[Pseudo-tumoral human African trypanosomiasis due to Trypanosoma gambiense. Clinical and tomodensitometry study (author's transl)].

The authors report an observation of african trypanosomiasis due to Trypanosoma Gambiense, clinical signs included massive and progressive hemiplegia, papillary edema and vascular shift from median line at arteriography. These pseudo tumoral clinical features are unusual in this disease. Asymetrical heterogenous hypodensities of the centrum semioval are dominant in the initial CT scanner aspect. The confrontation of CT scanner images to the clinical and evolutive data suggests the presence of associated cerebral edema and demyelination. With treatment, hypodensities were regressing while images of subcortical atrophy appeared. Lastly, in spite of severe general signs and the importance of neurological deficit, arsenical treatment associated with high doses of corticotherapy lead to a rapid improvement.

[Malignant gliomas in adult. Interest of immediate radiotherapy in the combined protocols : chemotherapy (VM26-CCNU), radiotherapy. A cooperative study on 137 cases (author's transl)].

The results of a multicentric study of three combined therapeutic protocols for malignant gliomas are reported. The goal was the evaluation of "Delayed" versus "Immediate" Radiotherapy. All patients were operated on, the radiotherapy was 5 800 rad. whole brain. The drugs used were : Day 1 - Day 2 Epipodophylotoxine 60 mg/m2/day. Day 3 - Day 4 CCNU 60 mg/m2/day. The mean survival times were : "total" surgery versus "partial" surgery: "total" group : 52 cases - mean survival : 44 weeks ; "partial" group : 60 cases - mean survival : 48 weeks : 1) protocol A : surgery + chemotherapy alone : 60 patients/34 weeks ; 2) protocol B : surgery + radiotherapy + chemotherapy :22 patients/43 weeks ; 3) protocol C : surgery + chemotherapy 4 cycles + radiotherapy + chemotherapy : 51 patients/71 weeks. The percentage of survivors at 12 months was Prot. A : 57% - Prot. B : 32% - Prot. C : 70%. These results are in favor of "Delayed" radiotherapy. They are statistically significant, and prompt us to pursue such a trial for getting confirmation upon numerically broader groups of patients.

[Malignant gliomas treated by combination chemotherapy and delayed radiation therapy (author's transl)].

The authors present the therapeutical results of two clinical and histological homogeneous series of malignant gliomas. The first group includes 46 patients operated upon and treated successively by chemotherapy associating VM26 and CCNU, a conventional delayed radiotherapy 6 months after surgery and again chemotherapy. In this group, the mean survival is 17 months and 46% of patients were surviving at 18 months. The second group includes 28 patients operated upon and treated with the same associated chemotherapy without radiation therapy. The mean survival was 11 months and 24% of patients were surviving at 18 months. The comparison of results shows that after surgery, the association of a chemotherapy and delayed radiotherapy is more efficient than chemotherapy alone. However, the systematic study of nervous system tolerance to irradiation post-chemotherapy suggests that, in some cases, the chemotherapy is able of increasing the pathogenic effect of irradiation on the brain.

[Interest of glioma 26 in the selected chemotherapy of intracranial tumors (author's transl)].

Glioma 26 (G26) is a chemically induced ependymoblastoma of mouse. It was used through sub-cutaneous and/or intracranial injection in 7 experiences which showed that this experimental model was able to select drugs acting on intracranial tumors : nitrosourea (CCNU), VM 26, procarbazine (PCB). Combination chemotherapy (VM 26-CCNU ; VM 26 CCNU-PCB) was more active than monochemotherapy in 2 out of 3 experiences. It seems that the variability of the results is at least partly due to the important tumoral necrosis in tumors treated by combination chemotherapy and to technical problems. The latters can be avoided by the mechanical dispersion of tumor cells and by their injection through a stereotaxic frame. Selection of drugs is easier and cheaper to perform with G 26 than with other in vivo and in vitro experimental models. The main problem for all models remains their lack of specificity.

[Isolation of Escherichia vulneris in drinking water].

Over a 2-year period, we performed 33 bacteriological controls of drinking water supplied by refrigerated fountains located in a nursing home. Amongst 24 strains of gram-negative bacilli isolated from 16 samples. 10 were identified as belonging to the species Escherichia vulneris. Viable bacterial counts were always less than 10 ufc/100 ml. During the same period no clinical isolate of E. vulneris was recovered from the nursing home. The signification of E. vulneris in drinking water is unknown. However, considering that E. vulneris has been implicated as cause of various infections, its presence in potable water supply systems would seem to be a potential risk factor for severely immunocompromised patients.
