Articular manifestations of acromegaly.

Forty-five patients with acromegaly or gigantism were reviewed for musculoskeletal abnormalities. Abnormalities of peripheral joints occurred in 74% of the patients and spinal involvement in 47%, leading to significant morbidity. Joint abnormalities most frequently affected the large joints (hips, knees and shoulders) but the wrist and hand were also involved. The radiological features of acromegalic arthropathy are described, including vertical widening of the hip joint, enthesopathy and osteophytosis. A favourable response to treatment is associated with a less severe arthropathy and a good functional outcome.

Cerebral systemic lupus erythematosus with apnea in a patient with cardiolipin antibodies and oligoclonal bands.

We describe a patient with cerebral systemic lupus erythematosus (SLE) who had 2 episodes of prolonged apnea in addition to other manifestations of brainstem dysfunction. Oligoclonal bands, uncommonly found in SLE, were detected in the cerebrospinal fluid. Also present in our patient were anticardiolipin antibodies which have often been reported to be associated with neurological events in patients with SLE. With mechanical ventilation and high dose corticosteroid therapy, she recovered with only minimal neurological deficit. Our case stresses the potential reversibility of life threatening cerebral SLE.

Pharmacokinetics and effects of isoxicam on renal function in patients with renal insufficiency.

The pharmacokinetics of isoxicam, a new non-steroidal anti-inflammatory drug, were studied in 20 osteoarthritis patients with varying degrees of renal insufficiency. A wide variation in pharmacokinetic parameters was seen between individuals but there was no suggestion that renal function influenced pharmacokinetics. Steady state plasma isoxicam concentrations varied from 20 micrograms/ml to 130 micrograms/ml, while the plasma half-life varied from 23 hours to 58 hours. Despite a reduction in urinary prostaglandin E2 excretion, isoxicam administration did not alter renal function over a 4-week period.

Bidirectional knee joint rupture.

Acute simultaneous bidirectional rupture of the knee joint is reported in a patient with recurrent hemarthroses due to degenerative arthritis.
