The mitochondrial respiration rate and morphometric indices in endomyocardial biopsy samples were measured in 43 patients with dilated cardiomyopathy selected in accordance to WHO criteria by endomyocardial biopsy studies after excluding of various forms of myocarditis, alcoholic cardiomyopathy and other specific diseases of the heart. A group of 13 patients with unusually high mean myocyte diameter, 30 +/- 4 microns, and nuclear size, 57 +/- 5 microns, was selected. The remainder of patients (n = 30) had significantly lower mean myocyte diameter and nuclear size, 23 +/- 3 and 42 +/- 6 microns, respectively, (p < 0.01). Creatine-stimulated elevation in mitochondrial respiration rate as measured in saponin-skinned fibers was found in the former group to be much lower (36 +/- 4%) as compared with the remainder (90 +/- 12%). Also, the former group of patients had higher left ventricular enddiastolic pressure and volume index with concomitantly decreased ejection fraction. The results indicate that marked nuclear and cellular hypertrophy is associated with lower creatine-stimulated mitochondrial respiration rate and more severe cardiac failure. They suggest that disorders in energy supply to myofibrils may be related to disturbances in cellular genetic apparatus.
The present study was undertaken to compare the ability of magnetic resonance imaging (MRI) and transrectal ultrasound (TRUS) to identify the presence or absence of extracapsular tumor extension and seminal vesicle involvement in patients with clinically localized (stages A and B, T1-T2) prostatic cancer. Sixty-four patients (ages 44-77 years) underwent radical retropubic prostatectomy and pelvic lymphadenectomy after staging with both MRI and TRUS. Surgical specimens were step-sectioned and examined at multiple levels. Radiographic staging was correlated with pathological staging. Both MRI and TRUS demonstrated better staging accuracy than conventional digital rectal examination [42% for digital rectal examination (DRE), 63% by TRUS, and 67% by MRI], although they suffered from understaging (31% by TRUS and 22% by MRI). The positive predictive value (PPV) for stage B disease was disappointingly low (42% for DRE, 53% for TRUS and 59% for MRI). However, the PPV for stage C disease was higher (81% for TRUS and 77% for MRI). In the evaluation of extracapsular invasion, the PPV of TRUS (81%) was higher than that of MRI (77%). The negative predictive value (NPV), however, for both TRUS and MRI was low (58 and 56%, respectively). In the evaluation of seminal vesicle invasion, both TRUS and MRI demonstrated poor PPV (50 and 40%, respectively), but the NPV was excellent (90% for TRUS and 96% for MRI). MRI failed to detect metastatic lymph nodes in 4 of 6 patients while falsely predicting their presence in 4 patients.(ABSTRACT TRUNCATED AT 250 WORDS)
Since potassium ions are very important in surgical pathology and the course of the postoperative period, the authors determined erythrocyte potassium in 211 patients (37 in the control group and in 174 in the pathological group). The statistically evaluated results were compared for both groups and the correlations of serum and red cell potassium with the acid-base status were studied. The authors used a Marongiu's modified method of potassium determination in a erythrocyte hemolysate with a flame photometer. The mean values for both the control and pathological groups were 84.61 (SD 5.24) and 85.98 (SD 9.10), respectively. The correlation of serum and erythrocyte potassium is subject to many influences and occurs only in the limited pH and HCO3 intervals (pH 7.4 +/- 0.5 and HCO3 21 +/- 2.0) so that it is insignificant even for the control group. If a concentration exceeding two standard deviations is considered abnormal and a deficit percentage is calculated, it is possible to approximate the total body deficit from the expected values of 50-55 mM/kg and to start proper replacement. In evaluating the results, precise laboratory work must be performed and the clinical condition must be taken into account. Therefore, the method of erythrocyte potassium determination is not suitable for routine work and is used only in case of serum potassium excess values and during prolonged postoperative parenteral treatment.
If your home care organization is preparing for a Joint Commission survey, you're probably very concerned about complying with the standards. The intent of meeting standards is not only to receive accreditation but also to improve patient care. And, as you will find, some of the standards are more difficult to comply with than others. This article identifies the 10 standards in patient-focused functions with which home care organizations had the most difficulty complying in 1995. These are the standards for which home care organizations most often received scores of 3 (partial compliance), 4 (minimal compliance), and 5 (noncompliance). The findings are from home care surveys that were conducted between January 1 and July 1 of 1995 in which the 1995 Accreditation Manual for Home Care was used. This manual will be used for surveys conducted through all of 1996.

This study was designed to illustrate how Q Methodology can be used as a tool for strategic planning. Potential plans for the future of a small Indiana hospital were formulated and Q sorted to determine support or resistance by key leaders from within hospital management, the board, and the medical staff. The hospital was able to identify stakeholder perceptions that resulted in strong consensus that integration should be a priority for the hospital. This exercise provided a list of objectives for hospital leadership and the results were also used to justify the cessation of several programs that the hospital leadership had been pursuing.
