HPLC-UV/PAD and HPLC-MS(n) analyses of leaf and root extracts of Vismia guineensis and isolation and identification of two new bianthrones.

In order to evaluate the possible use of the leaves instead of the roots of Vismia guineensis as a new source for the traditional use of this drug, the chemical composition of both organs were compared by HPLC-UV/PAD and HPLC-MS analyses. The leaves are analysed here for the first time. The results show the presence of five major classes of secondary metabolites having specific chromophores: anthraquinones, vismiones, flavonoids, xanthones and benzophenones. The molecular weights and characteristic fragments, compared with previous EI or HPLC-MS literature data, allowed the partial identification of the major peaks in the chromatograms. Six additional isomeric bianthrones and one anthraquinone were detected in the dichloromethane extract of the roots after long storage in solution; the targeted isolation of the bianthrones was performed and enabled the identification of two original C-geranyl derivatives. The chemical compositions of the extracts demonstrated that only a minority of the constituents is shared by both organs. Thus, in order to establish a definitive phytoequivalence, additional pharmacological investigations are required.

[Percutaneous maxillary nerve block anesthesia in maxillofacial surgery].

BACKGROUND: Personal experience in percutaneous maxillary nerve block anesthesia in association with transmucosal anesthesia of the sphenopalatine ganglion in oral and maxillofacial surgery, is presented. METHODS: Six Caldwell-Luc, 9 anthrotomies and biopsies of maxillary sinus, 8 removals of extensive odontogenic cysts and 12 surgical maxillary expansions were performed from 1994 to 1996 at our Department. Maxillary transcutaneous nerve block in association with transmucosal anesthesia of the sphenopalatine ganglion were performed. Carbocaine without adrenaline in association with NaCO3 1/10 for maxillary nerve block anesthesia and lidocaineoprilocaine cream (EMLA) for transmucosal anesthesia were employed. Intra- and post-operative pain were evaluated by visual analogue scale in all the patients. RESULTS: Anesthesiological procedures revealed to be effective in all surgical interventions and postoperative analgesia allowed easier pain control. CONCLUSIONS: The simplicity of execution, the effective pre- and postoperative anesthesia and the absence of side effects make this procedure particularly indicated in oral and maxillofacial surgery.

Endodontic, surgical and periodontal treatment of dens invaginatus. Case report.

The aim of this paper is to propose a single stage global treatment of endodontic, periapical and periodontal lesions in a lateral maxillary incisor with dens invaginatus. A 24 year-old woman presenting a lateral maxillary incisor with dens invaginatus in association with periapica1 and periodontal lesions underwent simultaneous surgical, endodontic and periodontal regenerative procedures. At 2, 6, 12, 18 months follow-up the radiographic healing appeared to be improved and the periapical lesion healed completely 1 year after surgical intervention. Surgery in association with endodontic and periodontal procedures represents the treatment of choice to maximize long term prognosis in cases of dens invaginatus with chronic periapical and periodontal lesions.
