[Dilation of stenoses of the oral part of the esophagus in the Kelly-Paterson syndrome (Plummer-Vinson)].

The dilatation of oesophagus oral portion in the Kelly-Paterson (Plummer Vinson) syndrome was made in two female patients at the age of 58 and 81 years. For the extreme stenosis (2 and 3 mm) of the lumen the dilatation was first performed by the Grüntzig Catheter and after extension above 5 mm special oesophageal catheters with a balloon of 15 mm diameter (Cook) were used. The dilatation proved successful in both patients. The dilatation in the K.-P. syndrome must be performed with care beginning with catheters of small diameter and narrow end with a step-wise slow filling of the balloon. A good anesthesia of upper respiratory pathways and swallowing passages is required.

Aged

[Use of the balloon catheter in dilatation of corrosive stenosis of the esophagus].

During the period between October 1985 and the end of March 1988 from a total number of 269 dilatations in 29 patients with benign stenoses of the oesophagus of varying origin and in one patient with malignant stenosis 109 operations were made in 5 patients with corrosive stenoses. Three of these five patients suffered from double stenosis. For dilatation balloon catheters of Cook Co. were used. In the whole group only once a complication was observed, in the group of patients with corrosive stenoses no complication developed.

Adult

[Dilatation of malignant esophageal stenoses using a balloon catheter].

The paper summarizes the experience with the dilatation of malignant oesophageal stenosis by a balloon catheter. In a patient after gastrectomy with a relapse of the tumorous process the author performed 10 dilatation interventions by a balloon 22 mm wide. After a short interval of renewed normal passage, marked swallowing difficulties always rapidly returned. The patient died 24 hours after a subsequent surgical revision and an attempt to remove the tumour.

Catheterization

[Suggestions for a rational approach to dilatation of benign esophageal stenoses using balloon catheterization].

The paper summarizes the experience in performing dilatation of benign stanoses of oesophagus of different origin. In the period from October 1985 through September 1987, 215 dilatation interventions were performed in 25 patients. The paper establishes necessary principles before the dilatation, evaluates the course and effect of dilatation in relation to the technique used and outlines a recommendation for rational procedure for safe execution of the therapeutic procedure.

Catheterization

[Multiple capillary aneurysms].

Recurrent eruptions of small, black nodules resembling metastases of malignant melanoma in a 64-year-old male patient with diabetes mellitus are reported. Clinically, the lesions seemed to be angiomas. Histological examination revealed capillary aneurysms. To date there has been no spontaneous regression. Sclerosing injections or compression therapy had no effect. Only total excision of lesions was successful. The etiology of the disease is unknown. The patient had diabetic retinopathy with increased plasma levels of cholesterol.
