trabeculae along with positive staining for both MMP-2 and MMP-3 were demonstrated in the healing defects. After 4 weeks radiology revealed mineralization in defects treated with TGF-beta and less pronounced mineralization after treatment with IGF-1, or with TGF-beta+IGF-1, whereas only partial healing of the defects was observed in control specimens. MMP-2 and MMP-3 were detected at sites of new bone formation after treatment with TGF-beta, IGF-1, and TGF-beta+IGF-1. It is concluded that TGF-beta and IGF-1 induced bone defect healing in aged rats. TGF-beta induced bone formation while IGF-1 induced cartilage and than bone formation via endochondral ossification. The localization of MMP-2 and MMP-3 in the healing defects reflected the synthesis of bone or cartilage matrices in the defect, reflecting the involvement of MMPs in the process of bone formation and endochondral ossification. The ability to induce bone defect healing in aging is of great clinical importance and understanding the involvement of MMPs in this process can contribute to future treatment with growth factors to enhance bone defect healing in 20-month-old female rats.
