[Sudden acute vascular diseases requiring surgery--still a reality].

In the introduction the authors emphasize that despite the fact that acute angiosurgical attacks have a fairly typical symptomatology with a characteristic clinical finding, physicians who participate in their diagnosis and treatment are not aware of it. In the clinical explanation and the material the authors mention the causes of acute angiosurgical attacks and the fact that after acute ischaemia reperfusion occurs. In a summary table they demonstrate the dynamics of the development and increase of acute angiosurgical attacks in 1985 and 1995 in the Slovak Republic, based on statistical data of the Institute of Health Information and Statistics. In the discussion it is emphasized that in the therapeutic results of acute angiosurgical attacks an important part is played by early restoration of the circulation, at least within 6 hours, but preferably within 3 hours after the onset of clinical symptoms. In conjunction with ischaemia and reperfusion attention is drawn to the fact that during early reperfusion in the tissue or organ oxygen radicals are formed which participate in a significant way in reperfusion damage. On account of the complexity of ischaemia and reperfusion in acute angiosurgical attacks it is useful to use the clinical term "acute ischaemic reperfusion syndrome". The authors mention possibilities how this syndrome can be favourably influenced by suitable angiosurgical procedures and pharmacological preparations. In the conclusion the authors emphasize the rising trend of acute angiosurgic attacks, whereby the therapeutic results are not always satisfactory.

[Antibiotic prophylaxis in thoracic surgery].

On a group of patients operated during the last three months under standard precautions the authors explain their views as regards antibiotic prophylaxis in thoracic surgery. They justify not only "short-term prophylactic administration of antibiotics" but also prolonged prophylaxis with antibiotics. Useful in this respect proved Mandol, cefalosporin of the 2nd generation, which is used as a standard drug without preoperative examination of the bacterial flora in sputum. Their patients did not develop infectious complications.

[Mesenteric venous thrombosis probably due to hereditary thrombophilia in an 18-year-old boy].

The authors describe the case-history of an 18-year-old patient with an extensive venous mesenteric thrombosis. The case proved fatal despite repeated surgery as a result of relapsing gangrene of the small intestine with diffuse stercoral peritonitis. Thrombosis of the mesenteric veins is a rare disease and accounts only for 4 to 10% of all acute intestinal episodes. The cause of the disease is either idiopathic but more frequently it is associated with various types of coagulopathy. In this context the authors discuss etiological factors, symptoms, diagnosis as well as possible treatment of acute mesenteric venous thrombosis.

[Comparison of therapeutic results of clinical stage II laryngeal carcinoma treated with various methods and concurrent monitoring changes in cellular and humoral protection].

The authors compare the dynamics of serum glycoproteins concurrently with monitoring of changes of cellular defence in patients with laryngeal carcinoma, IInd clinical stage. In 20 operated patients with subsequent radiotherapy [group A] better results were achieved as well as normalization of glycoproteins in the course of treatment. The immune response has in the early postoperative period a declining trend. Later in the course of radiotherapy the concentration of IgA and IgM globulins rises with the exception of IgG globulins the concentration of which declined below values of the initial concentrations. The activity of T lymphocytes did not change substantially during treatment, contrary to 19 patients treated by radiotherapy only [group B] where T lymphocytes declined by as much as 14.7%. Also the majority of glycoprotein concentrations remain reduced in group B at the end of the investigation [6 months] with a maximum drop after administration of half the radiation dose [with the exception of alpha 2-macroglobulin and the C3 component of complement]. The IgA and IgG globulin concentrations declined, IgM increased partly. The results correlate with the mortality: in the course of three years in group B 9 patients died, while in group A only three patients which suggests that combined treatment [surgical and radiotherapy] offer better chances of cure.

Changes in serum proteins following implantation of pacemakers.

The change in the protein concentration in blood serum was investigated after implantation of a pacing system with epoxide surface (group A, 20 patients) or metallic case (group B, 9 patients). The response to surgical intervention was equal in both groups. In the first period following operation the acute phase reaction took place in terms of increasing concentrations of alpha-globulins, especially alpha 1-acid glycoprotein, alpha 1-antitrypsin, ceruloplasmin, haptoglobin, and beta 1A/C-globulin, Simultaneously, a decrease in prealbumin, albumin, beta-globulin, in particular haemopexin and transferin concentration appeared. As for immunoglobulins, a decrease in IgG and increase in IgA and IgM globulins was observed. In the course of a longer period of time following implantation (group B--three months; group A--two years) a decrease of almost all investigated proteins was observed with exception of increased IgA globulin and transferin in group A.

Effect of inflammation on serum glycoproteins in rabbits.

Quantitative changes in the concentration of rabbit serum proteins and glycoproteins accompanying a non-specific injury (terpentine oil inflammation) were studied by conventional electrophoretic and quantitative crossed immunoelectrophoretic analysis. Most significant changes were observed in the alpha2- and beta-globulin region as well as in some individual glycoproteins (haptoglobin, ceruloplasmin, IgM globulin). In the paper the parallel changes in sugar components of glycoproteins were also recorded.
