The closure of urinary-vaginal fistulas.

Reported series of urinary-vaginal fistulas repaired by a vaginal approach show at least 85% primary closure. However, many patients have had several unsuccessful operations elsewhere prior to the final repair. This means that regularly successful closure is possible, but it demands a certain "know-how" and engagement. In search for the general principles leading to such successful primary closure, different published techniques have been studied. It appears that the essential features are the establishment of a proper distance between the inner vesical and the outer vaginal epithelium and the creation of broad raw surfaces to be ajoined. The operation should not involve any risk of enlarging the existing fistula, and the transfer of normal tissue to the site of the repair and the interposition of tissue between the cavities to be separated are sound reconstructive procedures. A personal technique based on these principles is presented. The gynecologists of to-day most often refer urinary-vaginal fistulas to the urologists, who almost without exception prefer a suprapubic transvesical approach. It is strongly recommended that the large majority of urinary-vaginal fistulas should be repaired by the vaginal approach which is a minor intervention. The plastic surgeon should be engaged in the management of urinary-vaginal fistulas.

The incidence of malignant transformation in giant pigmented nevi.

151 patients with benign giant pigmented nevi registered in the Danish Health system during the 60-year period 1915-75 were retrieved via the national register. A questionnaire was sent to all surviving patients asking for information about their health and especially whether they had had any treatment or had observed any changes in the nevus. All of the patients replied to the questionnaire. No patients had been cured from malignancy or were alive with known malignancy. Three patients had died from malignant melanoma during the period of observation. These case histories are reported. It is calculated that 4.6% of the patients with congenital giant nevi should be expected to develop malignant melanoma provided the incidence is the same in all age groups. Some uncertainty remains on account of the limited number of cases and an unsettled question about a higher incidence in childhood.

150 open fractures of the tibial shaft--the relation between necrosis of the skin and delayed union.

In 150 open fractures of the tibial shaft a close relationship between the presence of skin necrosis and delayed union was found. The incidence of skin necrosis rose with the severity of trauma. It was high (22 per cent) among the fractures that were treated conservatively, but even higher (41 per cent) among those treated with primary osteosynthesis. Neither conventional conservative treatment nor osteosynthesis seems to be satisfactory as a primary treatment of the most severe cases. It is instead suggested that external fixation by the Hoffmann-Vidal technique should be combined with transposition of viable muscle tissue across denuded fractured bone areas according to the method of Ger.
