[Pulmonary amyloidosis--own experience].

Three cases of amyloidosis were described. In all diagnosis was confirmed by histological examination. There was amyloidosis limited to the lungs in 2 cases and in 1 generalised. In 1 patient lobectomia was performed. Next 2 pts were treated with prednisone and cytostatic drugs (melphalane and cyclophosphamide).

[The relation between lymphocyte profile in BALF and the course of sarcoidosis based on short term observation].

The aim of this study was to assess how the extent of the number and percentage of lymphocytes in BALF and also the CD4 to CD8 ratio can help to predict the short outcome in sarcoidosis. Material consisted of 74 patients, 39 men and 35 women in the age from 23 to 58 years. 11 patients had chest lesions in stage I, 43 in stage II and 20 in stage III. Clinical markers of activity (fever, erythema nodosum) were present in 22 cases. Extrathoracic lesion were present in 31 and abnormal pulmonary function in 30. In all patients BAL was done before treatment and lymphocyte count, percentage and CD4/CD8 ratio was calculated. 50 patients were treated with corticosteroids and 24 were observed without treatment. After 6-12 month of observation regression of sarcoid lesions was observed in 46 of 50 patients treated with corticosteroids and in 17 out of 24 patients observed without treatment. There were no differences in lymphocyte count and percentage in BALF and in the short term outcome between group treated with corticosteroids and without treatment. The patients in whom regression of lesions was observed have however significantly higher CD4/CD8 ratio than others.

[Initial experience with a new type of high energy neodymium laser in correcting stenosis of the airway].

The aim of the study was to assess the use of a new Nd:YAG laser--mediLas fibertom--in correcting malignant and benign stenoses of the airways. Eighteen patients were included in the study. In 9 these were caused by malignant tumors, in 3 by nonmalignant tumors, in 6 by post-inflammatory stenoses of the airways. Altogether 99 laser sessions were carried out. In patients with malignant diseases complete recanalization was achieved in 6 patients, in 2 partial. One patients did not benefit form the laser procedure. In patients with benign tumors complete recanalization was achieved in all of the patients. In 5 out of 6 patients the post inflammatory stenosis was corrected. Satisfactory results observed after the second, fourth and one year after the initial laser procedure encourage to use this from of therapy in cases of malignant and nonmalignant narrowing of the airways.

[Use of fiberoptic bronchoscopy for removal of foreign bodies from the lower respiratory tract].

One of the initial indications for therapeutic bronchoscopy was removal of an aspirated foreign body. The aim of this study was to evaluate the role of bronchofiberoscopy in removal of aspirated foreign bodies. Thirty patients were included in the study (age range 4-81 years). Endoscopy was performed under local anesthesia in two sessions. During the first a complete evaluation of the tracheobronchial tree was carried out, while during the second the actual removal was performed. In the majority of the patients (23) the foreign body was aspirated into the right lung. In the majority of the patients (13) various fragments of bones were recovered. Only in one patient a major complication was observed (hemorrhage) which compelled to change from bronchofiberoscopy to rigid endoscopy. All complaints from patients subsided during 2-4 days after removal of the foreign body. Foreign bodies were removed using a bronchofiberoscope in 21 out of the 28 patients. In two patients a calcified lymph node could not be removed during bronchoscopy and the patients were referred to the Surgical Department for thoracotomy.

[Bronchial tuberculosis in bronchoscopic material].

A retrospective analysis of 14,461 patients examined at the Bronchological Unit in the years 1978-1988 was carried out in order to assess the incidence of bronchial tuberculosis confirmed histopathologically. 46 cases met the selection criteria (19 females, age range 21-75 years, mean 56.2 +/- 18.8. years, 27 males, age range 21-85 yrs, mean 50.0 +/- 17.4 yrs). Only in 22 patients acid fast bacilli were cultured. Analysis of endoscopical changes was carried out according to the Jaroszewicz-Krakówka classification. Most often diffuse inflammatory changes were seen.

[Severe steroid-induced myopathy].

In the presented case after 3 years and 3 months treatment with DIPROPHOS (betamethasone) a transient tertraplagy developed. Also disturbances in respiratory muscle and peripheral nerve function were present.
