Changes in coagulability of blood in patients with cerebrovascular disease in various meteorological situations.

The authors are dealing with the influence of meteorological factors on vascular diseases, primarily on patients with cerebro-vascular disturbances. The parameters mentioned in some studies are also presented in the introduction. There are still few studies by means of which it can be enlightened what happens in the human body affected by atmospheric disturbances. Therefore the basic intention of this study was to come up with answers to at least some segments of this complex problem. A group of patients with cerebrovascular symptoms and signs was chosen for examination. Tests were carried out during cold and warm front passages in anticyclonal situations and low gradient pressure fields within 47 days. In the first stage the tests were carried out only in one day and later for three consecutive days. Meteorological reports are given too. As for medical parameters, the following were controlled: the neurology status, blood pressure, blood test, ECG, REG. The coagulum was regularly checked. The examination was carried out in 12 male and 14 female patients, the age range 43-78. On the basis of the tests, the authors concluded that the most significant changes were noticed in the coagulum and thromboelastogram. According to the results, these changes are manifested during the cold front and during the cold spell caused by it. The findings point to a significant increasing of the thrombocyte aggregation and partly the aggregation index, which can be clinically manifested as the patient's health aggravation. Although other parameters of coagulation did not show significant changes, it cannot be deduced whether they are susceptible to the circumstances in the atmosphere or not.(ABSTRACT TRUNCATED AT 250 WORDS)

Observation of the changes of plasma proteins after long term plasmapheresis.

The influence of long term plasmapheresis on the health of donors was examined in two groups of plasma donors that donated mean volume of 411 ml of plasma during 176 weeks and 670 ml of plasma during 123 weeks (p less than 0,05). The control group consisted of 27 whole blood donors. Statistically no significant differences (p greater than 0,05) were found in the concentrations of total proteins, albumin, gammaglobulins, immunoglobulins, alpha 1 antitrypsin, alpha 2 macroglobulin, plasminogen, fibrinogen, factor V, factor VIII, GPT and alkaline phosphatase. Although the difference was significant for bilirubin and GOT the mean values were within the normal range. Significant elevations were found in alpha 1 globulins, and alpha 2 globulins in the group that donated 411 ml of plasma/week after 35 sessions. In this latter group of donors the elevation of beta globulins was observed after 100 sessions. On the basis of these results we suggest that plasma donors should not donate more than 500 ml of plasma per week and that the maximal number of regular plasmapheresis should not exceed 70. The yearly number of sessions should therefore not exceed 50 and the yearly donated volume of plasma should be not more than 25 liters.

Prevention of venous thromboembolism with low doses of heparin in urologic patients.

The effectiveness of low doses of heparin was investigated for the prevention of clinically manifested thromboembolic disease. In the group of 125 patients, who received 5,000 units of subcutaneous heparin two hours before the operation, and postoperatively 15,000 units per day for seven to ten days, deep vein thrombosis was not observed, and in only 1 patient (0.8%) pulmonary embolism was suspected. Of 216 patients from the control group, pulmonary embolism was clinically diagnosed in 4 (1.9%), deep vein thrombosis in 7 (3.4%), and in 2 patients (0.9%) fatal pulmonary embolism was found at autopsy. The difference between total thromboembolic disease of the control group (6.2%) and heparinized group was highly significant (p less than 0.02). Postoperative bleeding was seen two times more often in the heparin group, but bleeding was not severe and was easily controlled.

[Preparation of granulocyte concentrates from healthy donors using a continuous-flow centrifuge (CFC)].

By means of continous flow centrifuge the authors have obtained suspensions of granulocytes from the blood of healthy donors. Results of granulocytophereses are presented here with the main points in the therapy of granulocytopenic patients with granulocytes rich suspension. The mean volume of the obtained suspension was 418ml. and it contained 9,78 X 10(9) leukocytes. 70 percent of them were granulocytes, the said volume contained 48 ml of red blood cells. All the donors had been premedicated with i.v. dexamethason (4mg./kg body weight). No one of the 51 donors developed anaemia or hypoproteinemia. After the application of the protamin sulphate, however, two donors had face flushing breathlessness and gastric ache. These symptoms disappeared after the administration of corticosteroids. Although they are connected with the administration of protamin sulphate we can not explain them.
