[Preliminary evaluation of the diagnostic usefulness of bronchoalveolar lavage (BAL) in selected cases of interstitial lung diseases].

Diagnostic usefulness of BAL was assessed in 25 selected cases of interstitial lung diseases. There were 14 cases of allergic alveolitis, 6 cases of idiopathic pulmonary fibrosis and one case each of sarcoidosis, lymphangiomyomatosis, granulomotosis bronchocentrica, lymphogranulomatosis maligna, alveolitis of unknown etiology. BAL was assessed diagnostically usefull in 77% of the cases. More helpful was medical history which proved to be useful in all cases, and pulmonary function tests which were helpful in 84%. Lung biopsy was diagnostic in 66.6% of the cases, TBLB in 36.5%, while open lung biopsy carried out in 6 patients was diagnostic in 100%. Bronchoalveolar lavage is a useful element of diagnostic evaluation in patients with interstitial lung disorders, it is a safe method and well tolerated by the patients.

[Further studies on the occurrence of mycoses in the bronchial stump and bronchial anastomosis or trachea].

In the years 1980-1985 sutures were removed endoscopically from stumps and anastomoses in 52 patients operated in the years 1969-1985. The total number of bronchoscopies was 154, in 88 protruding threads into the airway lumen were removed. Interval between removal of first suture and date of surgery ranged from 0.5 to 192 months. Mycotic infection was diagnosed in 5 cases (9.6%). The causative agents being Aspergillus fumigatus and Candida albicans. In all cases removal of protruding threads lead to a complete cure. It was found that incidence of such mycotic infections decreased from 20.2% observed during the years 1962-1979 to 9.6% in the years 1980-1985.

[Allergic alveolitis in rural inhabitants].

27 rural inhabitants with allergic alveoltis were evaluated in order to determine the diagnostic usefulness of clinical, radiological, functional examination, BAL, lung biopsy, presence of serum antibodies against environmental antigens in determining the diagnosis and clinical stage of the disease. The effect of therapy and exposition on lung function tests were also discussed. Corticotherapy in most of the patients produced an improvement of vital capacity. In part of the patients isolation from environmental antigens produced stabilization of respiratory parameters. Small dose continuous corticotherapy in patients with prolonged exposition proved to be beneficial and prevented occurrances of acute relapses, as well as decrease of respiratory functional parameters values.

Effect of different volumes of BAL fluid on arterial oxygen saturation.

We monitored arterial oxygen saturation (StcO2) continuously using the OHMEDA Biox 3700 oximeter. We studied seven patients undergoing a 100 ml bronchoalveolar lavage (BAL-100), seven patients undergoing a 200 ml bronchoalveolar lavage (BAL-200), and seven patients during diagnostic fibreoptic bronchoscopy alone. Immediately following insertion of the bronchoscope a brief increase in StcO2 level (0.3-0.5%) was seen, followed by a gradual decline, never exceeding 2-5% during the diagnostic bronchoscopy. Introduction of lavage fluid into a segmental bronchus always produced a further decline in StcO2. In the BAL-100 group the fall did not exceed 7%, whereas in the BAL-200 group a fall of up to 15% from the baseline level was observed. Return to initial values was seen in most of the patients within 10 min following completion of the procedure. Only in those patients with the most profound StcO2 fall was this period increased, up to 30 min.

[Wegener's granulomatosis with tracheoesophageal fistula].

A case of Wegener's granulomatosis in a 54 year old female is presented. The disease course was fulminating, the necrotic process involving lungs, gingiva, skin, trachea, producing finally a tracheo-esophageal fistula. Therapy with cyclophosphamide and prednisone produced resolution of all organ involvement except for the tracheo-esophageal fistula.

Activity of angiotensin I converting enzyme in sarcoidosis, atopic bronchial asthma and acute bronchitis.

The lungs have an important role in the synthesis of ACE. In BAL fluid and serum the ACE activity was determined in 18 patients with sarcoidosis (11 with high intensity and 7 with low intensity alveolitis), 25 patients with atopic bronchial asthma and 17 with acute bronchitis. The activity of ACE was examined by a reagent set produced by Boehringer Mannheim Biochemica Test-Combination ACE cat. no. 789 011. In the high intensity alveolitis group of sarcoidosis patients the ACE activity was significantly increased in BAL fluid and serum in comparison to other observed patients. On the other hand, in patients with atopic bronchial asthma the ACE activity was also increased in comparison to acute bronchitis and referred norms. These findings suggest a role of atopic processes or administered therapy in ACE secretion in the airways.

[The role of BAL in the process of decision making with regard to the treatment of sarcoidosis].

The results of a preliminary analysis on the role of lymphocyte count on prognosis of therapy outcome and influencing the decision to start therapy carried out on 21 patients with sarcoidosis are presented. The decision to commence treatment was carried out after BAL was performed but the results of it were not taken in account. The patients were observed for 7 to 33 months. The course of the disease was assessed basing on clinical, radiological and functional examination. In 6 patients a control BAL was carried out. The results allow to state that high lymphocyte BAL counts do not have to be poor prognostic factors. On the other hand low counts cannot predict a benign clinical course. In the discussion the authors evaluate, basing on literature data, the role of BAL in sarcoidosis.

[Skewed shape of the maximal expiratory flow volume curve and bronchoscopic signs of lower airway collapse in chronic obstructive pulmonary disease].

We compared endoscopical signs of airway collapse seen during tidal breathing with the shape of the maximal-expiratory flow volume curve and selected functional parameters in 16 cases of chronic obstructive pulmonary disease (COPD). Depending on localization of airway collapse the group was divided into two. 12 patients with central collapse involving the trachea and main stem bronchi formed group A. The remaining 4 patients with peripheral collapse (lobar and segmental bronchi) formed group B. In the former group the MEFV curve had a characteristic biphasic shape. The shape of this curve in the group B patients was more curvilinear. In group A patients an increase of Raw, TGV and fall of FEV1 was seen which could not be demonstrated in group B. The biphasic shape of the MEFV curve seems to be characteristic of airway collapse, but due to a small number of patients included we can not assess the specificity of it in diagnosing airway collapse.
