[Prevention of thromboembolic complications in surgery].

It is known that during the postoperative period about 25-50% operated patients develop postoperative venous thrombosis. The mechanism of development of postoperative thrombosis is promoted in surgical patients by the so-called "wound healing process" with an enhanced synthesis of fibrinogen, inhibitors of fibrinolysis and other proteins of the acute phase for release of various cytokins (IL-1, IL-6, TNF, TGF and others). The development of thrombosis is promoted also by other risk factors: advanced age, extensive surgical trauma, infection, immobility, obesity etc. Preventive administration of so-called small doses a of heparin or rather so-called low molecular heparin (LMWH) substantially reduces the risk. The authors submit also a list of drug with an antithrombotic action and rheologics.

[The effect of corticoids on bone tissue in myasthenia gravis].

Myasthenia gravis is an autoimmune disease where corticoids are the basis of therapy. They are taken for short periods in large amounts, as well as for prolonged periods in medium or small doses. The authors investigated in the described groups the effect of corticoids on bone tissue. They provided evidence of a significant effect on the diffraction pattern, geometrical arrangement of the apatite grid and ion changes in relation to the period of corticoid administration without significant clinical manifestations of osteoporosis, when respecting therapeutic principles.

[Iatrogenic injury of the esophagus].

The author describes four patients treated in 1988 on account of iatrogenic oesophageal injury during diagnostic endoscopy. Twice the cervical portion of the oesophagus was perforated, once the lower thoracic portion and once subperitoneal perforation in hiatus hernia was involved. The latter was manifested only by a major pneumomediastinum and emphysema of the neck and face, while contrast examination of the oesophagus and stomach did not suggest perforation and recovery was achieved by conservative treatment. In the remaining three patients surgical revision and suture were performed and complete recovery was achieved, twice without complications after a hospitalization of 14 and 18 days and once the course was complicated by an abscess in the mediastinum. Its treatment called for repeated drainage and a total hospitalization period of 57 days. Finally the author emphasizes that early diagnosis of this serious complication and subsequent early treatment (with the exception of one female patient the others were treated within six hours after the injury) may get the patient over this complication without serious damage.

[Late reoperation in patients with carcinoma of the esophagus and cardia].

The authors give an account of 17 late re-operations performed after 14 resections and three palliative operations of carcinoma of the oesophagus and cardia, on account of stenosis of the anastomosis. After primary resection on account of carcinoma of the cardia 9 patients were re-operated and on account of carcinoma of the oesophagus five patients: three times because of a cicatricial stenosis and 11 times on account of narrowing of the anastomosis due to a relapse of the oesophageal tumour or in the surrounding nodes. The interval after resection was on average 13.7 months. Re-resection was possible only in two patients, seven times an endoprosthesis was introduced, three times a bypass was made, once jejunostomy and once the operation ended by probatory laparotomy. The mortality of all re-operations was 35.7%, the survival was 6-11 months.

[Chronic volvulus of the stomach].

An acute gastric volvulus calls for an urgent operation, while a chronic volvulus can be indicated for operation only after a careful clinical and paraclinical examination which rules out other causes of the symptoms. The authors describe a case of secondary gastric volvulus successfully treated by a causal surgical operation involving elimination of adhesions without gastropexy.

[Administration of Decaris (levamisole) patients with complicationsp6n surgical wound healing].

The authors report on their experience with the administration of Decaris (Levamisol) to patients with complication in the surgical wound after surgery. Decaris was administered on average 20.2 days after unsuccessful common local and general treatment. In patients with an anamnestic suspicion of immunodeficiency this drug was administered already before operation. After Decaris administration in 77% of the patients the wounds healed within 24 days. For patients with complications in surgical wounds Decaris treatment may be one of the possible conservative approaches to their condition.
