A comparative study of visuospatial memory and learning in Alzheimer-type dementia and Parkinson's disease.

Groups of patients with dementia of Alzheimer type (DAT) and idiopathic Parkinson's disease, together with age and IQ-matched normal controls, were compared on several computerized tests of visuospatial memory and learning. Two different groups of parkinsonian patients were studied: (1) a newly diagnosed group, early in the course of the disease, not receiving medication (NMED) PD) and (2) a group later in the course of the disease, receiving medication (MED PD). The DAT and MED PD group were significantly impaired in both spatial and visual pattern recognition memory. The DAT group exhibited a delay-dependent deficit (over 0-16 s) in a delayed matching-to-sample procedure, but were not impaired at simultaneous-matching-to-sample. By contrast, the MED PD group showed delay-independent deficits in the delayed matching-to-sample test and both the MED PD and the NMED PD group were also significantly impaired in simultaneous matching. In a form of delayed response test, the subjects were required first to memorize and then to learn the locations of several abstract visual stimuli which varied progressively in number from 1 to 8. The DAT group were severely impaired in this conditional associative learning task. A significant proportion of patients, but none of the controls, in the NMED and MED PD group also failed the test at the levels of 6 or 8 items. There was a significant correlation between the performance on the first trial, memory score in the delayed response task and indices of clinical disability and disease duration in the patients with Parkinson's disease. The results are discussed in terms of the utility of the comparison between DAT and PD in characterizing the nature of the cognitive deficits in these conditions and their relation to those findings from animal neuropsychology which use comparable paradigms.

The routine use of investigations in elderly psychiatric patients.

The use of investigations was studied in all new admissions (n = 167) to a psychiatric unit for the elderly over an 18-month period. Twenty per cent of the tests revealed abnormalities confirming the relatively high prevalence of physical pathology in this population. However, only 4% of the investigations requested influenced further management. It is suggested that full blood count, serum folate, urea and electrolytes and urine culture should be performed routinely, but that other investigations might be ordered selectively.

Preparation and properties of partially purified pulmonary cytochrome P-450 from rabbits.

Cytochrome P-450 from rabbit pulmonary microsomes was purified approximately 32-fold. The purification method involved solubilization of microsomes using sodium cholate, and recovery of cytochrome P-450 in the precipitate formed between 25 to 42% saturation of the digested microsomes with ammonium sulfate in the absence of glycerol. Further purification was achieved by chromatography on DEAE-cellulose and hydroxylapatite using Emulgen 913 as an eluent. Partially purified preparations containing up to 7.4 nmol of cytochrome P-450 per mg of protein were essentially free of NADPH-cytochrome c reductase activity and cytochromes b5 and P-420. However, epoxide hydrase was found to co-purify with cytochrome P-450. The CO-difference spectrum of dithionite-reduced purified cytochrome showed the expected peak at 450 nm. However, the magnitude of the peak was dependent on added microsomal lipid fraction in the assay medium. Purified pulmonary cytochrome P-450 formed typical types I and II substrate difference spectra with benzphetamine and pyridine, respectively. Sodium dodecyl sulfate-gel electrophoresis of partially purified cytochrome P-450 gave two major bands when stained with Coomassie blue. The faster moving band which contained peroxidase activity had an estimated molecular weight of 49,000 +/- 1,200. The cytochrome P-450 fraction, when combined with solubilized pulmonary microsomal NADPH-cytochrome c reductase and lipid fractions, was active in the O-deethylation of 7-ethoxycoumarin and the N-demethylation of benzphetamine.

A comparison of Aspergillus fumigatus antigens by counterimmunoelectrophoresis.

Counterimmunoelectrophoresis (CIE) has been evaluated for routine diagnostic work, using antigens prepared from 2 different isolates of Aspergillus fumigatus, at 2 different concentrations. Additional antigens prepared in a variety of ways and 2 commercially available antigens (Bencard, London; Institut Pasteur, Paris, France) have been compared with the routine antigens. It has been shown that the use of the routine antigens will detect the majority of positive reactions. At optimal reacting concentrations antibodies were detected in 75% of asthmatic patients. In sera from patients with a presumptive diagnosis of allergic aspergillosis, CIE will detect twice as many positive reactions as a conventional agar gel diffusion test.
