[Pourfour du Petit syndrome and intracranial aneurysms (author's transl)].

A 52 year old female suffered for more than 20 years from a recurrent pulsating headache on the left side. Clinical examination showed the retraction of the superior eyelid and mydriasis on the left. Exophthalmometry revealed 17 mm of protrusion for each eyeball. Angiographic investigations showed two little aneurysms: one of the anterior comunicating artery, the second of the left middle cerebral artery. In view of the clinical and angiographic findings the authors discuss the pathogenetic mechanisms of such oculo-sympthetic syndrome.

[Epidemiological observations on plasma levels of diphenylhydantoin in 130 patients on prolonged treatment (author's tranls)].

Plasma levels of diphenylhydantoin have been measured in 130 epileptic patients undergoing long-term anticonvulsant therapy. Only in a small percentage of the patients (15,38%) plasma levels were found to lie within the therapeutic range. Most patients (75,38%) exhibited subtherapeutic values, while in a few cases (9,24%) the upper limit of therapeutic range was exceeded. Although plasma concentrations significantly correlate with the administered dose, predictability of plasma levels in the individual patients given fixed dosage schedules is strongly limited by large interindividual variability. Sex, age and simultaneous administration of other drugs seem not to affect significantly DPH plasma levels. Our observations carried out in a meaningful sample of epileptic patients suggest that at present the practice of monitoring diphenylhydantoin plasma levels is still an essential tool for the management of epilepsy when a safe and effective therapeutic regimen is required.

Microheterogeneity of CSF alpha 2-macroglobulin in multiple sclerosis and other neurological diseases. A study performed by direct immunofixation after isoelectric focusing.

Cerebrospinal Fluid (CSF) and serum alpha 2-Macroglobulin (alpha 2M) was studied in patients affected by Multiple Sclerosis (MS) and other neurological diseases (OND) using Isoelectric Focusing followed by direct immunofixation. In the serum a simple broad band has always been observed; inside it several more stained subfractions were sometimes detected. In 74.3% of MS patients and in 65.7% of OND patients the CSF alpha 2-M was divided in two major bands. One corresponded to that observed in serum and the other was more anodal. The significance of this finding is discussed.

The quantitative parameters of intrathecal IgG synthesis. A comparison.

Cerebrospinal fluid IgG/Albumin, IgG Index, Tourtellotte's formula and Schuller's formula have been calculated in 258 patients with definite Multiple Sclerosis and oligoclonal bands at isoelectrofocusing examination of cerebrospinal fluid. A step wise discriminant analysis was performed on single parameters and on a combination of IgG Index, Tourtellotte's and Schuller's formulas. The latter showed the highest percentage of right classification.

Intrathecal methylprednisolone acetate in multiple sclerosis treatment. Clinical evaluation.

Twenty-eight patients with clinically definite Multiple Sclerosis were treated with intrathecal injections of Methylprednisolone acetate, 40 mg at four days intervals. Kurtzke's Disability Status Scale was evaluated in all the patients together with sum of scores for the neurological functions and the subjective assessment, before and after the treatment. Ten Multiple Sclerosis patients, treated with prednisone (100 mg at alternate days) were taken as a control group. No real advantages were found with the intrathecal treatment as compared to the orally administered steroids.

[Relation between the clinical and cerebrospinal fluid parameters in multiple sclerosis, with special reference to subjects without oligoclonal IgG in the cerebrospinal fluid].

120 patients with clinically definite or probable Multiple Sclerosis (MS) were studied by Isoelectric focusing of cerebrospinal fluid and serum proteins. Of the 62 patients with definite MS, 56 (90%) had IgG oligoclonal bands only in CSF, while in the group with probable MS 44 patients over 58 (75,8%) showed the same finding. The group of 20 patients with normal IgG profile has been compared with a group of 22 patients selected by random out of the 100 with IgG oligoclonal bands. No statistically significant difference was discovered between these groups but a tendency to present an higher age at onset and a longer duration of the disease in the group with a normal IgG pattern than in the patients with oligoclonal bands was pointed out.

[Clinical and CT findings in ischaemic cerebrovascular diseases (author's transl)].

120 patients with an ischaemic stroke visualized by computerized tomography were investigated: the clinical examination of the patients (double-blind study) showed a wrong diagnosis in 15,8%, the topographic diagnosis was correct in a much higher percentage (81%). Possible relationships between clinical severity, CT localization and extension of the damage were investigated; a statistically significant relationship was ascertained only in the group of 38 cases who were examined within 21 days from the stroke. The site and extension of the lesion did not seem to influence the clinical evolution or to be a good prognostic indicator in this study.

[(Negative) CT findings in subjects with ischaemic strokes (author's transl)].

50 subjects who had one or several ischaemic attacks without detectable attenuation area in the CT scan were examined. CT findings grouped as follows: normal, cortical generalized atrophy and cortical localized atrophy with or without ventricular or cysternal enlargement, diffuse cortical and ventricular atrophy. The relationships between CT scan, clinical evolution, number of ischaemic attacks and predisposing risk factors were also considered. The mainly morphological value of CT and the limited dynamic and functional aspects of this technique are emphasized in agreement with other authors.

[CT findings in chronic cerebrovascular insufficiency (author's transl)].

CT examination in 100 patients affected by chronic cerebrovascular insufficiency showed a normal picture in 16 cases, while 84 subjects showed different degrees of atrophy, generalised and focal, controlateral or sometime omolateral to the infarct, with differing patterns. The mean age of subjects with normal CT was significantly lower in comparison with patients with definite atrophy. Besides the neurological picture and the time course, the patients' psychiatric status was also assessed: no definite relationship seems to exist between psychiatric disturbances and brain atrophy. Pseudobulbar syndrome was found in 46 cases and many of them showed cortical and mainly ventricular atrophy. This picture was very seldom found in patients with vertebro-basilar insufficiency. Localised cortical atrophy, on the contrary, was often associated with ischaemic stroke. The incidence of predisposing risk factors (hypertension, diabetes mellitus or high lipid levels) was greater in subjects with brain atrophy and older age than in patients with normal CT scan.
